The results have indicated that older patients, deeper impaction status, and the 5 radiographic superimposition signs are significant risk factors for IAN injury. In the absence of specific radiographic signs, the risk of neurosensory deficit of the IAN might be negligible.
Kim et al. Risk Factors Associated With Neurosensory Deficits. J Oral Maxillofac Surg 201212842 patients!

RESULTS OF IAN repair surgery: POGREL 2013
• reports are few• heterogeneous patient demographics
• timing of repair is different
• success is measured in different ways
• IF repair within 2‐3 months: 50% of patients recover over 50% of sensation
• IF repair > 2‐3 months: results less satisfactory
• Interpositional grafts < direct anastomosis
• NO improvement of “pain”
Spontaneous healing of IAN lesions after third molar surgery (white) vs other surgery (grey)
Hillerup 2007
RESULTS OF LN repair surgery: Ziccardi, Cornelius 2013
• injury‐surgery INTERVAL is MOST critical indicator (timing of repair !)
• tension free approximation
• before neuropathic pain installs
• “Waiting for diagnostic certainty that spontaneous nerve recovery will not occur may preclude a successful nerve repair and condemn the patient to a permanent neurosensory deficit”
TIME‐LINE LN REPAIR
• vermoeden van iatrogene transsectie, goed localiseerbaar
– onmiddellijk herstel of binnen enkele dagen
• Pogrel: kritische tijdgens:
– 10 weken• Strauss: kritische tijdgrens:
– 12 weken• Bagheri:
• Pogrel: 10 weken
• Rutner: 12 weken
• Susarla: 90 dagen
• Hillerup (oudere studie): 6 maanden
• Bagheri: max. 6 maanden
• Bagheri:

• external or internal NEUROLYSIS
• re‐anastomosis, tension‐free
• nerve graft– allogeneic– autogenous
• vein graft
• nerve conduit– resorbable
• polyglycoid acid (Neurotube)
• polyester ( Neurolac)• collagen type 1 (NeuraGen, Neuroflex, Neuromatrix)
– non‐resorbable (Gore‐ Tex, Silicone)
lingual nerve anatomy
34 cadaver dissections• 17.6% LN at the level of the
alveolar crest or higher. • 62% horizontally the lingual
nerve in contact with lingual bone plate of the third molar
• 8,8% LN only 0,5 mm thick at that place
256 cases of third molar extraction• 4.6% LN visualized above the
height of the lingual plate of lower third molar

complications of trigeminal nerve repair
• worsening of neurosensory deficit• more surgery than necessary
• donor site morbidity
• (neuropathic pain)
neuropathic pain
• neuropathic pain, when present, will NOT improve by surgical trigeminal nerve repair
BESLUIT: microchirurgie van LN ‐ IAN
• énkel indien sensorieel deficit ‘ondragelijk’ is• afwezigheid van “visualisatie” van anatomisch defect is
problematisch
