CASE STUDY
Identification data
• ผู้ป่วยเด็กชำยอำยุ 1 เดือน 14 วัน
• ภูมิล ำเนำ อ.แม่จริม จ.น่ำน
• Admission date : 14 พฤษภำคม 2555
• แหล่งท่ีมำข้อมูล : จำกบิดำ มำรดำและเอกสำรทำงกำรแพทย์
Chief Complaint
• หำยใจเร็วมำกขึ้น 1 วันก่อนมำโรงพยำบำล
Present Illness
• 10 วันก่อนมำโรงพยำบำล ผู้ป่วยมีอำกำรแหวะนมเล็กน้อย ทุกครั้งที่ทำนนม
• 3 วันก่อนมำโรงพยำบำล ผู้ป่วยมีอำกำรร้องกวนบ่อยครั้ง ร่วมกับทำนนมได้ลดลง จำกเดิมประมำณ 20 - 30 นำที เหลือประมำณ 5-10 นำที หลับบ่อยขึ้น มีอำกำรแหวะนมที่ทำนอยู่ ไม่มีไข้ ขับถ่ำยปกติดี
Present Illness
• 1 วันก่อนมำโรงพยำบำล ผู้ป่วยมีไข้ต่ ำ ๆ ร่วมกับเวลำร้องไห้บิดำสังเกตว่ำจะมีใบหน้ำและแขนขำสีคล้ ำขึ้น หลังหยุดร้องอำกำรดังกล่ำวหำยไป แต่ยังมีอำกำรหำยใจเร็วอยู่ ถ่ำยอุจจำระสีเหลืองประมำณ 2 ครั้ง ปริมำณน้อย ไม่มีมูกเลือด จึงไปรักษำที่ รพ. แม่จริม

• ที่โรงพยำบำลแม่จรมิ
ผู้ป่วยมีถ่ำยเหลว ประมำณ 4 ครั้งสีเหลือง ไม่มีมูกเลือดปน ร่วมกับมีไข้ (38OC) มีอำกำรหำยใจเร็ว
Dx Infectious Diarrhea
Management (Admission day)
• Ampicillin 125 mg v q 6 hr
• Gentamicin 25 mg v OD
• IV fluid MT + 6% Deficit
Present Illness (Cont.)
• เช้ำวันท่ีส่งตัว ผู้ป่วยหำยใจหอบเหน่ือยมำกขึ้น ร่วมกับระดับออกซิเจนในเลือดลดต่ ำลง จึงส่งตัวมำรับกำรรักษำต่อที่โรงพยำบำลน่ำน
• RR 80/min Temp 38 Pulse 140
Sp02 Room air 80% --> 95% on O2 box
Looked dyspnea, tachypnea, Marked subcostal retraction, Mottling skin
Past History
• No underlying disease
• No current medication
• No history of drug and food allergy
• No history of admission

• ปี 2535 N/D ท่ีบ้ำน เพศ หญิง – เสียชีวิตเม่ืออำยุได้ 3 เดือน
• ปี 2540 N/D ท่ีบ้ำน เพศ หญิง – สุขภำพแข็งแรงสมบูรณ์
• ปี 2543 N/D ท่ีบ้ำน เพศ หญิง – สุขภำพแข็งแรงสมบูรณ์
• ปี 2546 N/D รพ.แม่จริม เพศ ชำย – เสียชีวิตเม่ืออำยุได้ 4 เดือน
• ปี 2548 N/D ท่ีบ้ำน เพศ ชำย – สุขภำพแข็งแรงสมบูรณ์
• ปี 2551 N/D รพ.แม่จริม เพศ ชำย – สุขภำพแข็งแรงสมบูรณ์
Antenatal History
• First ANC : 1 พฤศจิกำยน 2554
– GA 20 wk by LMP
• ANC ทั้งหมด 10 ครั้ง
– 28 กุมภำพันธ์ 2555 : Curd-liked discharge
– Dx: Candida vaginitis Rx: Clotrimazole Vg x 7 days
• น้ ำหนัก 41 กิโลกรัมส่วนสูง 142 ซม.
Antenatal History
• ผลตรวจทำงห้องปฏิบัติกำร
– VDRL : Non-Reactive 2 ครั้ง
– HBsAg : Positive
– HIV : Non-reactive 2 ครั้ง
– Hct : Normal (30 and 31 %)
– Thalassemia :
• มำรดำ OFT = Positive DCIP = Positive
• บิดำ Normal
Perinatal History
• Birth Date : 30 มีนำคม 2555
N/D รพ.แม่จริม
• GA 39+3 wk by ultrasound
• Birth Weight : 3,070 grams
Height : 48 cm.
HC : 34 cm.
• Apgar 8 -> 10
• หลังคลอด : no anomaly, no jaundice
Postnatal History
• Growth and Development : ปกติดี
• Vaccination :
– BCG + HBV แรกเกิด + 1 เดือน
– Vitamin K

A boy looked dyspnea with marked subcostal retraction on ET tube No.4

mild pale conjunctivae, no icteric sclerae, Lymph node can’t be palpated
• Lungs
decrease breath sound left lungs
marked subcostal retraction
• Heart
tachycardia, no murmur

soft, no guarding, hepatomegaly 6 cm below right costal margin
• Extremity
no edema, capillary refill < 2 cm, no skin turgor

• Ampicillin (100 MKD) 130 mg v q 6 hr

• No Fever, Dyspnea improve
• Lung : clear
• Heart : no murmur
• Liver span 2 cm below RCM
• CMV -> SIMV mode

– Folic acid 1x2

• Hemoculture -> no growth in 5 day
• Sputum Culture -> no growth in 2 day
• Urine Culture -> no growth in 2 day
Progression
• Off ET tube (19/5/55)
• Off Antibiotic (20/5/55)
• Discharge 22/5/55
– Home Med : Vitamin B complex 1x1 # 90(for mother)
THIAMINE DEFICIENCY
Vitamin B1 Thiamine
• Water soluble vitamin
• Thiamine pyrophosphate (TPP) : Active form of the vitamin
• Essential Coenzyme in carbohydrate metabolism
• RDA : 1.5 mg
Effect of thiamine deficiency
• Lack of ATP
• Decrease acetylcholine synthesis
• Effect to Cardiovascular System(Wet beriberi)
– Decrease myocardial contractility
– Massive capillary dilatation in skeletal muscle
• High output cardiac failure
Effect of thiamine deficiency
• Lack of ATP
• Decrease acetylcholine synthesis
• Effect to Neurological System (Dry beriberi)
– Peripheral neuropathy
CARDIAC BERIBERI
Clinical Manifestation
Infantile beriberi
Juvenile beriberi
Adult or Adolescent beriberi
Infantile Beriberi
• Age < 2 years old (Usually 2-3 month)
• When suspected in Infantile beriberi
– Sign of Thiamine deficiency in mother
– Child with subclinical thiamine deficiency prior came with fever
Infantile Beriberi
• Sign and Symptoms
– Irritable, Anorexia
– Biventricular failure
• Right side heart failure
– Signs of low output stage
– If Severe (Shoshin)
• Pallor, Peripheral cyanosis, cool extremities, severe metabolic acidosis

Investigation
• Chest x-ray
– global dilation or RV dilation with pulmonary vascular congestion
• EKG
– RVH with strain and Right axis deviation
– Prolong QT interval, low voltage, abnormal T wave (Flat or invert)

• Increase serum lactate and pyruvate
• Decrease urine thiamine excretion
• Decrease erythrocyte transketolase (ETK) activity and increase Thiamine pyrophosphate (TPP) effect

Therapeutic Diagnosis
• Treat with Thiamine intravenous and follow clinical response
– Diuresis
– Transient decreased heart rate and increased blood pressure
– CHF symptoms improved
– CXR may decrease pulmonary congestion early
Management
• Thiamine hydrochloride(B1) 25-50 mg IM,IV
– infant : 7-14 day then
– thiamine or B 1-6-12 at least 1 month
• In severe case
– diuretic and inotropic drug
