Head injury patientหมายถึง ผูป่้วยทมีีภาวะเหล่านีขอ้ใดขอ้หนงึหรือหลายขอ้ก็ได้
มีประวตัิแน่นอนว่าศีรษะถูกกระทบ
ตรวจพบมีบาดแผลฉีกขาดทีหนงัศีรษะหรือหนา้ผาก
มีการเปลียนแปลงความรูส้กึตวัหลงัจากอบุตัิเหตแุมเ้พียงชวัขณะ
ไม่รวมถึง facial laceration, fracture, foreign body, epistaxis

1. Diagnosis
1. Irremediable and irreversible structural brain damage
2. Coma นี ไม่ได้เกดิจาก
1. 1 ยา
1.2 Hypothermia (<32 C)
1.3 Endocrine and metabolic disturbance
1.4 Shock

4. ทําการตรวจยืนยันโดย
4.1 No spontaneous movement ยกเว้น spinal reflex
4.2 Absence of brain stem reflexes 6 ประการ ต่อไปนีคือ
1) Pupillary reflex
2) Corneal reflex
3) Motor response within the cranial nerve distribution
4) Oculocephalic reflex (Doll’s eye phenomena)
5) Vestibular response to caloric stimulation
6) Gag and cough reflex
Brain Death
Diagnosis
ข้อ 4.1 - 4.2 ต้องไม่เปลียนแปลงในเวลา 6 ชัวโมง
ยกเว้นเด็กอายุ 7 วัน - 2 เดือน ใช้ 48 ชัวโมง, อายุ 2 เดือน - 1 ปี ใช้ 24 ชัวโมง
1. ทําการตรวจยืนยันโดย
4.3 Apnea test 10 นาที โดย PaCO2 > 60 mmHg. หรือเปลียนแปลงมากขึน > 20
mmHg
4.4 สภาวะการตรวจพบในข้อ 4.1, 4.2 และ 4.3 นี จะต้องไม่มีการเปลยีนแปลงเป็นเวลา
อย่างน้อย 6 ชัวโมง จึงจะถือได้ว่าสมองตาย
5. ถ้าไม่สามารถทํา Apnea test ได้ อาจใช้ cerebral angiography หรือ isotope brain scan
6. ทารก 7 วัน - 2 ด. EEG 2 ครัง ใน 48 ชม., ทารก 2 เดือน - 1 ปี EEG 2 ครัง ใน 24 ชม.
7. เวลาการตายทีลงในหนังสือรับรองการตาย คือการเจาะ ABG ครังที 2 ในการตรวจ apnea
test
Brain Deathวิธีการปฏิบัติในการวินิจฉัยสมองตาย
1. การวนิิจฉยัสมองตายตอ้งกระทาํโดยองคค์ณะของแพทยไ์ม่นอ้ยกว่า 3 คน
2. องคค์ณะของแพทยผ์ูว้ินิจฉยัสมองตาย ตอ้งไม่ประกอบดว้ยแพทยผ์ูก้ระทาํการผา่ตดั
ปลูก ถ่ายอวยัวะรายนนั
3. ผูอ้าํนวยการโรงพยาบาล หรือผูไ้ดร้ับมอบหมายเป็นลายลกัษณ์อกัษรจะตอ้งร่วมเป็นผู ้
รับรองการวินิจฉยัสมองตายและเป็นผูล้งนามรับรองการตาย
