Important Information:1. Application can only be made online. There is no paper application.2. Candidates paying fee through cash at Axis Bank/Allahabad Bank can fill online application only after

5. Candidates are required to assess their eligibility, as given in the eligibility section of thisinformation brochure, themselves before applying. The examining body will not assess theireligibility. The eligibility will be checked from original documents at the time of

counselling/admission and candidates found not eligible will be denied admission for which

they themselves will be responsible.6.Last date for receiving application online Monday, January 31, 2011 upto 12 Noon. 7. A candidate may download the pdf copy of the brochure online or may obtain a printed copy of the

brochure by sending a self addressed A4 size envelope with Rs. 70/- postage affixed on it, to theCoordinator. The brochure does not contain application form. Application form is only online.

Important Note regarding photograph

Photograph must be taken with a placard in front of the candidate carrying the

name of the candidate and the date of taking photograph. The name and date on thephotograph must be clear and legible. The photograph should not be modified in any

way after it has been taken. If the Examining body or the invigilator, or the centre

superintendent, or the Counselling Board and/or the University/College (at any

stage) considers that the photograph has been manipulated in any way or does not

match the candidate the candidature/admission of the candidate will be summarily

rejected and no representation will be entertained.

General Information1.Examining body: As Per Government of Uttar Pradesh Order No. 3259/71-3-10-79/2010 dated

24.11.2010, C.S.M. Medical University, Lucknow shall be conducting the U.P. Post GraduateEntrance Examination, 2011 to select the candidates for admission to the State quota seats of PostGraduate M.D./M.S./M.D.S./ P.G. ELIGIBILITY FOR ADMISSION:

(i) Candidates who have passed MBBS/BDS course from the State Medical Colleges of Agra,Allahabad, Gorakhpur, Jhansi, Kanpur, Meerut, Medical/Dental Faculty of CSM MedicalUniversity, Lucknow and recognized Private Medical/ Dental Colleges of U.P. are eligible to

appear in the entrance examination.(vide G.O. No 09/71-03-02-104/2001 Dated 22.01.2002)(ii) Candidates who have passed MBBS from Himalayan Institute of Medical Sciences, Dehradun,

and are bonafide residents of U.P. and selected through CPMT-2000 conducted by UndividedUttar Pradesh are also eligible. (Vide G.O. No 1202/71-03-04-sa-64/2000 dated 11.08.2004 andG.O. No.4917/71-03-04 sa -64/2000 dated 20.03.2007). Candidates who have passed MBBS fromthe Himalayan Institute of Medical Sciences, Dehradun, and are bone fide students of U.P. butselected after year 2000 in that Institute are not eligible.

(iii) Candidates must have completed or completing their Compulsory Rotatory Internship by 30 thMay, 2011.

(iv) A candidate who has already taken admission on the basis of earlier U.P.P.G.M.E.E/A.I.P.G.E.E.is not eligible to appear in the examination until he/she completes and passes the course wherehe/she is presently admitted. However, such a candidate shall be eligible if-

(a) he/she has resigned from the said course, his/her resignation has been accepted by thePrincipal of the Medical/Dental college or the Vice Chancellor in the case ofcandidates of CSM Medical University and he/she has refunded the full amount ofsalary/stipend received by him/her during the said course, before the date ofnotification of this examination, ie. 13.1.2011

(b) he/she is presently pursuing P.G. Diploma course in any subject, with the conditionthat he/she will be considered for the Postgraduate Degree course in that subject only.

APPLYING: Application can only be made online.Important Information about online application:1.1You will get an opportunity to print out a copy of your application, please print and retain it.1.2Once the application is filled up, no change can be made nor will be entertained. No

correspondence/communication for any correction/modification will be entertained in respect to theapplication under any circumstances.

1.3 Incomplete applications will be rejected and no correspondence in this regard will be entertained.

1.4 If you wish to submit a revised application you may submit a new application. Tick theappropriate box and fill the number of the original application in the revised application. Your originalapplication will be cancelled. No fee credit of the original application will be given. Fresh fees willhave to be paid for the revised application. No candidate should submit more than one validapplication. If you do so it will entail rejection of both applications.

1.5No documents should be sent to the Examining body.1.6The serial number of the application must be mentioned in all future correspondence.

(b) a receipt of cash deposit of Rs 1220.00 (in the account number 911020000493881 of UPPGMEE- 2011) at a designated branch of Axis Bank with a Unique Transaction Id, or(c) a receipt of cash deposit of Rs 1200.00 (in the account number 50043097590 of UPPGMEE2011) at a designated branch of Allahabad Bank with a Journal Number.

2.2.5 A .jpg file of your photograph taken with a 4MP (or more) camera. File size should not biggerthan 1MB. For instructions to be given to your Photographer.

2.2.6 A scanned (resolution 300 dpi) .jpg file of your signature. File size should not be bigger than 512KB.

2.2.7 A scanned .jpg file ofany one of the following documents to prove your identity (Not more than 1MB in size):

2.2.7.1PAN Card2.2.7.2A certificate from the Bank Manager where you hold your account certifying your account

number, your signature and photograph. The Verification should be in such a manner that neitherthe signature of the Bank Manager nor the seal should cover the face of the candidate. Candidateswith defaced photograph or signature on Bank Managers certificate will be denied admission tothe examination hall/the counselling, as the case may be.Download the format by clicking here.

2.2.7.3A certificate from the Principal/Dean of the College/University from where you did yourMBBS/BDS certifying your Name, photograph, and signature. The Verification should be in sucha manner that neither the signature of the Principal/Dean nor the seal should cover the face of thecandidate. Candidates with defaced photograph or signature on Principal/Deans certificate will bedenied admission to the examination hall/the counselling, as the case may be. 2.3Enter your credit/debit card number to make payment or enter the unique transaction id of Axis Bankor the Journal number of Allahabad Bank to show that payment has been made. If you are making acash payment at Axis Bank or Allahabad Bank (it will be reflected on the website after 2 workingdays), begin the application process after 2 days of depositing cash. Those making payment by creditcard/ debit card can make application process instantly.

ADMIT CARD

Admit card will be available online and sent to all candidates on their email address on or before Feb. 7, 2011.It will not be sent by post.

1. Candidates should bring the admit card on the day of examination.2. Venue/Centre will be communicated through Admit Card.3. Coordinator, UPPGMEE-2011, shall not be responsible for non-receipt or delay of admit card due to

internet connection problems or mailbox problems.4. A successful candidate must preserve the Admit Card till admission in a college/institution.5. You may reprint your admit card. Duplicate admit card will not be issued by the examining body.

EXAMINATION

1. Date1.1.The examination will be held as follows:

1.2.The date of examination will not be changed.1.3.Every candidate is required to bring his/her admit card and identification document in original with

him/her and occupy his/her seat in the examination hall 30 minutes before the commencement ofexamination.

1.4.No candidate will be allowed to enter the examination hall after the commencement of theExamination under any circumstances.

2. UNFAIR MEANS2.1.Use or possession of cellular phone, pager, calculator or any other mechanism of communication, any

data retrieval system, written material in any form, electronic and nonelectronic gadgets, and anyother modality that may be considered unlawful by the Examination Centre Superintendent and theUnfair Means Committee shall be treated as use of unfair means.

2.2.Use of any unlawful means or unfair means before, during or after the examination shall result in thecancellation of candidature/selection of the candidate. Intimation will also be sent to theMCI/DCI/State MC/DC and the concerned Medical/Dental College for necessary action.

2.3.Candidates shall maintain silence and attend to their papers only. Any disturbance in the examinationhall shall be deemed as misbehaviour and the candidates shall forfeit the right to continue to write theexamination. The decision of the Centre Superintendent shall be final.

2.4.Submitting false information in the Application form will be treated as unfair means and theapplication of the candidate will be liable to be rejected even if he/she has successfully obtainedadmission in a course.

2.5. Photograph must be taken with a placard in front of the candidate carrying the name of the candidate andthe date of taking photograph. The name and date on the photograph must be clear and legible. Thephotograph should not be modified in any way after it has been taken. If the Examining body or the

invigilator, or the centre superintendent, or the Counselling Board considers that the photograph hasbeen manipulated in any way the candidature of the candidate will be summarily rejected and norepresentation will be entertained. The same photograph should be used at all places. Using differentphotographs will be taken as unfair means.

QUESTION PAPER

1. The examination will be conducted in English medium only.2. The examination shall consist of one paper of 190 MCQ (Multiple Choice Question) as follows:

a. 90 questions on pre and para-clinical subjects.b. 90 questions on clinical subjects.c. 10 questions on comprehension

3. There will be two sections of multiple choice questions (MCQs):Section A. There will be 180 questions with 4 options or choices of which one will be best

suited or best response.

Section B. There will be two paragraphs followed by 5 questions each based on that

paragraph. Each question will have 4 alternatives of which one will be best suited or best

response.

4. The Bestresponse to each shall be as determined by the Board of Examiners of the UPPGMEE-2011.The decision of the Board of Examiners shall be final and binding.

5. Each question in Section A shall carry 1 mark. Each question in Section B will carry 2 marks.6. There will be no negative marking.7. The answer key shall be published on the website www.kgmcindia.edu after the declaration of the

results before the last date of scrutiny.8. SYLLABUS: The syllabus will be the same as prescribed for the MBBS and BDS courses by the

Medical/Dental Council of India respectively. No books are prescribed.9. The questions will generally be of MBBS/BDS level. As per Government Order 50% questions will be

of average level, 25% will be more difficult and 25% will have a high difficulty level. The number ofquestions from different subjects in the paper shall ordinarily but not necessarily be as follows:-

3. EXAMINATION PROCESS

1. Candidates will have to report to the Examination Centre at 9:00 AM one hour in advance.2. Candidates should bring the Admit Card and the original Id proof to the examination centre.3. Candidates are required to take their seats by 9:30 AM. The Examination process shall start at

9:45 AM. No candidate will be allowed inside the Hall after 9:45 AM.

4. Once seated the candidates will be provided with an OMR sheet and question paper. Candidatesshould read instructions on the Question papers and OMR sheet carefully before filling the

OMR sheet. They will be guided through the process of filling the identification information on

the OMR sheet. Any defect in the OMR answer sheet should be brought to the notice of the

Invigilator who will change the OMR sheet if found defective. No change in OMR sheet will be

made after the start of the examination.

5. After distribution of question papers, candidates must immediately ensure that they havereceived question paper of the correct course, viz., MD/MS/Diploma or MDS as applicable.Subsequent representation will not be entertained.

6. Candidates will be photographed and fingerprinted in the examination Hall.7. Candidates should be careful while filling the circles on the Answer Sheet. Use only black ball pen

for filling the circles. Once filled, no change is possible. THINK BEFORE YOU INK.8. A dot or any other mark like X, -, , 0, or any other mark read by the optical mark reader will be

considered as a response marked by you.9. You will be given one copy of your OMR answer sheet at the end of the examination. Please mark

answers with sufficient pressure so that the mark is easily seen on the last copy of OMR sheet.

RESULT AND MERIT LIST

1. General category candidates shall have to secure a minimum of 50% marks and the reserved categorycandidates a minimum of 40% marks to qualify for admission to a postgraduate medical/dental course.Qualifying does not mean admission. Admission will be given according to merit at the time ofCounselling.

2. Merit list for only those candidates who have qualified will be declared. A marks sheet will be sent toall candidates.

3. Candidates securing minimum qualifying marks (50% for general and 40% for reserved) shall be givenranks according to the total marks obtained. There shall be a combined merit list (with State rank) forall category candidates. The merit rank of reserved category candidates in their respective categories(Category rank) shall also be indicated along with their combined merit (State rank).

4. In case of two or more candidates obtaining equal marks in the competitive entrance examination, theirinter-se rank will be determined in order of preference as under :

a. Candidate scoring more marks in Section B.b. According to age, older will get preference over younger.c. If rank cannot be decided by above criteria, then tie shall be decided by toss of coin.

5. There is no provision of re-evaluation. However, there is provision of scrutiny for whichcandidate may apply within seven days of declaration of result along with a demand draft ofRs.300/-in favour of UPPGMEE-2011 payable at Lucknow.

DECLARATION OF RESULTS

The result of successful candidates with their combined rank along with category rank will be declaredon the website of the University www.kgmcindia.edu , and www.indiaresults.com andwww.exametc.com. Acopy of the result will also be sent to the Secretary, Medical Education,Government of U.P., Director General, Medical Education and Training, U.P. Lucknow, the Principalsof U.P. State Medical Colleges, and Registrar, CSMMU, Lucknow.

ONLINE COUNSELLING:

1. Online Counselling shall be conducted by the Director General of Medical Education Uttar Pradesh.

The detailed schedule and details of Online Counselling shall be announced by the Director General ofMedical Education and Training, Jawahar Bhawan, 6th Floor, Ashok Marg, Lucknow 226001 at thetime of declaration of result in leading newspapers of Uttar Pradesh. The Examining body shall not beable to provide this information.

2. Online Counselling and allotment of the Courses will be done by the Director General of MedicalEducation and Training and the Examining body is not responsible for the same.

3. As per the Government of India, Directorate General of Health Services, letter No. C-18018/08/2005-MEC, dated 24.03.2008 there shall be TWO COUNSELLINGS. Candidates who get seat in 1stcounselling shall also be eligible for second counselling for reshuffling of seats with candidates whowere either absent or did not opt a seat in the first counselling.

4. Candidates are required to bring their application form duly verified by the Dean/Principal of theUniversiy/College from where the candidate has done his/her MBBS/BDS.

5. At the time of online counselling each candidate is required to:

a.sign the attendance sheet.b.deposit counselling fees ofRs. 500/- in cash.c.present all the original documents along with one extra photocopy as listed below-

(i) Admit Card.(ii) High School certificate as proof of date of birth.(iii) Mark Sheet of all professional examinations.(iv) MBBS/BDS passing out certificate or degree.(v) Compulsory Rotatory Internship completion certificate.

(vi) Permanent/Provisional Registration Certificate (U.P. State/MCI/DCI).(vii) Employer's No Objection Certificate, if employed.(viii) SC/ST/OBC/Physically Handicapped Certificate, if applicable. The OBC certificate musthave been issued after 15.10.2010.(ix) Identification certificate as mentioned in the application.(x) Verification sheet of the Principal/Dean of the College/University concerned.(xi) Two photographs attested by Principal with a clear stamp. Photographs must be the sameas pasted on the application form.(xii) Candidates who have passed MBBS from Himalayan Institute of Medical Sciences,Dehradun must submit the following additional certificates

i) Certificate of bona-fide resident of U.P.ii) Certificate of selection in the college through UPCPMT/Management

quota, before 9th November, 2000 issued by Director General MedicalEducation/Medical Health Services of Uttarakhand as per proforma.

If found eligible will proceed to

d.submit a security deposit in the form of a demand draft/bankers cheque of Rs. 10,000/-(Rupees ten thousand only) in favour of Director General Medical Education &Training, U.P., payable at SBI, Jawahar Bhawan Branch, Lucknow. The same will bereturned to the candidate after he/she takes admission. Those who do not join or leave thecourse after allotment will forfeit their claim for refund of the security deposit.

e.Choice filling- candidate then shall be given registration document and is allowed in to thecounselling hall where he/she shall be allowed to study the seat matrix displayed throughscreen in different deciplines/ specialties/medical college/medical university. After thishe/she shall be called for choice filling at a computer. He/she shall be given enogh time forchoice filling. After the candidate has filled his/her choices the official present there shalllock choices on the request of candidate. A Print out OF LOCKED CHOICES SHALL BETAKEN OUT IN duplicate on which the candidate and counselling official shall sign and acopy is given to the candidate. After all the candidate has locked choices the processingshall be done and the result shall be declared in the same night and the allotment letters shall

be given next day.6. Candidates who are applying for Physically Handicapped Category are required to submit attested

photocopy of certificate on attached proforma (Annexure No. 4), issued by the Medical Board(constituted by Director General, Medical Education and Training, U.P. under chairmanship of Principal,Medical College, Agra /Allahabad /Gorakhpur /Jhansi/Kanpur/ Meerut and Dean, CSM MU, Lucknow)of any Medical college on notified dates. Physically Handicapped Certificate issued by any otherauthority other than above mentioned Medical Board, shall not be entertained. The DisabilityCertificate shall be issued as per MCI norms. Claim of Physical handicapped category shall not beallowed if the handicapped certificate is not brought at the time of counselling. Claim for this categoryshall not be entertained after the last date of receiving the completed application form.

7. Candidates who have passed MBBS from Himalayan Institute of Medical Sciences, Dehradun and bringthe domicile certificate of U.P. and certificate of being selected in MBBS through CPMT/Management

quota before 9.11.2000 from the Director General Medical Education/Medical Health Services ofUttarakhand, on attached proforma (Annexure Nos. 5 & 6).

8. Candidate eligible for the reserved category will be given admission as per rules of the StateGovernment, applicable on the date of counselling.

9. Candidates who fail to produce required original documents during counselling or if any, concealment orfalsification of facts comes to light at the time will not be entitled to any benefit of having appeared inthe entrance examination and/or any benefit of acquiring a seat subsequently.

10. If candidate is absent in the counselling or does not join any course in a college after allotment of theseat by the due date shall forfeit any right of admission on the basis of said counselling/allotment.However, after second counselling, candidates who have not opted any but wish to keep their name in

the waiting list shall indicate their choice of college and in writing to the Chairman, Counselling Boardthat they should be considered for seat allotment after second counselling on any vacancy on the basis ofthe choices submitted during second counselling according to their merit as and when the vacancies arisebut before the last date of admission i.e. 31.5.2011.

11. If a candidate is absent when her/his name is called for counselling and comes at a later time, he/sheshall only be considered for a seat from among the seats available at the time of his/her appearance in thecounselling.

12. The option once exercised in the counselling cannot be altered under any circumstances.13. Every candidate who wishes to opt for a seat in the counselling shall present himself/herself before14. the counselling board. Their representative, through authority letter is not allowed to participate in the

counselling. Candidates will be photographed and their identity checked at the time of counselling. If itdoes not match it will be counted as unfair means and their candidature cancelled.

ADMISSION

1. Admission of the candidate to various P.G. Degree/Diploma course will be given by thePrincipal/Dean of the respective Medical College or Medical/Dental faculty of CSM MedicalUniversity, Lucknow as the case may be.

2. A candidate who has been allotted a P.G. Degree/Diploma course in a particular Medical/DentalCollege shall have to complete all the admission formalities including depositing the prescribed fees,by the date notified for this purpose by the respective Principal/Dean of Medical College or CSMMedical University, Lucknow as the case may be. Those who fail to do so will loose their claim foradmission.

3. Admission shall be given only after the candidate produces, in original and one photocopy each of thefollowing documents at the office of the Principal/Dean of the Medical College concerned or theRegistrar, CSM Medical University, Lucknow, as the case may be

a. High School Certificate as proof of date of birth.b. Mark sheets of all professional examinations.c. MBBS/BDS passing out certificate or degree.d. Compulsory Rotatory Internship completion certificate.e. Permanent Registration Certificate (U.P. State/MCI/DCI).f. Employers relieving certificate, if employed.

g. S.C./S.T./O.B.C./Physically handicapped certificate, if applicable.h. Allotment letter, issued by Chairman of the Counselling Board of UPPGMEE-2011.i. Admit Card of UPPGMEE-2011.j. Identification certificate mentioned in the application form.k. Verification sheet of the Principal/Dean of the College/University concerned.l. are required to submit a domicile certificate of U.P. on the prescribed proforma (Annexure 6)only and a certificate from the Director General Medical Education/Medical Health Servicesof Uttarakhand of being admitted in the year 2000 only.

4. After the admission process is completed the candidate will submit in writing his/her joining report tothe Head of the Department concerned.

5. The identity of the student will be established on the basis of his/her photograph, signature and fingerprints after his/her admission. He/She should make sure that he/she is available for this checking. Ifhe/she is not available for checking or if the identity of the candidate does not match with that of theidentity established at the time of taking examination it will be treated as impersonation and thePrincipal of the College/Registrar of the University will cancel the admission of the candidate andlodge a police complaint for impersonation.

Photographs should be

1. High contrast 3.5 4.5 cm size colour photographs (full front face, no cap, no hat, no dark glasses)with white/light background. Top of hair to bottom of chin should measure 2.5 to3.0 cm.

2. Polaroid and inkjet photos are NOT acceptable.3. Photograph MUST be taken on or after 15.12.2010.4. Photograph must be taken with a placard in front of the candidate carrying the name of the candidate

and the date on which the photograph is being taken. The name and date on the photograph must beclear and legible.

5. The photograph should not be modified in any way after it has been taken. If the Examining body or the invigilator, or the centre superintendent, or the Counselling Board or the

College/University(at any stage) considers that the photograph has been manipulated in any way or

does not match the candidate, the candidature/admission of the candidate will be summarily

rejected and no representation will be entertained.

6. Instruct your photographer to take the photograph with a 4MP or better camera and provide you a softcopy of the photograph in .jpg format. The file size should not exceed 1MB

7. Copies of this photograph should be brought at the time of Counselling and at the time ofadmission to the college.

8. Keep several identical photographs in reserve for use at the time of counselling/admission.
