ภูมิลําเนา ต. หนองหอย อ.เมือง จ.เชียงใหม

มีจ้ําเลือดตามแขนขา 2 วัน กอนมารพ.

แปรงฟน แตไมไดสนใจอะไร

2วัน กอนมารพ. ผูปวยเริ่มสังเกตเห็นจ้ําเลือดตามขา

ผูปวยทั้งสองขาง เปนจุดๆขนาดประมาณ 2 ซม.จํานวน

2-3 ตําแหนง ตอมาพบที่แขนทั้งสองขางรวมดวย ขนาด

ใกลเคียงกัน โดยผูปวยไมมีประวัติไดรับการกระแทก

บริเวณที่เปนมากอน

1 วัน ตอมา มารดาผูปวยสังเกตเห็นวาจํานวนของจ้ําเลือดเพิ่มมากขึ้น ปฏิเสธประวัติไข การเขาปาหรอืเขาคาย ไมมีจุดเลือดออกตามตวั รูสึกตกใจจึงพาไปพบแพทยที่คลีนิก แพทยแนะนําใหมาตรวจตอทีร่พ.มหาราช

Clot retraction

หลังจากเกิดลิม่เลือดในหลอดแกวสะอาด จะเกิดการหดตัวของลิ่มเลือด (clot retraction) ภายในเวลา 1-2 ชั่วโมง และเกิดการหดตัวสมบูรณในเวลา 24 ชัว่โมงClot retraction จะบกพรองในภาวะ

- Platelet < 20,000/mm3

- Platelet dysfunction- ภาวะพรอง fibrinogen รุนแรง- หลอดแกวสกปรก clot retraction จะเกิดไดไมดีหรือไมเกิดเลย

Venous clotting time (VCT)

บอกถึงความบกพรองของกลไกการแข็งตัวของเลือดสวน intrinsic pathway อยางคราวๆ ใชทดสอบขางเตียงผูปวยได

คาปกติ 5-15 นาที

Coagulogram

เปนการทดสอบวาความผิดปกติเกิดขึ้นกับสวนใดของการแข็งตัวของเลือด

แบงเปน

1. Prothrombin time (PT)ทดสอบ extrinsic (VII) and common pathway Normal 12-16 sec.

2. Activated partial thromboplastin time (APTT)ทดสอบ intrinsic and common pathway Normal 30-38 sec.

Clot lysis

Normal 24-48 hrs.พบในภาวะ hyperfibrinolysis

Vascular defect

Disorder of blood vessels.1. Scurvy(Vitamin C deficiency)

Formation of collagen and chondroitinsulfate is impaired.(Tendency to hemorrhage, defective tooth dentin, and loosening of the teeth.)Age at on set is between 6 and 24 month.Irritability, tachypnea, digestive disturbances, and loss of appetite.Generalized tenderness=>Pseudoparalysis;froglegposition.Change in gum ; Bluish purple, spongy swelling of the mucous membrane.Scorbutic rosary.

ScurvyDiagnoses is usally based on the characteristc clinical picture, the radiographic appearance of the long bones and the Hx of poor vitamin C intake.Prevented by an adequate intake of vitamin C. Lactating mothers should consume about 100 mg of Vit C daily.Rx: Daily intake of 3-4 oz of orange juice or tomato juice or Ascorbic acid 100-200 mg orally or parenterally.

Thrombocytopenia( decrease platelet number)

Cause of Thrombocytopenia

1. Decrease marrow production of megakaryocytes

IMMUNE THROMBOCYTOPENIC PURPURA (ITP)

Most common cause for acute onset of thrombocytopenia in an otherwise well child.Following exposure to a common viral infection, a small number of children develop an autoantibody directed against the platelet surface.Circulating antibody-coated platelets are recognized by the Fc receptor on the splenic macrophages

CLINICAL MANIFESTATIONS healthy 1- to 4-yr-old childsudden onset of generalized petechiae and purpuraOften there is bleeding from the gums and mucous membrane Splenomegaly is rare When the onset is insidious, especially in an adolescent, the possibility of chronic ITP or that thrombocytopenia is a manifestation of a systemic illness

CLINICAL MANIFESTATIONS

7-8 percent of children who present with acute ITP will have spontaneous resolution of their ITP within 6 mo. Less than 1% of cases develop intracranial hemorrhage 10-20% of children who present with acute ITP go on to develop chronic ITP.

LABORATORY FINDINGS Platelet count < 100,000 / mm3

Platelet size is normal or increased Bone marrow examination reveals normal granulocytic and erythrocytic series with characteristically normal or increased numbers of megakaryocytes. Antinuclear antibody (ANA) test is more often positive in adolescents with ITP and may indicate a higher likelihood of eventual chronic ITP.

TREATMENT

Intravenous immunoglobulin (IVIG) 0.8 to 1 g/kg/day × 1-2 day

There is a high frequency of headaches and vomiting suggestive of aseptic meningitis following IVIG infusions.

Corticosteroid therapy • 1-4 mg/kg/24 hr of prednisone • suggested that a bone marrow examination

be performed to rule out other causes for thrombocytopenia

TREATMENTIV Anti-D Therapy

• Therapy of acute ITP • RBC-antibody complexes bind to Fc receptors and

interfere with platelet destruction, thereby causing a rise in platelet count.

SplenectomyIndication : - The older child (4 yr) with severe ITP that has lasted longer than 1 yr (chronic ITP)

- Symptoms are not controlled with medical therapy (1 yr.)associated with a lifelong risk of overwhelming postsplenectomy infection caused by encapsulated organisms eg. pneumococcus

พบโรคนี้กระจายในทกุกลุมอายุ

ในเด็กพบวารอยละ 90 อายุมากกวา 6 ป (สูงสุดในชวงอาย ุ10 ป)

APLASTIC ANEMIA

กลไกการเกิดโรค

1. Stem cell failure : ความผิดปกติของ stem cell โดยมีจํานวน หรือมีคุณสมบัติผิดปกติ

2. ความผิดปกติของmicroenvironmentของเซลลและเนื้อเยื่อตางๆในไขกระดูก หรือมีความผิดปกติของเซลลที่ควบคุมการสรางเม็ดเลือด

3. ความผิดปกติของระบบอิมมูนที่ควบคุมการสรางเม็ดเลือด

APLASTIC ANEMIA

อาการและอาการแสดง

Anemia : ซีด, ออนแรง, เหนื่อยงาย, เบื่ออาหารLeukopenia : ไข ติดเชื้องาย

Thrombocytopenia : petechiae, bruise, epistaxis, gum bleeding, GI bleeding, hypermenorrhagiaGradual onsetไมมตีับ มาม หรือตอมน้ําเหลืองโต

increased risks of fatigue, cardiac failure

APLASTIC ANEMIA

LaboratoryAnemia : normochromic normocytic RBCReticulocytopeniaLeukopenia , GranulocytopeniaThrombocytopenia : Plt.มักนอยกวา 30,000 /cummBM aspiration : hypocellularity, มีเซลลไขมันเพิ่มขึ้น, จํานวน erythoid/myeloid/megakaryocyte cell ลดลง

อาจพบลักษณะเมด็เลือดแดงอยูกันเปนกลุม โดยเปนเม็ดเลือดแดงที่ทํางานไมไดตามปกติ(Dyserythropoiesis)

APLASTIC ANEMIA

2. WBC < 3,500 /cumm or Absolute neutrophilcount < 1,500 /cumm

or BM ม ีhematopoietic cell < 25%

APLASTIC ANEMIA

ACUTE LEUKEMIA

เปนมะเร็งที่พบบอยที่สุดในเด็ก

ชวงอาย ุ4-6 ป พบไดบอยเพศชายมากกวาเพศหญิง

Acute lymphoblastic leukemia พบ 75-80 %Acute non-lymphoblastic leukemia พบ 20-25 %เกิดจาก hematopoietic stem cell มีการแบงตัวมากผิดปกติ โดยทีไ่มมี differentiation

ACUTE LEUKEMIA

Clinical presentation• ซีด เลือดออกผดิปกติ ไข จากการที่มีจํานวนเม็ดเลือดลดลงเนื่องดวยการถูก

เบียดดวยเซลลมะเร็ง

• ปวดกระดูก จากการที่เซลลมะเรง็ไปที่ perichondral bone หรือเบียด marrow space

• มีเลือดออกตามเยื่อบุ หรือมีจุดเลอืดออกแบบ petechiae และ ecchymosis

• มตีับ มาม ตอมน้ําเหลืองโต

ACUTE LEUKEMIA

Laboratory• CBC : Hb และ Hct ลดต่ําลง

WBC > 10,000 /cummPlt. ลดลง

• Blood smear พบ lymphoblast จํานวนมาก• BM examination

พบวามีจาํนวนเซลลเพิ่มขึ้น โดยมีจํานวน blast cell มากกวารอยละ 30 ของ nucleated cellErythroid cell และ Megakaryocyte มีจาํนวนลดลง

• Immunophenotype เพื่อดูวาเปน blast cell ชนิดใด

ACUTE LEUKEMIATreatment

1. Specific treatment : Chemotherapy สามารถหายขาดไดถึงรอยละ 70

2. Supportive treatment• Blood transfusion : Packed red cell, Platelet

concentration• Febrile neutropenia : physical exam, CXR,

Septic work up, ATB empirical treatment• Prevention for tumor lysis syndrome : acute

RF, ArrhythymiaHydration keep urine specific gravity < 1.010Urine alkalinization keep urine pH 6.5-7.5 ปองกันการตกตะกอนของ uric acid ที่ไตAllopurinol ลดการสราง uric acid

DENGUE HEMORRHAGIC FEVER

associated with second infections with dengue types 1-4


อาการและอาการแสดง

1. ระยะไขสูง ไขขึ้นเฉียบพลันและจะสูงลอยอยูระหวาง 39-40 Cปวดศีรษะ ปวดทองแถวลิ้นปหรือชายโครงขวา ปวดตามกลามเนื้อเบื่ออาหาร อาเจียนเจ็บคอ คอแดงตับโตพบไดรอยละ 60-90

ประมาณวันที่ 2-3 จะเริ่มมีจุดเลือดออกเล็กๆตามผวิหนัง หนาและตัวอาจจะแดงอาการไขจะคงอยูประมาณ 3-7 วัน แลวจะลดลงสูปกติเขาสูระยะที่ 2


2. ระยะเลอืดออก

ไขจะลดลงอยางรวดเร็ว

ผูปวยจะกระสับกระสาย มือเทาเย็น ชีพจรเตนเร็ว มี pulse pressure แคบ(< 20 mmHg)

อาเจียนมาก ปสสาวะนอยอาจมีอาการหอบจากภาวะ acidosisอาจเสียชีวิตได

ระยะนี้จะคงอยูประมาณ 24-48 ชม.


3. ระยะฟน

จะกลับสูสภาพปกติอยางรวดเร็วใน 2-3 วัน เริ่มกินอาหารได ลุกขึ้นนั่งไดอาการตับโตจะคอยๆลดลงภายใน 1-2 สัปดาหมีผื่นแดงขึ้นตามแขนขา ลักษณะกดไมจางเหมือนจุดเลือดออก อาจพบลักษณะ Convalescent rash : วงขาวๆขนาดเล็กสลับกับผิวหนังที่มีผื่นแดง

อาจใหผล Tourniquet test เปนบวกได


แบงความรุนแรงของโรคออกเปน 4 ระดับ1. มีไข และ Tourniquet test ใหผลบวก2. มีอาการเลือดออกตามผวิหนัง หรือที่อวัยวะอื่นๆรวมดวย ความดันเลือดอยูใน

เกณฑปกติ

3. มีชีพจรเบา มี pulse pressure แคบ(< 20 mmHg) มือเทาเย็น กระสับกระสาย

4. คลําชีพจรหรือวัดความดันเลือดไมได


Laboratory1. Complete blood count

Hct. : พบภาวะ hemoconcentration ยกเวนในคนที่มีเลือดออกมากWBC : มักอยูในเกณฑปกติหรือต่ํา(< 5,000 /cumm)

ใน Peripheral blood smear อาจพบ Atypical lymphocytePlaetelet : มักมีระดับลดต่ําลงกอนระยะไขลด (< 100,000 /cumm)

2. Urine analysisSpecific gravity สูงพบ Albumin ได 70 %

พบ RBC ได 80 %

3. Erythrocyte sedimentation rateใน DHF จะมีคาปกติในระยะไข และลดต่ําลงในระยะที่มีการรัว่ของพลาสมาใน Septic shock จะมีคา ESR สูงขึ้น

DENGUE HEMORRHAGIC FEVER3. Tourniquet test

พบจุดเลือดออกมากกวาหรอืเทากับ 10 จุดตอตารางนิว้ใหผลบวกประมาณ 80-85 % ในระยะไขและระยะพักฟน

4. Chest X-Rayมักพบวามีน้ําในชองเยื่อหุมปอดขางขวามากกวาขางซาย

อาจมีน้ําในชองทองรวมดวย

5. Serological test : hemagglutination-inhibition test (HI)ตองเจาะ 2 ครั้งหางกัน 1-4 สัปดาห โดยถือระดบั antibody เพิ่มขึ้นมากกวาหรือเทากับ 4 เทาใน primary dengue infection จะมี titer ขึ้นสูงเฉพาะ serotype ที่เปนสาเหตุใน secondary dengue infection จะมี titer ขึ้นสูงกวาและมี titer ขึ้นสูงตอทุก serotype สามารถพบคา titer เพิ่มขึ้นไดแมเจาะเลอืด 2 ครั้งหางกันนอยกวา 7 วัน


Treatmentใหสารน้ํา

ลดไขดวย Paracetamolเฝาระวังภาวะ shock : ปสสาวะนอยลง, กระสับกระสาย, มือเทาเย็น และภาวะเลือดออก

HYPERSPLENISMเปนภาวะที่มีมามโตรวมกบั

1. Anemia, Leukopenia, Thrombocytopenia อยางใดอยางหนึ่งหรือพบรวมกัน2. Cellular bone marrow3. ภาวะ Cytopenia ดีขึ้นหลงัไดรับการตัดมาม

CBC : พบ normochromic anemia และmild to moderate thrombocytopenia ไดบอยBM aspiration : พบ normocellularity หรือ moderate hypercellularity โดยมี maturation ของเซลลตางๆปกติการรกัษาตองทําการหาสาเหตแุละรกัษาตามสาเหตนุั้นๆ

การทาํ Splenectomy จะไดผลดีเฉพาะในกลุมที่มีสาเหตุจาก mechanical destruction หรอื sequestration ของเม็ดเลือดและเกรด็เลือด

Platelet dysfunction

Acquired disorders of platelet function.

Systemic illness : liver disease, kidney disease(uremia). Cause prolonged bleeding time and often associated with orther abnamalities of coagulation mechanism.Management is to treat the primary illness.Rx: Infusion of desmopressin.

Tranfusion of platelets and/or cryoprecipitate.

Acquired disorders of platelet function.

Drug:Aspirin : irreversibly acetylates Enz. Cyclooxygenase.Other drugs :

Nonsteroidal anti-inflammatory drugsValproic acidHigh dose penicillin

Acquired platelet dysfunction with eosinophilia(APDE) เปนสาเหตุของ purpura ที่พบไดบอยที่สุดในเด็กไทย

Acquired platelet dysfunction with eosinophilia (APDE)

เปนสาเหตุที่พบไดบอยที่สุดในเดก็ไทยรองมาจากโรคติดเชื้อ จะตรวจพบวามี ecchymosis ขนาดกลาง เปนไตแข็งเล็กๆขางใต อาจเกิดเองหรือจากการกระทบกระแทก

อาจพบเลือดกําเดาไหล เลือดออกระบบทางเดินอาหาร อาการเลอืดออกมักจะไมรุนแรงตางกับ ITP ที่รุนแรงกวาLab : พบ platelet ปกติอาจพบวามีจํานวน eosinophil สูงขึ้นรวมดวยมี bleeding time ยาวกวาปกติไมทราบสาเหตุของโรค

หายไดเองในเวลา 1-2 เดือน

ปองกันโดยการใหวิตามินเคแกทารกแรกเกิด

มักพบในเด็กอายุ 2 wks-2 mo ที่ไมไดรับวิตามินเค

Late HDN หรือ idiopathic vitamin K deficiency in infancy

(IVKDI) หรือ APCD (acquired prothrombin complex

deficiency)

Prothrombin complex ต่ําจากการขาด Vit K

มักพบในเด็กอายุ 2 wks-2 mo ที่ไมไดรับวิตามินเค

1:1800

อัตราตายรอยละ 23-52 (เลือดออกในสมอง)

อัตราพิการเชน ปญญาออนรอยละ 37-48

ปองกนัโดยการใหวิตามนิเคแกทารกแรกเกิด

Secondary prothrombin complex deficiency

พบในทารกที่มีโรคตับ เชน biliary atresia , short

bowel syndrome ที่การดูดซึมวิตามนิเคจากลําไสเล็ก

บกพรอง

Snake bite

พิษงูแมวเซา – DIC , hyperfibrinolysis

พิษงูกะปะ และงูเขียวหางไหม - hyperfibrinolysis

เลือดออกในบริเวณแผลที่ถูกงกูัด เลือดออกตาม

ไรฟน อาเจียนเปนเลือด

การรักษาคือใหเซรุมพิษงู และใหสวนประกอบ

ของเลือดทดแทนในผูปวยบางราย
