• NJWELL aims to cultivate healthy lifestyle choices and improve wellbeing among actively employed members of the SHBP and SEHBP.

SHBP/SEHBP coverage.g• Anyone enrolled in COBRA or retiree

• Empower individuals to take steps to improve their health.

• Reduce risks for conditions such as: heart disease, diabetes, stroke, and b itobesity.

Health Assessment and Screening must be completed by October 31, 2014 in order for eligible participants to qualify for a gift card.

Health AssessmentThe on-line Health Assessment Questionnaire allows members to gauge their potential health risks by answering a comprehensive self-assessment. The assessment captures the following information: p g

• Current health conditions • Vital statistics, including cholesterol and blood , g

pressure measurements • Lifestyle factors: alcohol consumption, tobacco use,

stress, exercise and diet ,• Life events: family, children, key relationships,

NJWELL participants can earn points for completing a Health Assessment on the Horizon or Aetna/ActiveHealth online portals starting January 1, 2014. Employees must

register for their health insurance carrier’s portal in order to complete the Health Assessment.

The focus of year two is on taking action and engaging employees to participate in healthy activities.

Year 2 Continued…• In year 2 participants will have additional opportunities to earn points in NJWELLIn year 2, participants will have additional opportunities to earn points in NJWELL

by getting age appropriate, doctor recommended screenings, and by utilizing services available on their health plan’s online portal.

• Points for “Online Activities” are earned when participants utilize tools on their health plan’s online portal including (but not limited to) exercise logs and food journals.

• The number of points necessary to earn a financial reward increases.

• The potential value of the financial reward also increases.

• If both an employee and their covered spouse or partner earn 500 points, they each receive a $200 Visa Gift card, for financial rewards

Designate a Wellness Champion• NJWELL is asking each Employer to designate• NJWELL is asking each Employer to designate

a Wellness Champion as soon as possible. Designation form is available at www.state.nj.us/treasury/pensions click “NJWELL” in the Special AnnouncementsNJWELL in the Special Announcements Column.

• An ambassador to promote NJWELL and encourage healthy behaviors by employees of SHBP/SEHBP lSHBP/SEHBP employers.

• Implements and coordinates wellness initiatives for the employer.

• Works with the Division of Pensions and Benefits’ designated NJWELL coordinator to implement on site health and wellness initiatives.

• Supports the goals of the program and theSupports the goals of the program and the health goals of the Employer.

• Encouraged to form a “Wellness Committee” in locations with multiple worksites.

• Wellness Champions will receive a toolkit from the SHBP/SEHBP that will include electronic and printed materials including promotional flyers and posters.

• At employers with multiple worksites, it is recommended that the Wellness Champion form a “Wellness Committee” to assist them in promoting NJWELL.

• They can also get a headstart on starting healthy workplace initiatives to maximize participation in 2015.

Wellness Champions Year 2 (2015) and Beyond( ) y• Starting in year 2 (2015) the Wellness Champion can help

colleagues obtain points by encouraging participation in healthy workplace initiatives such as “walk at lunch” or “healthy eating” programs.

• Participants in the healthy workplace initiatives can log their participation into their health plan’s online portal and receiveparticipation into their health plan s online portal and receive points towards their financial reward for completing online activities.

• Wellness Champions will receive frequent suggestions for healthy workplace initiatives from the SHBP/SEHBP, Horizon BCBS, and Aetna.

• They will also receive periodic trainings in the form of webinars.

• They are encouraged to provide feedback and communicate successfully implemented initiatives to the NJWELLCoordinator at [email protected]

• Healthy Workplace initiatives offer a sense of camaraderie among coworkers.

• Employees have a chance to participate in activities together that aren’t related to work, through programs including walk at lunch g p g gclubs, or healthy eating groups.

• This leads to improved employee relations and morale, as well as increased productivity.

Employer Requirement: Taxation of RewardsRewardsBeginning in February 2014 and continuingthroughout the year, members who meet theeligibilit and participation req irements foreligibility and participation requirements foryear 1 of NJWELL will be issued a$100 Prepaid Visa gift card.

• As an Employer, you should know about the tax requirements that go along with NJWELL

• Prepaid Visa Gift cards are considered taxable income to the employee and their spouse/partner.

• Centralized Payroll will handle taxation for State Biweekly locations. State Monthly locations will receive a report listing employees who received rewards via EPIC.

• State Monthly locations should check with their payroll vendor for information on reporting and withholding taxes on the value of the financial rewards issued to
