• คนไขปฏเสธโรคประจำตว (ไมเคยตรวจ) และปฏเสธยาททานประจำ

• ไมทราบประวตทนตกรรมของสมาชกในครอบครว

Oral hygiene practice-แปรงฟนวนละ 2 ครง (หลงตนนอนตอนเชา และกอนนอนตอนกลางคน)

-ใชแปรงสฟนขนนมปานกลาง หนาตดหยก รวมกบยาสฟนสมนไพรชนดผง

-ทำความสะอาดดวยการถไปมาในแนวนอน

-ใชนำยาบวนปาก ทำความสะอาดเพมเตม ตอนเชาทกวน (ใชไมจมฟนเขยเศษอาหารออก)

Habit -ปฏเสธการสบบหร -ปฏเสธการดมแอลกอฮอล

Extra-oral examination-ใบหนาปกต สมมาตร ซาย-ขวา ใบหนาดานขางเปนลกษณะเวาเลกนอย

-ไมพบความผดปกตของขอตอขากรรไกร ใบหนา ลำคอ ตอมนำเหลอง และกลามเนอบดเคยว

Intra-oral examination1.Soft tissue examination

• ไมพบความผดปกตทรมฝปาก เยอบผวขางแกม เพดานออน เพดานแขง และลน

2.Occlusion

• Right : Class I molar relationship and Class I canine relationship.

• Left : Class I molar relationship and Class I canine relationship.

• Anterior deep bite tendency

• Right and left lateral movement : Group function

3.Gingival examination The general, the gingiva is bluish-red color except buccal surface of lower posterior teeth are red color.

Round margins except palatal surface of 23,24,25 are roll margins. IDPs are edematous papillae except lower posterior teeth, 33/32, 41/42, 42/43 are blunted papillae. Soft in consistency and glossy texture. Presence of supra & sub-gingival calculus. Pus exudate was detected at all of upper teeth.

4.Dental deposit

Supra-gingival plaque and stained deposit.

Heavy sub-gingival calculus deposit on lingual aspect of lower teeth, and palatal aspect of upper posterior teeth.

ผปวยมฟนในชองปากจำนวน 30 ซ•ไมพบฟนซ14,15 ในชองปาก (คนไขใหประวตวา ถอนเนองจากฟนโยก) •PFM crown : 11,21,22•Dental caries : 35(O),38(O) •Defective filling : 36(B) over-hanging margin•Food impaction : 35/36, 26/27, 27/28•Trauma from occlusion : 11/41,21/31 •Spacing 13/12 (1.0 mm.), 21/22 (1.0 mm.), 22/23(0.5 mm.), 32/33(1.5 mm.)•พบ Physiologic attrition

Etiologic of periodontal disease

สาเหตหลก คอ Dental plaque สาเหตเสรม คอ Calculus, Trauma from occlusion 11/41, 21/31

Trauma from OcclusionTrauma from occlusion has been defined as structural and functional changes in the periodontal tissues caused by excessive occlusal forces. Occlusal trauma can be acute if caused by external impact forces or chronic if caused by internal occlusal factors (premature contacts, grinding). Chronic occlusal trauma can be understood as primary and secondary trauma.

>> Primary occlusal trauma

Primary occlusal trauma is caused by excessive and non- physiological forces exerted on teeth with a normal, healthy and non-inflamed periodontium.

>> Secondary occlusal trauma

Secondary trauma from occlusion is defined as the trauma caused by excessive and premature occlusal forces on teeth with an inflamed periodontium.

“In the 1960s, Glickman (Glickman & Smulow 1967) formulated the hypothesis that premature contacts and excessive occlusal forces could be a co-factor in the progression of periodontal disease by changing the pathway and spread of inflammation into the deeper periodontal tissues.”

Diagnosis • Generalized severe chronic periodontitis (AAP, 1999)

CAL: 1 - 2 mm. = 2.78 %

3 - 4 mm. = 41.67 %

≥ 5 mm. = 55.56 %

•Dental caries : 27(D) [พบในx-ray] , 35(O), 38(O) [Mild caries risk]

• 36(B) Defective restoration

• 14, 15 Edentulous space [PDI Classification : Partial edentulism Class I]

Overall prognosis (Thai Association of Periodontology,2556)

• Poor – พยาธสภาพสวนใหญเปนการละลายตวของกระดกเบาฟนแบบ horizontal bone loss ในระดบประมาณ 1/3 – 2/3 of root length – ผปวยม trauma from occlusion จากครอบฟน ใหประวตทำครอบฟนมาแลวประมาณ 2 ป – แตผปวยม attitude ทดในการจะดแลสขภาพ และผปวยใหความรวมมอในการรกษาดมาก คาดวาจะสามารถทำความสะอาดภายในชองปากไดดเมอไดรบคำแนะนำ

- Prosthodontist แจงวาสามารถ เกบซ 11,21,22 สำหรบเปน abutment ครอบฟนใหม แต prognosis ไมด จงควรบอก ขอดขอเสย ใหผปวยทราบและตดสนใจอกครง

- หลงจากแจงขอด-ขอเสย ใหผปวยไดทราบถง ผลหลงจากการรกษาโรคปรทนต ทอาจจะทำใหเกดขอบเหงอกรวน อาจจะทำใหเหนขอบของครอบฟน ซงอาจจะสงผลตอความสวยงามของใบหนา หรอหากทำครอบฟนใหม กอาจจะตองมการรกษาดวยวธทซบซอน รวมกบ ฟนซ 11,21 ม Trauma from occlusion เปนปจจยเสรม ทำใหคาดการผลการรกษาไดยาก ซงหลงจากทผลทอาจจะเกดหลงการรกษาโรคปรทนต ผปวยไมตองการใหเหนขอบของครอบฟน ยนยอมทจะถอนฟนเหลาน แตไมอยากใหฟนหนาหรอ

- Consultation to Prosthodontist for evaluation and planning Immediate denture ซงสามารถทำ Immediate denture ได ใน upper arch ทดแทนฟนซ 16,11,21 และ 22 ได ออกแบบ immediate denture เปนแบบ short-flange ใสฟนทดแทนฟนซ 16,15,14,11,21,22 และวางตะขอเพอเพมความยดอย (Retention) โดยใชตะขอลวดดด วางทตำแหนงฟนซ 17, 13, 23
