TYMPANOGRAM
Type A: Normal tympanic membrane (TM) mobility and normal middle ear (ME) pressure. Type As: Reduced TM mobility and normal ME pressure; consistent with presence of otosclerosis. Represents stiff ME system. Type AD: Hypermobility of TM and normal ME pressure; consistent with flaccid TM or disarticulation of ossicular chain. Type B: Reduced TM mobility; consistent with presence of fluid in the middle ear space. Type C: Normal TM mobility and negative ME pressure; consistent with eusta-chian lube dysfunction.
5 TYPE OF TYMPANOGRAM
Type air pressure(Pme)daPa
Compliance (Peak Ytm)
Cause
Type A -150 to +100 0.2 - 1.5 normal
Type A s (shallow)
-150 to +100 < 0.2 ossicular chain fixation
Type A d (deep)
-150 to +100 > 1.5 flaccid TM or
ossicular chain disruption
Type B no peak - MEE
Type C: more negative pressure than -150
0.2 - 1.5 Negative middle ear pressure

OTOSCLEROSIS
A CALLUS OF BONE ACCUMULATES ON THE STAPES CREATING A PARTIAL FIXATION>> LIMITS THE MOVEMENT OF THE STAPES BONE>>HEARING LOSS.
Schwartze sign in otosclerosis
WHAT CAUSES OTOSCLEROSIS?
Unknown About 50 percent of patients with
otosclerosis have a family history of otosclerosis.
Measles virus as a factor in causing otosclerosis.
Women, pregnancy can accelerate the process.
WHAT ARE THE SYMPTOMS OF OTOSCLEROSIS?
Hearing loss Dizziness Tinnitus Balance problems
HOW IS OTOSCLEROSIS DIAGNOSED?
A history of gradually progressive hearing loss in one ear, beginning in a patient's late teens or early adulthood.
On examination, the ear canal and eardrum are normal.
The audiogram indicates a conductive type of hearing loss. A special type of testing, tympanometry, indicates limited or no movement of the bones of hearing.
TREATMENT
Observation Medications : fluoride A hearing aid Surgery
POST STAPEDECTOMYWIRE PROTHESIS
PREVENTIONPrimary prevention ใหูคาแนะนาด็านกรรมพูนธ"Secondary prevention Screening in high risk people Early diagnosisTertiary prevention Aural rehabilitation
Drinking fluoridated water may help prevent otosclerosis in people who are susceptible. There are no other known ways of preventing this condition.
