Reactions 1264 - 8 Aug 2009 S Amoxicillin Biphasic anaphylaxis in an elderly patient: case report Biphasic anaphylaxis occurred in a 68-year-old man after amoxicillin administration. The man, who had a history of ischemic heart disease, hypertension and diabetes mellitus, took one amoxicillin tablet [dosage and therapeutic indication not stated], and developed itchy palms and soles, erythema on his thorax and conjunctival injection 15 minutes later. The man took a histamine H1 receptor antagonist, and his symptoms resolved. However, 3 hours after drug intake, he developed weakness and chest oppression, for which he took nitroglycerin [glyceryl trinitrate]; he experienced a feeling of tightness in his throat, and eventually lost consciousness and was admitted to an emergency department. On admission, he was hypotensive with a BP of 85/55; an acute cardiac event was excluded. After 9 hours of observation, his BP spontaneously normalised (120/80). Thoracic pain and allergic reaction were diagnosed, and he was discharged with prescriptions for a histamine H1 receptor antagonist and cortisone. The man underwent allergy testing 3 months later, in November 2008. Patch tests were negative for all tested antibacterials, including amoxicillin. However, intradermal skin tests showed an immediate-type reaction to amoxicillin 2 mg/mL, confirming the diagnosis of allergic biphasic anaphylaxis. Zisa G, et al.
