MANIFESTACIONES CLINICAS
Primera semana:insidioso incremento de lafiebre (unremiting), asociado a sntomassistmicos: cefalea, letargia, malestar general,mialgias y dolor abdominal.
Segunda semana:hepatoesplenomegalia,machas rosas (rose spots), stupor, bradicardiarelativa.
Tercera y cuarta semana:fiebre vespertina,hemorragia intestinal, perforacin, gradualdeclinacin de picos febriles, miocarditis,meningitis, neumonia.

Estudio prospectivo en nios, en elHospital Universiti Sains Malaysia:Octubre 1993 Agosto 1998:
9292 admisiones: 192 pacientespeditricos: 102 cultivos positivos a
S. typhi. Tratamiento: 54%Ampicilina.46% CAF.
1/3: complicaciones: hepatitis,supresin de medula osea, ileoparaltico, miocarditis, psicosis yosteomielitis.
Mayor riesgo de complicaciones:esplenomegalia, trombocitopenia,leucopenia.

Fiebre tifoidea MDR: falta de respuesta altratamiento (48-72h), complicaciones mayorproporcin de casos fatales.
reas donde es poco comn MDR: 14 das detratamiento va oral: amoxicilina, cotrimoxazol,cloranfenicol.
reas donde cepas MDR prevalentes:
ciprofloxacino, cefixime, ceftriaxona. Tiempo de tratamiento: 7 a 14 das.
