What is the incidence of DKAin diabetic patients?
Data on the exact incidence of DKA amongst diabetic patients is limited:
•Incidence of DKA in patients with unspecified type of diabetes− Crude and age-adjusted hospital discharge rates in the US in 2009 for DKA as the first-listed
diagnosis was 7.1 to 22.0 per 1,000 patient years (PY)− Rates by age group varied: 32.4/ 1,000 PY age < 45 years; 3.3/ 1,000 PY age 45–64 years;
1.4/ 1,000 PY age >65 years
• Incidence of DKA in patients with T2D− A study in Northern Sweden reported an estimated DKA incidence rate in adult patients − A study in Northern Sweden reported an estimated DKA incidence rate in adult patients
reported to have T2D of 0.5 per 1,000 PY [Wang, 2008].− In preliminary data from the ongoing BI-sponsored epidemiological study 1218.161, the
incidence of DKA in adult patients reported to have T2D in the 6 months prior to initiation of a medication varied from 4-8 per 1,000 PY [BI unpublished data].
• Incidence of DKA in patients with T1D− Information on the incidence of DKA in adult patients reported to have T1D is limited and
reported estimates vary. Most studies conducted in North America and Europe reported the incidence of DKA to be between 10-30 per 1,000 PY

Comparisons across studies and regions are difficult and should be done with caution given the differences in mean age, diabetes duration, level of glycemic control, and DKA definitions
What are the signs andsymptoms of DKA?
Signs and symptoms are non-specific and may typical ly include
• Persistent hyperglycaemia
• Nausea and anorexia
• Vomiting
• Abdominal pain
• Dehydration and excessive thirst
•• Kussmaul respiration (deep and laboured breathing pattern)
• Confusion and coma
• The diagnosis of DKA is multifactorial and usually based on the presence symptoms in combination with increased ketone bodies in the blood and/or urine as well as measurements indicating the blood being acidic, e.g., reduced arterial pH, reduced bicarbonate and an increased anion gap

� Based on the low number of DKA cases described above, the current data from BI sponsored placebo-controlled clinical trials in patients with T2D do not indicate an imbalance in the proportion of patients developing DKA between empagliflozin and placebo
� Until July 2015, BI is aware of 5 further cases of investigator reported DKA that were collected throughout the EMPA-REG OUTCOME™ trial (n> 7,000 pat); however, these cases have not been unblinded and could therefore relate to the empa or placebo arm
