Evaluation of the effect of signalment and owner-reported impairmentlevel on accelerometer-measured changes in activity in osteoarthriticdogs receiving a non-steroidal anti-inflammatory

In veterinary medicine, evaluation of osteoarthritis (OA) treatment efficacy remains challenging.Measurement of activity, utilizing accelerometers, provides a surrogate measure of pain throughmeasuring effects on activity, and the objective data collected can be used to assess the efficacy oftreatments. However, little is known about how dog characteristics impact the accelerometry-measuredresponse to treatment. The objectives of this study were to evaluate the effect of signalment and initialimpairment level on accelerometer-measured changes in activity in osteoarthritic dogs after receiving anon-steroidal anti-inflammatory (NSAID). Fifty-seven client-owned dogs with OA-associated pain andmobility impairment were administered meloxicam for 2 weeks, following a 2-week baseline, andspontaneous activity was measured using an Actical accelerometer unit. Signalment factors and diseasevariables were recorded (age, sex, weight, impairment level, forelimb or hindlimb pain).Initial degree of impairment had a significant effect on changes in weekly (P = 0.009), weekday
(P = 0.044) activity following NSAID treatment. Greater initial impairment was associated with largerpositive changes in activity. Degree of impairment should be taken into consideration during thedevelopment of a clinical trial. Appropriate selection of candidates based on initial degree of impairmentmay permit a greater treatment effect, therefore increasing the power of the study.

Introduction
Osteoarthritis (OA) is a common cause of joint pain incompanion animals, with the true prevalence unknown. Somedata suggest that at least 20% of all dogs >1 year of age per year(Johnston, 1997), but one author (BDXL) has recently generateddata suggesting 35% of all dogs are clinically affected by OA(unpublished data). Currently, there is no cure for the disease;therefore, treatment is focused on controlling the clinical signs.
Evaluation of treatment efficacy remains challenging, particu-larly when most measures assess changes in behavior. Recently, there have been several subjective Clinical Metrology Instruments
(CMIs) developed and validated, with the most rigorously testedbeing the Canine Brief Pain Inventory (CBPI; Brown et al., 2008)and the Liverpool Osteoarthritis in Dogs instrument (Hercock et al.,2009; Walton et al., 2013). These appear to work well under thesettings of a randomized, placebo-controlled, blinded clinicalstudy. However, CMIs are limited by their subjective nature and thefact that they are generated by a caregiver, and not the patient. Objective outcome measures are beneficial because potentiallythey are able to circumvent the limitations associated with CMIs,particularly the caregiver placebo effect (Muñana et al., 2010;Conzemius and Evans, 2012). There is currently a dearth ofobjective measures that can be applied in the home environment.Activity measurement, using accelerometers, is the most widelystudied objective measure of patient activity in veterinarymedicine (Hansen et al., 2007; Dow et al., 2009; Michel andBrown, 2011; Yam et al., 2011). Assuming that pain impacts activity, accelerometry is thought to provide a surrogate measure of painand pain relief, and they have been used as such in several recent
