• Il semble que le facteur déclenchant du SHR sur un terrain de cirrhose soit principalement hémodynamique:– Aggravation de la vasoplégie systémique (sepsis)– Accentuation de l’hypovolémie relative (hémorragie) SHR type I aigu, rapidement léthal, associé à une
défaillance multiviscérale– Ou simplement l’évolution de la cirrhose
SHR type II plus lent et moins grave
Ces phénomènes concourent à la majoration de la vasoconstriction rénale et donc la diminution du DFG
