BỆNH LAO LÀ GÌ?
Bệnh lao là một bệnh nhiễm khuẩn do trực khuẩn lao gây nên và là một bệnh lây giữa người và người chủ yếu qua đường hô hấp.
Biến chứng và di chứng nặng nề :
- Lao phổi +biến chứng : ho ra máu,TKMP,SHH cấp/man,xẹp phổi… +di chứng :xơ hóa,vôi hóa,xơ cứng màng phổi.giãn phế quản.- Lao thần kinh :liệt ,động kinh, mù, điếc,rối loạn tâm thần- Lao màng bụng: hình thành các dải xơ,dây chằng gây tắc ruột-Lao tiết niệu-sinh duc:xơ hóa thận , THA ,vô sinh-Lao xương khớp-Lao hạch

-tăng đề kháng với mọi thể lao-giảm nguy cơ nhiễm lao 50%,đb ngăn chặn được thể lao sơ nhiễm và liền sau sơ nhiễm

Chú ý:-Bảo vệ 80% trẻ được tiêm chủng - Kéo dài tối đa 15 năm -Sau khi tiêm BCG nếu trẻ tiếp xúc lâu dài với nguồn lây mạnh thì vẫn bị mắc lao nhưng thường bị các thể lao nhẹ

Chỉ định chủng BCG:Tốt nhất là trước khi nhiễm lao,khi IDR (-) .Tuy nhiên nếu đã nhiễm lao vaccine BCG không gây tác hại
Chống chỉ định: rất hiếm, không có CCĐ tuyệt đối-Không nên tiêm khi: K,bệnh BC,bệnh mạn tính:viêm thận
mạn,thận hư..,giảm lympho…
-Trì hoãn : suy dinh dưỡng,bệnh lý cấp tính,bệnh ngoài da đang tiến triển
-Đối với người HIV/AIDS+Trẻ HIV không biểu hiện Ls hoặc có Mẹ nhiễm HIV kèm
nguy cơ nhiễm lao cao tiêm BCG ngay khi sinh.+Trẻ HIV có biểu hiện lâm sàng của AIDS thì không nên
tiêm vì không dự phòng được lao
Diễn tiến-Biến chứng Diễn tiến: 1 tháng- 1,5 tháng- 2.5 tháng
Biến chứng:
-NK tại chỗ:nốt loét lớn,làm mủ trong nhiều tuần đắp gạt mỏng tẩm S/H,không băng kín.
-Viêm hạch :<1%,thường sau 3-6w to dần trong 3w rồi nhỏ dần và lành tự nhiên(ở nách/trên xương đòn,đau/ không,di động,cứng ,mêm dần ,dò…)
-ở bệnh nhân suy giảm miễn dich nặng có thể lan truyền theo đường máu đi xa hoặc tử vong (hiếm)
Tiêm trong da
là tiêm một lượng thuốc rất nhỏ (1/10ml) vào lớp thượng bì. Thuốc được hấp thu rất chậm.Chỉ định:_ Tiêm vaccin BCG
_ IDR _ test trước tiêm thuốc (những thuốc dễ gây sốc phản vệ như: penicilin, streptomycin…)
-Dụng cụ: Bơm tiêm nhỏ có vạch chia 1ml,đầu vát ngắn.
-Vị trí tiêm : thường 1/3 trên, mặt trước trong cẳng tay
Kĩ thuật :_ Sát khuẩn_ Để mũi vát của kim ngửa lên trên. _ Khẽ gảy mũi kim vào mặt da, khi mũi kim đã bén vào mặt da thì hạ thấp bơm tiêm xuống sát mặt da (10 – 15 độ)
_ Rồi đẩy nhẹ kim cho ngập hết mũi vát của kim_ Kiểm tra thuốc có vào đúng trong da không bằng cách: nhìn chỗ tiêm nổi cục to bằng hạt ngô, sần da cam. Và cảm giác lúc đẩy thuốc thấy chặt tay_ Khi rút kim ra thì đồng thời kéo chệch căng da chỗ tiêm vài giây cho thuốc khỏi trào ra

Trước nhiễm lao (ngăn xâm nhập)và sau nhiễm lao (ngăn lan tràn) -Đối tượng tất cả người nhiễm HIV đã được sàng lọc không mắc lao tiến triển,không cần làm IDR
-INH 5mg/kg/j (<300mg/j) 1lần/j trong 9 tháng kèm vitB6 25mg/j
Hóa dự phòng
Tăng sức đề kháng (bệnh kèm,độc hại,mức sống)

Tuberculine là một sản phẩm chuyển hóa trực khuẩn lao.
Tuberculine dùng trong y tế hiện nay là dẫn chất protein tinh khiết (tuberculine PPD) được chiết xuất từ sản phẩm chuyển chịu nhiệt của môi trường lỏng nuôi vi khuẩn lao.
