Neurological Complications
Respiratory Complications
Respiratory Complications
Neurological Complications
Delirium third most common complication seen in older patients after orthopedic surgery
• hypoxemia,
• hypotension
• hypervolemia
• abnormal electrolytes
• infection
• sleep deprivation
• pain
• administration of benzodiazepines and anticholinergic medications
How to prevent
• identifying risk factors and vulnerable and affected patients early
• preserving orientation
• early mobilization
• adequate pain control
• maintenance of normal sleep cycles
• avoidance of psychotropic medications
FAT EMBOLISM SYNDROME
Gurd's Diagnosis of Fat Embolism Syndrome
respiratoryneurologic
hematologiccutaneous signs
one major finding plus four minor criteria and evidence of
fat microglobulinemia was required
Schonfeld Fat Embolism Syndrome Index
petechial rash is pathognomonic of FES, with the rash usually present on the conjunctiva, oral mucosa, and skin folds of the
neck and axillae
Deep vein thrombosis(DVT) and pulmonary embolism

SURGERY
Common complication after spinal anesthesia and management
• Postdural puncture headache
• Respiratory depression
• Backache
• Nausea/vomitting
• Urinary retention
Prevention
• Orienting a needle bevel parallel with the axis of the spine
• Small needle
• Pencil point needle
Post–Dural Puncture Headache
TreatmentConservative management
• supine positioning, hydration, caffeine, and oral analgesics
Specific treatment
• Epidural blood patch is the definitive therapy for post-dural puncture headache
RESPIRATORY(associated with neuraxial opioids)
Incidence0.01-3.0%
No difference in frequency of RC depression compare with parenteral opioids
Less somnolence and sedation
Patient at risk : obesity, obstructive sleep apnea, coexisting disease
COMPLICATIONS
BACKACHE
• 25% of all surgical patients undergoing anesthesia, regardless of anesthetic technique, experience backache, the incidence of which increases to 50% when surgery lasts 4 to 5 hours
• neuraxial techniques do not play a role in the development of back pain after delivery
COMPLICATIONS
NAUSEA AND VOMITING
mechanisms
• direct exposure of the chemoreceptive trigger zone in the brain to emetogenic drugs (e.g., opioids)
• hypotension associated with generalized vasodilation
• gastrointestinal hyperperistalsis secondary to unopposed parasympathetic activity
COMPLICATIONS
URINARY RETENTION
• one third of patients after neuraxial anesthesia
• blockade of the S2, S3, and S4 nerve roots inhibits urinary function as the detrusor muscle is weakened
• administration of intrathecal morphine is strongly associated with this complication
COMPLICATIONS
PRURITUS
• most common side effect related to the intrathecal administration of opioids, with rates between 30% and 100%
