Introducción
Frecuentemente los estudios a las poblaciones depacientes con alguna enfermedad crónica se limitana las enfermedades mas comunes, dejando de ladoa los estudios de las minorías.
El Parkinson es una enfermedad neurodegenerativacon una prevalencia de 1 a 2% en personasmayores de 65 años, un 15% de las personas enedades entre los 65 y los 74 y más de la mitad detodos los mayores de 85 años.
El Parkinson en México
Se estima que en nuestro país existenalrededor de 400 mil personas que padecenesta enfermedad con una incidencia estimadade 44 casos nuevos al año por cada 100 milhabitantes.
Este padecimiento altera de formatrascendental la vida de los individuos que lopadecen y por lo general desencadenaconsecuentes trastornos físicos y emocionalesen los mismos.
Es un Sx que comprende cuatro signos
cardinales
Bradicinesia
Rigidez muscular
Temblor en reposo
Alteración del equilibrio
Generalidades clínicas
Manifestaciones extramotoras
Trastornos psiquiátricos fisiopatológicosDepresión
Ansiedad

La variante mas comun de laenfermedad es la idiopática, descrita porjames parkinson en 1817 con el nombrede paralisis agitante
Sin tto la enfermedad evoluciona en unlapso de 5 a 10 años hasta llegar a unestado de acinesia rígida en que lapersona queda incapacitada para cuidarde si misma
El déficit dopaminérgico en el mal deParkinson proviene de la desaparición de lasneuronas de la pars compacta de la sustancianegra, que envía axones al cuerpo estriado
Los conocimientos actuales de los aspectosfisiopatológicos de PD se basan en que elcontenido de DA disminuye mas del 80% quecorresponde a la perdida de neuronas de lasustancia negra; esto sugiere que lareposición de DA podría restaurar la función
Fisiopatología
El neoestriado es la estructura principal de losganglios basales y recibe estímulosglutamatenérgicos excitadores de muchaszonas de la corteza
Gran parte de las neuronas dentro del cuerpoestriado pertenecen a proyecciones queinervan otras estructuras de ganglios basales
Las interneuronas conectan las neuronasdentro del cuerpo estriado pero no establecenproyecciones mas alla de sus bordes
Mecanismo nervioso del mal de
Parkinson
La Ach y los neuropéptidos son utilizadoscomo transmisores por dichas interneuronasestriatales
El flujo de salida del cuerpo estriado sigue dosvías diferentes llamadas directa e indirecta
La directa esta formada por neuronas en elcuerpo estriado que establecen proyeccionesdirectamente con las fases de generación deimpulsos de los ganglios basales
La vía indirecta esta compuesta por
neuronas del cuerpo estriado que
proyectan sus extensiones hasta la
porción externa del globo pálido
