We employ a team of mainly local people called Community Navigators to help improve access to services for the local community. We do this by working with people to:

� Achieve their own health and wellbeing goals

� Find out about what services are available locally

� Navigate through services and access those that meet your needs

� Get practical support from other people who live in the local community

Community Navigators will support you in improving your health, wellness and connection to your community. We help you to identify priorities and develop plans that you think may help you lead a happy and healthy life.

Our support includes (but is not limited to):

� One to one community outreach

� Practical support to solve real life difficulties, such as housing, employment, managing health conditions, etc

� Emotional support, including building new social networks

� Information, advice and guidance

� Signposting to appropriate services and local community resources

� Links to local people and groups so that you don’t feel isolated

� Practical support to access other services and community resources

� Help to build confidence and self-esteem

� Assistance with improving work and life skills

� Wellbeing coaching to help you toward achieving your own health and wellbeing goals whatever they might be

� Mentoring and befriending

� Practical help from local volunteers with anything from gardening to help with computers or even dog walking. Please see our Time bank leaflet for more information

The frequency, nature and duration of support is agreed with you and tailored to your specific needs.

Volunteer Community Mentors work alongside Community Navigators and provide additional support.

About the Community Navigator ServiceFinding out which local services can help and support you is sometimes difficult to do. This is where we can be of assistance. Our aim is to help you to improve your health and wellbeing through staying independent and becoming socially active within your local community.

For up to 12 sessions, we provide a personalised, responsive, flexible and friendly community-led service that enables you to access the care and support you need within your community.

We can work with you to design a high quality, individual support package, enabling you to learn new skills, focus on personal goals and make the most of the available opportunities within your local community. We can help you to take control and access the services you need to improve your quality of life and face the future with confidence.
