Quizá el factor más determinante de la supervivencia global de la paciente NO SEA el primer intervalo libre de progresión (PFS-1) si no lo que queda más allá…es decir la SPP*.
Cuanto más se alargue la SPP (tras la primera linea de tratamiento) tanto más se difumina la relación entre PFS y OS.
Resulta imprescindible un seguimiento más largo de todas las pacientes en los diferentes ensayos clínicos.
En el modelo matemático estudiado, si SPP es de 12 meses, la relación entre PFS y OS obliga a tamaños muestrales > 1000 enfermas y a seguimientos de 44 meses.

We believe the present META-ANALYSIS provides four useful messages to the clinical and scientific community: (1) in metastatic breast cancer, taxanes do not improve survival when
compared with anthracyclines, either as single agents, or in anthracycline combinations;
(2) taxanes in combination with anthracyclines modestly improve response rates and PFS;
(3) more attention needs to be paid to cross-over from an older agent to a newer drug when designing clinical trials and reporting their results; and
(4)clinical trial databases should be co-owned by academia and industry, to facilitate analyses beyond the primary report or publication.

¿Duración de la primera línea de tratamiento? Punto oscuro: - esquemas rígidos - sesgo si tratamiento hasta progresión o toxicidad
Como el «mantenimiento», el efecto de la duración del primer tratamiento se refleja en ventajas en PFS, pero menos en OS.
Results: Eleven eligible studies, including 2,62 patients were identified. Overall longer CT duration was associated with a 34% reduction in the hazard of progression (HR 0.66, CI 0.61-0.72; p<0.0001) and with an 8% reduction in the hazard of death (HR 0.92, CI 0.84-1.00; p=0.06). The magnitude of this effect was remarkably similar across groups of trials, both in terms of PFS and OS, suggesting its independence of study design (p for interaction 0.60/0.10), duration of maintenance CT (p for interaction 0.70/0.92) and concomitant maintenance endocrine therapy (p for interaction 0.14/0.63). Conclusions: Longer duration of first line CT in MBC is associated with a significant improvement in PFS and with a marginal improvement in OS.
