Tubuh manusia secara konstan terekspose oleh mikroorganisme
Melalui aktivitas sehari-hari, kita terekspos pada mikroorganisme
yang tidak terhitung jumlahnya di lingkungan kita
Tubuh beraksi sebagai inang dan menyediakan lingkungan yang
‘favorable’ untuk perkembangan banyak mikroorganisme.
Kaya dengan nutrien organik dan faktor pertumbuhan
Menyediakan pH yang terkontrol Tekanan osmosis yang terkontrol
Temperatur

Mutualism.
In a mutualist relationship, both the host and the microbe benefit from the
relationship.
The best example if this is E.coli. This organism lives in the intestines, where
it receives nourishment, and in turn produces Vitamin K, which the human body requires for the process of blood
clotting.

A commensalist relationship is where one partner of the relationship benefits, and the
other partner is neither benefitted nor harmed.

Parasitism.
A parasitic relationship is where one organism benefits at the expense of the host. The cost to the host can vary from

Pathogenic.
A pathogenic relationship is where an organism causes damage to the host during infection.
An Opportunistic Pathogen causes disease in a host that is physically impaired or debilitated. Normally the
opportunist organism is harmless, but it takes advantage when the hosts defences are impaired, for example
when the normal flora have been destroyed by antibiotics, or when the immune system has been
suppressed by drug treatment or by other illnesses.

Membran mukosa:terdiri dari single atau multiple layers of epithelial cells, yang terdiri dari sel yang tersusun secara ketat dan berhubungan

glikoprotein yang soluble dan viscous yang disebut dengan mucous.

2. Berkompetisi dengan patogen dalam ketersediaan makanan, sehingga patogen yang menjadi penginvasi
tidak dapat bermultiplikasi3. Beberapa flora normal mengeluarkan zat antibakteri (bacteriocins), sebagai efek samping metabolisme,
sehingga memberikan efek antibiotik
Yang menguntungkan dari flora normal…..
Why do the normal flora not induce an immune response from
the host? The short answer to this question is that they do. Antibodies to the normal flora exist in our bodies, but at lower concentrations than would exist for pathogenic bacteria. They provide a "sparring partner" for our bodies that keeps our immune systems in tune.
The precise role that our immune systems take in regulating the
populations of the normal flora is not known.
the presence of microbes is not without a cost. Their metabolism allows them to
attack a broad variety of compounds in our diets, some of which they can convert to compounds that are harmful to us. They
can be opportunistic pathogens whenever luck or injury allows them into regions of the bodies where they are not wanted.
Ternyata, flora normal tak selamanya menguntungkan……
Kelompok mikroorganisme menggunakan sumber langsung atau tidak langsung dari tubuh
untuk berkolonisasi, menginvasi atau merusak tubuh manusia,
sehingga menyebabkan penyakit infeksi.
Yang patogen…..
Mikroorganisme jenis, ini, disebut dengan patogen, memakai
beberapa strategi untuk meningkatkan akses nutrien di
dalam inang. Strategi ini meliputi produksi struktur perlekatan yang spesial, faktor pertumbuhan yang unik, dan beberapa menghasilkan
potent biological toxin
Patogenesitas berbeda-beda dan bervariasi untuk setiap patogen.
Ukuran kuantitatif dari patogenesitas disebut dengan
virulensi
Virulensi diekspresikan sebagai jumlah sel inang yang terkena
dampak akibat respons patogen di dalam inang dalam suatu
periode waktu tertentu.
* Tubuh kita telah mengembangkan sistem pertahanan yang efektif
untuk menekan atau menghancurkan mikroba yang
masuk. * membuat penyakit infeksi oleh
mikroba relatif jarang terjadi.
Kemampuan parasit untuk menyebabkan kerusakan inang
sifatnya bervariasi, seperti resistensi atau susceptibility dari
inang pada parasit.
Tidak satupun virulensi patogen atau resistensi relatif inang merupakan faktor konstan. Host-parasite interaction merupakan hubungan yang sangat dinamik, antara dua organisme, dan dipengaruhi
perubahan kondisi baik patogennya maupun inangnya.

Koch postulat : apabila seseorang terekspos pada mikroba penyebab
penyakit, maka orang tersebut definitely akan terserang penyakit
Ternyata…… Tidak semua manusia itu memiliki response yang sama terhadap mikroba
(perbedaan susceptibility) Tidak semua galur (strain) mikroba
tertentu menyebabkan penyakit
Contoh : Vibrio cholerae, terbagi atas banyak serogroup dan serotipe, juga biotipe, tapi yang menyebabkan penyakit? Beberapa saja…..
Hepatitis B Virus, ada genotipe A-G, tapi tiap negara/area memiliki tipe yang spesifik
Salmonella typhi, ada satu strain yang hanya hidup di Indonesia dan tidak ada di tempat lain….
Infeksi dan penyakit
Infeksi : segala situasi dimana mikroorganisme ada dan
berkembang (berkolonisasi) di dalam inang, apakah inang
dirusak atau tidak
Infeksi yang menghasilkan gejala disebut dengan penyakit
Penyakit adalah kerusakan atau luka pada inang yang merusak
fungsi pada inang.
Infeksi tidak sama dengan penyakit, sebab pertumbuhan
mikroorganisme di dalam inang tidak selalu menyeabkan
kerusakan inang.
Anggota normal flora mungkin memproduksi infeksi, tetapi tidak
menyebabkan penyakit. Meskipun demikian,
mikroorganisme yang flora normal kadang menyebabkan
penyakit apabila resistensi inang berkurang, seperti aids
Asymptomatic carrier : Orang yang diinfeksi tetapi tidak
terdeteksi adanya gejala Symptom : gejala
Virulensi/patogenesitas : kemampuan mikroorganisme untuk menghasilkan penyakit
Faktor virulensi : produk/strategi yang digunakan mikroorganisme
agar berkontribusi terhadap virulensi/patogenesitasnya.
Kuntasisasi virulensi : LD50
Dua pengukuran yang paling umum untuk menyatakan virulensi adalah
dengan menggunakan model binatang yaitu LD50.
LD50 adalah jumlah bakteri yang diperlukan untuk membunuh sekitar
50% binatang
Plot antara jumlah binatang yang terinfeksi atau terbunuh terhadap
jumlah bakteri yang ada di inokulum adalah sigmoidal, tidak
linier. Untuk melokasikan 50% secara tepat, digunakan metoda
statistik.
Semakin kecil LD50 atau ID50, menjadi lebih mematikan atau infeksi suatu bakteri, semakin
kecil semakin buruk
Competition assays :Binatang diinfeksi dengan bakteri mutan
dan wild type. Setelah diberikan waktu beerapa lama, sampel diambil dari
berbagai bagian binatnag, kemudian rasio antara mutan dan wild type
dibandingkan. Kalau rasionya sama dengan dosis infeksi, berarti mutan
tidak memberikan efek virulensi, tetapi kalauwild type tidak dapat
berkompetisi dengan mutan, mutasi memiliki efek negatif.

Normal microbiota : resident microbiota
Resident microbiota: population of normally non virulent bacteria
found routinely in a specific site of the body of most normal adults
The normal flora of the skin consists of either transient or resident of
microorganisms. The skin is continually being
inoculated with transient microorgansims, virtually all of which
are unable to multiply, not merely survive, on this skin.
Resident microorganisms are able to multiply and ussually die
(Madigan&Martinko, Brock,Biology of Microorganisms, 2006)

produce infections but seldom cause disease. However, the normal flora microorganisms if
a host resistance is compromised, as happens in disease such as cancer and AIDS

Colonization is not synonyms with disease, nor is it necessarily
synonyms with infection. Many bacteria that colonize the human
body do not cause disease, and thus do not considered to be infecting it.
The distinction between colonization and infection has been blurred by the
realization that some of the supposedly noninfectious bacteria that normally inhabit the human
body can cause disease in surgical patients or immunosuppressed
people. So, the terms colonization , infection and disease may vary in their aplicability depending on the
status of the person colonized.
