•TG (folículos)•Yodo (TPO, pendrina)•TBG
TIROIDES(T4, T3)
HIPÓFISIS(TSH)

(TRH)

HIPOTIROIDISMO CONGÉNITO
![Page 4: H I P O T I R O I D I S M O2011.Ppt](https://reader034.fdocument.pub/reader034/viewer/2022051515/556e9c6ad8b42ad7458b5373/html5/thumbnails/4.jpg)
![Page 5: H I P O T I R O I D I S M O2011.Ppt](https://reader034.fdocument.pub/reader034/viewer/2022051515/556e9c6ad8b42ad7458b5373/html5/thumbnails/5.jpg)
• Tiroides (primario)• Hipófisis (secundario)• Hipotálamo (terciario)• Transitorio– medicamentos anti-tiroideos maternos– exceso de yodo – anticuerpos maternos contra TSHr
• 80% ausencia, ectopia, hipoplasia• 15% defecto de síntesis • Screening neonatal
![Page 6: H I P O T I R O I D I S M O2011.Ppt](https://reader034.fdocument.pub/reader034/viewer/2022051515/556e9c6ad8b42ad7458b5373/html5/thumbnails/6.jpg)
• Síntomas/ Exploración– Normal– Fontanela posterior grande (>1 cm)– Centros epifisiarios hipoplásicos o ausentes en
fémur distal y tibia proximal – Ictericia (>1 semana) – Menos frecuentes: • Macroglosia, llanto ronco, hernia umbilical
agrandada, estreñimiento, piel seca
![Page 7: H I P O T I R O I D I S M O2011.Ppt](https://reader034.fdocument.pub/reader034/viewer/2022051515/556e9c6ad8b42ad7458b5373/html5/thumbnails/7.jpg)
• Diagnóstico– T4 o TSH; 24-48 horas posparto– Gammagrama tiroideo 99Tc (ausencia o presencia)

![Page 9: H I P O T I R O I D I S M O2011.Ppt](https://reader034.fdocument.pub/reader034/viewer/2022051515/556e9c6ad8b42ad7458b5373/html5/thumbnails/9.jpg)
![Page 10: H I P O T I R O I D I S M O2011.Ppt](https://reader034.fdocument.pub/reader034/viewer/2022051515/556e9c6ad8b42ad7458b5373/html5/thumbnails/10.jpg)
HIPOTIROIDISMO ADQUIRIDO
![Page 11: H I P O T I R O I D I S M O2011.Ppt](https://reader034.fdocument.pub/reader034/viewer/2022051515/556e9c6ad8b42ad7458b5373/html5/thumbnails/11.jpg)
![Page 12: H I P O T I R O I D I S M O2011.Ppt](https://reader034.fdocument.pub/reader034/viewer/2022051515/556e9c6ad8b42ad7458b5373/html5/thumbnails/12.jpg)
•CAUSAS: •Cuadros posvíricos•Radiación•Exceso de yodo (efecto de Wolff-Chaikoff)•Deficiencia de yodo•Disfunción hipotalámica o hipofisiaria•Autoinmunitaria o linfocítica
![Page 13: H I P O T I R O I D I S M O2011.Ppt](https://reader034.fdocument.pub/reader034/viewer/2022051515/556e9c6ad8b42ad7458b5373/html5/thumbnails/13.jpg)
•SÍNTOMAS/ EXPLORACIÓN•Velocidad crecimiento reducida•Retraso de edad ósea•Mixedema•Estreñimiento•Intolerancia al frío•Pubertad tardía o precoz (pseudopubertad)•Bocio (linfocítica crónica; simétrica, firme, no nodular)•Hernia umbilical•Hipotonía•Galactorrea
![Page 14: H I P O T I R O I D I S M O2011.Ppt](https://reader034.fdocument.pub/reader034/viewer/2022051515/556e9c6ad8b42ad7458b5373/html5/thumbnails/14.jpg)
![Page 15: H I P O T I R O I D I S M O2011.Ppt](https://reader034.fdocument.pub/reader034/viewer/2022051515/556e9c6ad8b42ad7458b5373/html5/thumbnails/15.jpg)
DIAGNÓSTICO•HT primario: disminuyen T4 y FT4, aumenta TSH•HT secundario/terciario: disminuyen FT4, TSH; TSH normal•Anticuerpos anti-TPO y anti-TG•Prolactina elevada, GH disminuida
TRATAMIENTO•L-tiroxina (75–100 mcg/m2/d); neonatos (10–15 mcg/kg/d)
