A Word from the President of the Council

on Senior Services By Suzi Jam� The Council on Senior Services (COSS) is a community council

of professionals who work for and volunteer for agencies and busi-nesses that serve senior citizens. The goal of COSS is to improve access to services for seniors in Allen County. This directory has been developed to educate senior citizens and professionals about the numerous services that exist.

Proceeds from the advertising sales in this directory pay for printing of the book and for grants to organizations that submit projects created for the health, education and/or welfare of se-niors in our community. Grants awarded in 2011 included Adam and Bruce Apartments, Community Transportation Network, Al-len County Council on Aging, Homebound Meals, A Day Away Adult Daycare, Allen County TRIAD, Aging & In-Home Services, The Volunteer Center @ RSVP, Turnstone Center for Children and Adults with Disabilities, Northeast Indiana Radio Reading Services, League for the Blind and Disabled, Long Term Care Ombudsman, In-Home Hair Care, Hope Alive, ResCare Adult Day Services and Health Visions.

In 2011, over 41,000 older adults and persons with disabilities, as well as their fami-lies and supporters, received services from or through Aging & In-Home Services of Northeast Indiana (AIHS). As the Area III Area Agency on Aging as designated by the U.S. Administration on Aging and the State of Indiana, AIHS is the primary resource for aging and disabled populations and the largest funder of services including support for the Council on Aging in the following counties: Allen, Adams, DeKalb, Hun-tington, LaGrange, Noble, Steuben, Wells, Whitley. KEY SERVICES

Anterior-Approach Hip Replacement The hip is replaced through a single incision with no detachment of muscle from the pelvis to the femur. This approach has facilitated shorter hospital stays and shorter recovery times.

Physical and Occupational Therapy Physical and occupational therapists work to help restore patients to the same quality of life they lived prior to their illness, accident or surgery. Physical therapists use exercise and physical activity to condition patients’ muscles to improve their activity level, while occupational therapists focus on functionality of the hands, arms and upper extremities and the activities of daily living.

Sports Medicine The Orthopedic Hospital has teamed up with Fort Wayne Orthopedics to create a sports medicine program that treats all athletes, professionals and weekend warriors with the same or next-day priority of care.

Spine Surgery Spine surgeries, including artificial disc procedures, spinal fusions and fracture repairs, are the second most common type of surgery performed at The Orthopedic Hospital. We also offer pain management treatments for back pain and participate in national spine research protocols to offer the latest in techniques and technologies.

Hand and Foot Surgery The Orthopedic Hospital specializes in surgically repairing hands damaged by conditions such as arthritis, lupus, tendonitis, genetic abnormalities and traumatic and workplace injuries. Ankle, heel, toe, forefoot and stress fractures, as well as broken ankles and bones, are all regularly treated at the facility.

Pain Management Patients living with back and/or leg pain caused by injuries or genetic conditions can be treated on an outpatient basis. Treatments take approximately 30 minutes, and a physician referral is required.

THE PROBLEMIt is common for a person with memory impairment (Such as: Dementia or Al-zheimer’s Disease) to wander and roam. This can happen without prior warn-ing and be dangerous to the patient and worrisome for caregivers.

THE SOLUTIONIn cooperation with local healthcare and law enforcement agencies, the SAFENET Program, can assist in the return of an individual who has wandered. This FREE service will help family members and the community to keep your loved one safe.

Veterans’ Benefits for Long Term CareThe VA provides home and community-based services to veterans. These medically necessary services include home care, hospice, respite care, assisted living, domiciliary care, geriatric assessments and nursing home care. The veteran must be enrolled in VA’s health care system. Some of these services may be offered to veterans who do not have service-connected disabilities but who may qualify because of low in-come or because they are receiving pension income from VA. Call the VA Northern Indiana Health Care System at 426-5431.

Aid and Attendance is an under used benefit. It is a special pension benefit for wartime veterans and their wives. Some who are not eligible for VA benefits because of income, may still be eligible for this benefit if they have large medical expenses. The benefit is for those receiving in-home care, inability to take care of bodily needs, or bedridden. They may also qualify because of mental injury or illness that requires regular assistance to protect them from the hazards in their daily environment. They may be living at home, in an assisted living facility, or a nursing facility. For additional information, call Allen County Veterans’ Services at 449-7861 or access the internet at www.va.gov.

Long Term CareLong term care is a variety of services that include medical and non-medical care to people who have a chronic illness or disability. It helps the individuals meet their health or personal needs. It often assists peo-ple with support services such as activities of daily living like dressing, bathing, and using the bathroom. These services can be provided in the home, in the community, in assisted livings, or in nursing facilities. Generally, Medicare does not pay for these long term care services, referred to as “custodial care.” Medicare pays only for medically neces-sary skilled nursing and therapies in the home and for skilled care in nursing facilities.

Medicaid and Long Term CareMedicaid is a combined State and Federal Government program that pays for certain health services and nursing home care for older people with low incomes and limited assets. It is an entitlement program which means that all who qualify for it will receive it. For information and to apply, call 1-800-403-0864 or access the internet at www.in.gov/fssa or www.ifcem.com.

Medicaid Waiver is the Medicaid program that pays for in-home long term care services. Income allowed in order to be eligible for this pro-gram is three times the amount allowed for the regular Medicaid pro-gram. This is not an entitlement program so there may be a wait

For over 46 years, the U.S. Administration on Aging (AoA) has pro-vided home and community-based services to millions of older persons through the programs funded under the Older Americans Act. President Lyndon Johnson on August 14, 1965, signed into law the Older Ameri-cans Act (OAA). The OAA set out specific objectives for the maintaining the dignity and welfare of older individuals and created the primary vehicle for organizing, coordinating and providing community-based services and opportunities for older Americans and their families.

The original legislation established authority for grants to States for com-munity planning and social services, research and development projects, and personnel training in the field of aging. The law also established the Administration on Aging (AoA) to administer the newly created grant programs and to serve as the Federal focal point on matters concerning older persons. The mission was later expanded to include those individu-als with disabilities.

Although older individuals and individuals with disabilities may receive services under many other Federal programs, today the OAA is consid-ered to be the major vehicle for the organization and delivery of social ad nutrition services to this group and their caregivers. It authorizes a aide array of service programs through a nation network of 50 states

agencies, 629 Area Agencies on Aging, nearby a national network of providers and a number of Tribal and Native Hawaiian organizations.As the Area III Area Agency on Aging as designated by the U.S. Admin-istration on Aging and Indiana Family and Social Services Administra-tion, Division of Aging, Aging & In-Home Services of Northeast Indiana (AIHS) is the primary resource for aging and disabled populations and the largest funder of services including support for the Council on Ag-ing in the following nine counties: Allen, Adams, DeKalb, Huntington, LaGrange, Noble, Steuben, Wells, Whitley.

Each year, AIHS completes an Area Plan with information on the needs and preferences of older individuals and persons with disabilities and their family caregivers in relation to a comprehensive, coordinated sys-tem for providing long-term care in home and community-based settings in AIHS’ nine county region. The AIHS Area Plan is part of the overall State Plan on Aging which is submitted to the Administration on Aging in accordance with provisions of the Older Americans Act.

Services under that plan include Meals on Wheels home-delivered meals programs, Senior Dining in congregate settings, transportation, adult day care, legal assistance, health promotion or chronic disease self-management programs, among others.

Locally, access to information and resources on services you need now or in the future, and how to pay for services, is available through Ag-ing & In-Home Services of Northeast Indiana’s Aging and Disability Resource Center. Aging and Disability Resource Centers (ADRCs) were created by AoA, in partnership with Centers for Medicare & Medicaid (CMS), to provide consumers and caregivers information on home and community-based long-term care services. ADRCs provide consumers information, options counseling, referral, assessment, educational and assistance in planning for the future needs. AoA is emphasizing the importance of integration of proven evidence-based health promotion

Safe & Independent at Homeinterventions, which can lessen disability related to chronic illnesses, prevent falls, and reduce the burden experienced by family caregivers of individuals who are older and/or disabled.

The ADRC operated by AIHS receives a wide range of calls. The most requested services are: Case Management; Personal Attendant Care/Home Health Aide; Homemaker/Chore; Meals on Wheels; Respite Care; Adult Day Services; Emergency Response Systems; Assistive Technology Assisted Living; Home & Vehicle Modification; Adult Foster Care.

The challenge to the individual and family is often in funding those ser-vices. ADRC staff are able to assist in informing about and applying for a variety of federal, state, local programs that assist with pay-ing for in-home services, such as Medicaid Aged & Disabled Waiver, CHOICE (Community Home Options to Institutionalized Care for the Elderly and Disabled. SSBG (Senior Service Block Grant, OAA (Older Americans Act), as well as outlining private-pay options. It may be a concerned neighbor or a family member who has a fear for their safety or a 9-1-1 call that is answered, the caller is unable to speak.

COSS Grant CriteriaThe program request must follow the Mission Statement of the Council on Senior Ser-vices…To promote the service and benefits of seniors in Northeast Indiana through a network of individuals and organizations providing some type of service to our matur-ing population.A maximum total amount will be awarded annually via the COSS grant process. The amount requested and the annual maximum award per organization is not guaran-teed. The awarded amount will be determined by the COSS Board of Directors.The focus of each request should mirror the purposes of the Council on Senior Services. These purposes include providing education, information and advocating for seniors. These purposes are to be accomplished using a grassroots approach. Grant recipients are notified and checks are awarded at the November membership meeting.In order to continue to qualify for subsequent grant awards, a written report evaluating the project will be required by the COSS Board of Directors by May 1st. There will also be an opportunity to provide an oral report to the general COSS membership at the May meeting.

Activities of Daily Living (ADL’s) - Bathing, dressing, eating, walking and all necessary activities required to function on a daily basis.

Advance Directives - Legal documents outlining how a person’s medical decisions are to be handled including living will, power of attorney for health care and health care representative.

Adult Protective Services (APS) - The official agency mandated to re-ceive investigate, and respond to complaints of neglect and abuse of the elderly.

Ambulatory - The ability to walk freely and independently.

Assisted Living (AL) - Housing for an elderly or disabled person that provides room, meals and various levels of care.

Assessment - An evaluation performed of a person’s mental, emotional and social capabilities.

Do Not Resuscitate (DNR) - A written and legal order that a patient does not want to be resuscitated.

Dementia - A group of symptoms that accompanies certain diseases. A loss of thinking, remembering, and reasoning. May also include behav-ioral changes and moods.

Durable Medical Equipment (DME) - Reusable medical equipment such as walkers, wheelchairs and hospital beds.

Durable Power of Attorney (DPOA) - A legal document giving another person authority to make financial and legal decisions if a person be-comes physically or mentally incapable.

Geriatrics - Medical specialty of treating the problems and diseases of old age.

Hospice - A way of providing care, support, and comfort to people ap-proaching end of life.

Medicaid - A jointly funded state and federal program to assist those with a financial need with medical care.

Medicare - A federal insurance program for those age 65 and over or disabled.

Memory Care - Special programs dedicated to caring for those with Alzheimer’s Disease, dementia or other cognitive impairments. Usually services include group activities, socialization and assistance with ADL’s.

Nurse Practitioner (NP) - A professional registered nurse who has spe-cial training to provide primary healthcare.

Palliative Care - Care provided to manage pain and make the patient as comfortable as possible.

Support Group - A group, usually family members or caregivers that meet for mutual support and to discuss practical means of addressing their loved one’s needs.

Long Term Care (LTC) - A term used to describe care given in the form of medical and support services to people who have lost some or all of their capacity to function without assistance.
