• Prevalencia 2 veces mayor que en la poblaciónjoven
• Hipotiroidismo clínico 2-7%
• Hipotiroidismo subclínico aumenta en mayores de 65 años 8-20%**

**Prevalencia depende de Ingesta de iodo: Aumenta en áreas de iodo suficiencia (EU 4.2 a 23.9%), Varia de acuerdo a rango TSH de referencia, presencia de autoinmunidad y es 8-10 veces mas frecuente en mujeres

• Adulto mayor : precaución por posibilidad de enfermedad coronaria subclínica
• Requerimientos mas bajos: disminución aclaramiento metabólico, disminución de masa magra, pero valorar interacciones con otros medicamentos
• VIGILANCIA ESTRECHA– 25-50 mcg/día
• Enfermedad Coronaria establecida – 12.5-25 mcg/día– LT4 aumenta demanda de O2
Ajustar dosis cada 6-8 semanas, dosis entre 12.5 y 25 mcg (NO realizar cambios grandes de dosis)
EVITAR SOBRETRATAMIENTO
