Abstract. Introduction: ameloblasts are cells responsible for the production and mineralization of the organic matrix of enamel through several stages: pre-secretory, secretory, transition, and maturation. The organic matrix components are produced in the secretory phase. In the maturation phase, the organic component is removed and the mineralization process starts. This process requires the involvement of matrix metalloproteinase 20 (MMP-20), also called enamelysin. Several studies have shown the presence of MMP-20 in tooth development and its relationship to alterations in enamel formation. The objective was: to classify the different studies and laboratory techniques used to demonstrate the involvement of enamelysin in tooth development and its relation to pathologies during enamel formation. Methods: a systematic review was conducted with the following bibliographic databases: PubMed, Science-Direct, Hinari, and SciELO, in order to classify the different studies related to the involvement of MMP-20 in tooth development and the methods to detect its expression, between the years of 2009 and 2014. Results and conclusions: in vitro models show that MMP-20 has specific cleavage sites for enamel matrix proteins. This process is altered by chemical composition, ions, and the presence of hydroxyapatite. Enamel morphology is altered in the knockout models. In human studies, MMP-20 has been associated with increased susceptibility to dental caries, enamel thickness, and dental agenesis.

rEsUmEn. Introducción: el ameloblasto es la célula encargada de la producción y mineralización de la matriz orgánica del esmalte. Atraviesa varias etapas: la fase pre-secretora, secretora, de transición y maduración. En la fase secretora se producen los componentes de la matriz orgánica. En la fase de maduración se elimina el componente orgánico y se inicia el proceso de mineralización. Este proceso requiere de la participación de la metaloproteinasa de matriz 20 (MMP-20) o también llamada enamelisina. Diversos estudios demuestran la presencia de MMP-20 en el desarrollo dentario y su relación con alteraciones en la formación del esmalte. El objeto fue clasificar los diferentes estudios y técnicas de laboratorio empleadas que demuestren la participación de enamelisina en el desarrollo dentario y su relación con patologías en la formación del esmalte. Métodos: se realizó una revisión sistemática de la literatura con las siguientes bases bibliográficas: PubMed, Science-Direct, Hinari y SciELO, con el fin de clasificar los diferentes estudios relacionados con la participación de MMP-20 en el desarrollo dental y los métodos utilizados para detectar su expresión, entre los años de 2009 a 2014. Resultados y conclusiones: los modelos in vitro evidencian que MMP-20 tiene sitios específicos de escisión para las proteínas de matriz de esmalte. Este proceso se ve alterado por la composición química, iones, y la presencia de hidroxiapatita. En los modelos knockout la morfología del esmalte está alterada. En los estudios en humanos, se ha relacionado la MMP-20 con una mayor susceptibilidad de caries dental, el grosor completo de esmalte y agenesias dentales.

introdUcción
Amelogénesis
La morfogénesis de los órganos dentarios inicia a la sexta semana de vida intrauterina en seres humanos (aproximadamente a los cuarenta y cinco días), y en el ratón inicia en el estadio E12.51. El primer indicio consiste en la diferenciación de la lámina dental o listón dentario, originado del ectodermo que reviste la cavidad oral. El ectomesénquima induce a las células basales del epitelio oral a proliferar y formar dos nuevas estructuras: la lámina vestibular y la lámina dentaria. La lámina vestibular formará el surco vestibular y la lámina dentaria dará origen a los crecimientos epiteliales correspondientes a los futuros dientes; 20 deciduos y 32 gérmenes de dentición permanente. De acuerdo a su morfología, los gérmenes dentales seguirán una evolución en: estadio de brote macizo (o yema), estadio de casquete, estadio de campana y estadio de folículo dentario, terminal o maduro.
Es en el estadio de campana el órgano del esmalte pre-senta una nueva capa, el estrato intermedio, ubicado en-tre el retículo estrellado y el epitelio interno. Al finalizar esta etapa de campana, inicia la histogénesis o aposi-ción de los tejidos duros dentarios (dentina y esmalte). Las células que conforman el epitelio interno, también llamadas preameloblastos, se diferencian en ameloblas-tos jóvenes y serán las encargadas de la producción del esmalte.
El esmalte, también conocido con el nombre de sustan-cia o tejido adamantino, está ubicado en la porción co-ronal cubriendo la dentina subyacente. Se origina del ór-gano del esmalte, y es el tejido más duro del organismo. Está constituido por los prismas de esmalte, los cuales están altamente mineralizados, ubicados desde la cone-xión amelodentinal (CAD) a la superficie externa o libre en contacto con la cavidad oral. La matriz inorgánica del esmalte la conforman los cristales de hidroxiapatita y re-presenta un 95%. Estos están constituidos por fosfato de calcio y se encuentran densamente compactados. La matriz orgánica del esmalte es de naturaleza proteica;

Amelogenesis
The morphogenesis of dental organs begins during the sixth week of intrauterine life in humans (approximately at forty-five days), and in mice it begins during the E12.5 stage.1 The first indication is the differentiation of dental lamina or slat tooth, originated from the ectoderm that covers the oral cavity. The ectomesenchyme induces oral epithelial basal cells to proliferate and form two new structures: the vestibular lamina and the dental lamina. The vestibular lamina forms the vestibular groove while the dental lamina generates the corresponding epithelial growth of future teeth: 20 deciduous teeth and 32 germs of permanent teeth. According to their morphology, dental germs will continue their evolution in these stages: stage of massive outbreak (or yolk), cap stage, bell stage, and stage of tooth follicle, either terminal or mature.
It is at the bell stage the enamel organ presents a new layer—the intermediate layer—located between the inner epithelium and the stellate reticulum. Histogenesis or apposition of the dental hard tissues (enamel and dentin) begins at the end of the bell stage. Cells forming the inner epithelium, also called pre-ameloblasts, differentiate from young ameloblasts and will be responsible for enamel production.
Enamel, also known as adamantine substance or tissue, is located in the coronal portion covering the underlying dentin. It originates from the enamel organ, and is the hardest tissue of the body. It consists of enamel prisms, which are highly mineralized, located from the dentin-enamel junction (DEJ) to the external surface in contact with the oral cavity. Hydroxyapatite crystals form the inorganic enamel matrix and represents 95% of it. These are composed of calcium phosphate and are densely compacted. The organic matrix of enamel is protein in nature;

sin embargo, en su composición química no participa el colágeno. Está en menor proporción y representa el 0,36-2%.
El estadio de campana será de gran importancia para la formación de la morfología coronal por acción de se-ñales específicas del ectomesénquima adyacente sobre este epitelio interno. Este patrón coronal se determinará previo a la aposición y mineralización de los tejidos den-tales.
El ameloblasto es la célula encargada de la secreción de la matriz orgánica, y durante la formación del ger-men dentario atraviesa una serie de etapas o estadios, las cuales se caracterizan por presentar cambios funcio-nales y ultraestructurales que dependen de la actividad celular, de acuerdo a los procesos de formación o ma-duración del esmalte. Por tal motivo encontramos cuatro etapas importantes para el desarrollo del esmalte: la fase pre-secretora, secretora, de transición y maduración.2
El desarrollo del esmalte o amelogénesis comprende dos etapas: la primera es la formación de una matriz orgánica extracelular y la segunda es el proceso de mineralización de esta matriz, la cual comprende a su vez la formación y elongación de los cristales y la eliminación de la matriz orgánica y la maduración de los cristales.
Los ameloblastos se diferencian a partir del epitelio inter-no del órgano del esmalte. Antes de que se lleve a cabo este proceso, se requiere de la presencia de la dentina. A este estadio se le denomina presecretorio, donde antes de la formación mineral se requiere la deposición de predentina por los odontoblastos en la futura unión amelodentinal.
En el estadio secretorio, se caracteriza porque los preameloblastos se transforman en células secretoras, diferenciadas, muy especializada y ha perdido la capacidad de hacer mitosis. Son células cilíndricas de 60 μm de altura aproximadamente. A nivel ultraestructural presenta abundantes mitocondrias, complejo de Golgi, RER distribuido por toda la célula y más desarrollado en el polo proximal. Presenta microfilamentos de tubulina, α-actinina, vinculina y prequeratinas. Además, presenta vesículas llamadas

Ameloblasts are cells responsible for the secretion of organic matrix, and during the formation of the tooth germ they go through a series of steps or stages, which are characterized by functional and ultrastructural changes dependent on cell activity, according to the formation processes or enamel maturation. This is why there are four important stages in the development of enamel: pre-secretory, secretory, transition, and maturation.2
Enamel development or amelogenesis includes two stages: a) the formation of extracellular organic matrix and b) the process of matrix mineralization, which at the same time includes the formation and elongation of crystals and the elimination of organic matrix and crystals maturation.
Ameloblasts differentiate from the inner epithelium of the enamel organ. For this process to take place, it requires the presence of dentin. This stage is called pre-secretory, which prior to the formation of mineral requires pre-dentine deposition by odontoblasts in the future dentin-enamel junction.
During the secretory stage, pre-ameloblasts are transformed into secretory differentiated cells, which are highly specialized and have lost the ability to undergo mitosis. They are columnar cells of nearly 60 μm in height. At the ultrastructural level, there are abundant mitochondria, Golgi complex, and RER distributed throughout the entire cell and more developed in the proximal pole. It presents microfilaments of tubulin, α-actinin, vinculin, and pre-keratins. In addition, it has vesicles called

cuerpos ameloblásticos o cuerpos adamantinos, las cuales son formaciones de tipo granular, y son precursores de la matriz orgánica del esmalte. Su contenido no se conoce con exactitud, se cree que son de naturaleza proteica y algunos autores consideran que pueden tener sales minerales cálcicas en forma soluble.
Una vez estos cuerpos ameloblásticos son formados en el complejo de Golgi, migran al polo proximal del ameloblasto, donde se liberan contra la dentina formada. Es así que se forma la primera capa de esmalte aprismático. Los ameloblastos se alejan de la superficie de la dentina y se forman los procesos de Tomes, estructura que se encarga de la formación de los cristales. Mientras esto ocurre, el ameloblasto produce cuatro proteínas diferentes y las secreta en la matriz de esmalte. Tres de estas son proteínas estructurales y una es una proteinasa. La amelogenina (AMELX) representa un 80-90% de la matriz orgánica, y la ameloblastina (AMBN) y enamelina (ENAM) representan el 5% y 3-5% respectivamente, y la proteinasa, que es una metaloproteinasa de matriz-20 (MMP-20, enamelisina), que está en cantidades variables. Finalizando la etapa secretoria, se alcanza el espesor total de la capa de esmalte.
El inicio del estado de transición del diente en desarrollo varía según el diente, al igual que entre las especies. En esta etapa el ameloblasto reduce su tamaño, aumentan su diámetro transversal y complejo de Golgi y su RER disminuye de volumen. El proceso de Tomes desaparece y en el polo proximal aparecen microvellosidades e invaginaciones tubulares. Estas estructuras les permiten tener capacidad absortiva y eliminar agua y matriz orgánica del esmalte. Esto facilita el posterior aumento de componente orgánico y la transformación a un esmalte maduro. En este estadio los ameloblastos sintetizan ATPasa dependiente del calcio y enzimas lisosómicas y fosfatasa alcalina. En la fase de transición muere el 25% de los ameloblastos y en la etapa de maduración muere el otro 25%.
Cuando se ha conformado el esmalte maduro, el ameloblasto entra en estado de regresión,

Once these ameloblastic bodies are formed in the Golgi complex, they migrate to the proximal pole of ameloblast, where they are released towards the formed dentin. This is how the first layer of aprismatic enamel forms. Ameloblasts move away from the surface of the dentin forming the Tomes processes—a structure responsible for the formation of crystals—. While this happens, the ameloblast produces four different proteins and secretes them into the enamel matrix. Three of these are structural proteins and one is a proteinase. Amelogenin (AMELX) represents 80-90% of the organic matrix, while ameloblastin (AMBN) and enamelin (ENAM) represent 5 and 3-5% respectively. Proteinase, which is a matrix metalloproteinase 20 (MMP-20, enamelysin) is in variable amounts. The total thickness of the enamel layer is achieved at the end of the secretory stage.
The beginning of the transitional stage varies among species and according to the developing tooth. At this stage, the ameloblast reduces in size, while it’s transverse diameter and Golgi complex increase and its RER decreases in volume. The Tomes’ process disappears and in the proximal pole there are microvilli and tubular invaginations. These structures allow them to have absorptive capacities and to eliminate water and organic matrix from the enamel. This facilitates the subsequent increase of organic component and the transformation to mature enamel. At this stage, ameloblasts synthesize calcium-dependent ATPase as well as lysosomic enzymes and alkaline phosphatase. 25% of ameloblasts die in the transition phase, and another 25% dies in the maturation stage.
When the mature enamel has formed, the ameloblast enters in a state of regression,

las células se fusionan con el resto de las capas del órgano del esmalte. Estos estratos conformarán una capa estratificada llamada epitelio reducido del esmalte, cuya función será la protección del esmalte maduro. Si este epitelio se degenera prematuramente, no puede haber erupción dentaria.3

Enamelisina
Las metaloproteinasas de matriz (MMPs) son una familia de proteinasas secretadas como proenzimas o zimógeno inactivo responsables de la degradación de componen-tes de la matriz extracelular (MEC), y su actividad está regulada por sus inhibidores endógenos o TIMPs. Las MMPs participan en procesos normales del desarrollo embrionario, en la reproducción (ciclo endometrial) y el mantenimiento (remodelado del hueso), y en procesos patológicos como la destrucción del tejido (enfermedad periodontal, caries, inflamación pulpar) y enfermedades fibróticas (esclerosis múltiple). Los miembros de esta familia se han clasificado en seis grupos diferentes, de acuerdo a su organización estructural y funcional.4
Al grupo de las colagenasas pertenecen la MMP-1,
MMP-8, MMP-13 y MMP-18. Estas enzimas se encargan
de degradar colágeno intersticial I, II, y III y otros
componentes de la matriz extracelular. El siguiente grupo
son las gelatinasas, la gelatinasa A (MMP-2) y gelatinasa B
(MMP-9). Estas se encargan de la degradación de
colágenos desnaturalizados y laminina. El tercer grupo
son las estromelisinas; estromelisina 1 (MMP-3)
y estromelisina 2 (MMP-10) y degradan diferentes
componentes de la MEC. Entre estos encontramos
fibronectina, laminina, proteoglicanos y colágenos no
fibrilares IV, V, IX, y X. Las matrilisinas se caracterizan por
la ausencia de un dominio de hemopexina, y pertenecen
la matrilisina 1 (MMP-7) y matrilisina 2 (MMP-26). Las
metaloproteinasas tipo membrana (MT-MMPs) son: MMP-
14, MMP-15, MMP-16, MMP-17, MMP-24 y MMP-25.
Por último, encontramos siete MMPs que no han sido

Enamelysin
Matrix metalloproteinases (MMPs) are a family of proteinases secreted as proenzymes or inactive zymogen responsible for the degradation of extracellular matrix (ECM) components, and their activity is regulated by its endogenous inhibitors or TIMPs. MMPs are involved in normal processes of embryonic development, in reproduction (endometrial cycle) and maintenance (bone remodeling), as well as in pathological processes such as the destruction of tissue (periodontal disease, tooth decay, pulp inflammation) and fibrotic diseases (multiple sclerosis). Members of this family have been classified into six groups, according to their structural and functional organization.4
The group of collagenases belong to the MMP-1, MMP-8, MMP-13, and MMP-18. These enzymes are responsible for degrading interstitial collagen I, II, and III and other components of the extracellular matrix. The next group is the gelatinases: gelatinase A (MMP-2) and gelatinase B (MMP-9). These are responsible for the degradation of denatured collagen and laminin. The third group are the stromelysins: stromelysin-1 (MMP-3) and stromelysin 2 (MMP-10), which degrade various components of the MEC. These include fibronectin, laminin, and proteoglycans and non-fibrillar collagens IV, V, IX, and X. Matrilysins are characterized by the absence of hemopexin domain, and include the matrilysin 1 (MMP-7) and matrilysin 2 (MMP-26). Membrane-type Metalloproteinases (MT-MMPs) are: MMP-14, MMP-15, MMP-16, MMP-17, MMP-24, and MMP-25. Finally, there are seven MMPs that have

clasificadas en ninguno de los grupos mencionados,
entre estas encontramos la enamelisina (MMP-20) que
se encarga de la degradación de la amelogenina.5
En los últimos años se demostró que el componente proteico del esmalte maduro disminuía significativamente en comparación con el esmalte inmaduro. En el proceso de amelogénesis, encontramos los siguientes componentes de la matriz orgánica del esmalte: la tuftelina, o también llamada proteína de flecos, y la sialofosfoproteina dentinaria (DSP) en la unión amelodentinaria. Posteriormente, se producen las amelogeninas, que conforman el 90% de la materia orgánica y van disminuyendo a medida que el esmalte avanza hasta su estado de maduración. La enamelina y la ameloblastina se forman en último lugar, y en este momento participan las MMPs presentes en la fase secretora de los ameloblastos y proteasas de serina, las cuales se asocian en la parte superficial de los cristales de esmalte y en la fase de maduración.
Múltiples investigaciones sugieren que, en el esmalte en desarrollo, las MMPs intervienen en un cambio de los componentes de la matriz orgánica cuando los ameloblastos pasan de la fase secretora a la fase de maduración. Se ha demostrado que la MMP-20 tiene habilidad para escindir la amelogenina y enamelina y es la única MMP durante la fase secretora. También es capaz de escindir la pro-KLK4 (peptidasa relacionada con la calicreína 4) para producir la serinproteasa KLK4 activa. También se demostró que la MMP-20 degrada E-cadherina, caseína y/o gelatina, agrecano, colágeno tipo IV, V, tenascina-C, laminina-1, laminina-5.6
La enamelisina o MMP-20 fue inicialmente clonada del órgano del esmalte de porcino, además por técnicas de hibridación in situ se ha demostrado que tanto los ameloblastos como odontoblastos presentan transcriptos de MMP-20.7 Asimismo, muy pocas líneas celulares expresan MMP-20, como el diente en desarrollo y el intestino grueso, pero no se halló expresión en tejidos como el intestino delgado, riñón, hígado, páncreas, cerebro, pulmón, bazo o estómago.8 Esta MMP fue localizada en algunas condiciones patológicas

In recent years, it has been shown that the protein component of mature enamel significantly decreases in comparison with immature enamel. During amelogenesis, there exist the following components of the organic matrix of enamel: tuftelin, also known as fringe protein, and dental sialophosphoprotein (DSP) in the amelodentinal junction. Amelogenins appear later, making up 90% of the organic matter and decreasing as the enamel progresses to its maturation stage. Enamelin and ameloblastin form in the last place, with participation of the MMPs that are present in the secretory phase of ameloblasts and serine proteases, which are associated in the superficial part of enamel crystals and the maturation stage.
Several studies suggest that, in developing enamel, MMPs participate in a change of the components of the organic matrix when ameloblasts move from the secretory phase to the maturation phase. It has been shown that MMP-20 has the ability to split amelogenin and enamelin and is the only MMP during the secretory phase. It is also capable of splitting the pro-KLK4 (4 kallikrein-related peptidase) to produce active KLK4 serinprotease. It has also been shown that the MMP-20 degrades E-cadherin, casein and/or gelatin, agrecane, type IV and type V collagen, tenascin-C, laminin-1, and laminin-5.6
Enamelysin or MMP-20 was originally cloned from porcine enamel organ, and by in situ hybridization techniques it has been shown that both ameloblasts and odontoblasts present transcripts of MMP-20.7 Also, very few cell lines express MMP-20, such as tooth development and the large intestine, but its expression has not been found in tissues such as small intestine, kidney, liver, pancreas, brain, lung, spleen, or stomach.8 This MMP was found in some pathological

como en quistes odontogénicos calcificantes, tumores odontogénicos, células del carcinoma de lengua humana.9-11 Por estas razones, es considerada una MMP específica del diente.
En las últimas décadas se ha destacado la importancia de la enamelisina en el desarrollo dentario, específica-mente en la formación del esmalte, y cómo alteraciones en su expresión pueden llevar a la formación de patolo-gías como la amelogénesis imperfecta.12-13 Por tal moti-vo, el objetivo de esta revisión es clasificar los diferentes estudios y técnicas de laboratorio empleadas, que de-muestren la participación de enamelisina en el desarrollo dentario y su relación con patologías en la formación del esmalte.
mÉtodos
Se realizó una revisión sistemática de la literatura con las siguientes bases bibliográficas: PubMed, Science-Direct, Hinari y SciELO, con las palabras clave enamel develop-ment, tooth development, enamelysin, MMP-20, amelo-genesis imperfecta, con el fin de clasificar los diferentes estudios relacionados con la participación de MMP-20 en el desarrollo dental y los métodos utilizados para de-tectar su expresión, entre los años de 2009 a 2014.
rEsUltAdos
En esta revisión sistemática se presentan los dife-rentes estudios, tanto en modelos in vitro, cultivos celulares, estudios en animales como en humanos, relacionados con enamelisina y desarrollo dentario. Se tuvieron en cuenta 19 referencias. Los resultados se presentan en la tabla 1.

rEsUlts
This systematic review presents the different studies in in vitro models, cell cultures, and animal studies as well as in humans, related to enamelysin and tooth development. 19 references were taken into account. The results are listed in table 1.

In vitro models
In 2009, Nagano et al.,14 in an in vitro model showed, through the digestion of fluorescent peptides, that MMP-20 is able to digest amelogenin sequences in specific sites. On the other hand, Klk4 (kallikrein-related peptidase 4) has affinity for different excision sites. Therefore, in the development of pig tooth, MMP-20 is the only that presents proteolytic activity in the extracellular space during the secretory phase. This is confirmed by Chun et al.,15 who concluded that MMP-20 —and not Klk4— catalyzes enamel proteins that are processed during the amelogenesis secretory phase. Other authors, such as Sun et al.,16 studied MMP-20 and Klk4. They designed a series of in vitro experiments systematically assessing the effect of HAP (hydroxyapatite) in rP172 (recombinant porcine amelogenin), rPMMP-20 (recombinant pork metalloproteinase 20), and rP148 (recombinant porcine amelogenin lacking 24 hydrophilic C-terminus amino acids) by rhKLK4 (recombinant human kallikrein-related peptidase 4). They found out that the activity of rPMMP-20 against the recombinant porcine amelogenin was more sensitive to the presence of hydroxyapatite crystals compared with the rhKLK4 against rP148, or rhKLK4 against rP172.
This suggests that there is an intimate relationship between the step-by-step degradation of amelogenin in the presence of HAPs in the early stage of enamel mineralization. Similarly, in 2013, Yamakoshi et al17 concluded that MMP-20 activates proKLK4, and KLK4 inactivates MMP-20 in vitro, and these actions can happen in vivo during amelogenesis.
These findings disagree with those by Takahashi et al18 in 2012. These authors designed an in vitro model with cell cultures of tissue explants from periodontal ligament, containing Malassez

y fibroblastos del ligamento periodontal de terceros molares de 28 individuos entre 21-28 años de edad, con el fin de determinar la expresión de amelogenina, MMP-20, (KLK4) y sus efectos en la interacción entre las células epiteliales de Malassez y fibroblastos del ligamento periodontal. Esto puede deberse a que las funciones de estas proteínas de esmalte y sus proteasas, en la formación de las raíces dentales, se desconoce.
Por otro lado, Bromley y colaboradores19 estudiaron el crecimiento de los cristales de calicita en presen-cia de amelogenina de longitud completa y su proteó-lisis por una metaloproteinasa de matriz recombinante (rhMMP-20). La amelogenina recombinante porcina (rP172) altera la forma de los cristales de calcita por in-hibición del crecimiento de los pasos en las caras que se ocluyen dentro de los cristales. Estos autores encontra-ron que, en muestras con rP172-rhMMP-20, la oclusión de la amelogenina en los cristales de calcita disminu-yó drásticamente. La reducción del extremo C-terminal disminuyó la afinidad de la amelogenina a los cristales y, por lo tanto, previó la oclusión, tal como fue hallado en los estudios de Uskokovic y colaboradores,20 donde, en un modelo in vitro, determinaron la capacidad de la amelogenina de promover la nucleación y el crecimiento de los cristales.
Para el proceso de mineralización y formación del esmalte maduro, Khan y colaboradores21 estudiaron la proteólisis de la cinética de enzimas de MMP-20 de longitud com-pleta amelogenina recombinante humana bajo diferentes composiciones minerales. Hallaron que la MMP-20 fue más eficiente en altas concentraciones de calcio, mien-tras que fue más bajo en altas concentraciones de fosfato y altas concentraciones de calcio y fosfato. Estos estudios in vitro demuestran que la química de las soluciones pro-teínicas pueden alterar significativamente el tratamiento de amelogenina por MMP-20, que puede tener efectos signi-ficativos en el ensamblaje de la matriz in vivo y, posterior-mente, en la mineralización de fosfato de calcio.
epithelial cells and fibroblasts of the periodontal ligament of lower third molars of 28 individuals aged 21 to 28 years, in order to determine the expression of amelogenin, MMP-20, (KLK4) and their effects on the interaction between Malassez epithelial cells and fibroblasts of the periodontal ligament. The immunohistochemistry results showed a weak expression of amelogenin, ameloblastin, MMP-20, and KLK4 in Malassez epithelial cells. This may happen because the functions of these proteins in enamel and their proteases in the formation of dental roots are still unknown.
On the other hand, Bromley et al19 studied the growth of calcite crystals in the presence of full-length amelogenin and its proteolysis by a recombinant matrix metalloproteinase (rhMMP-20). Recombinant porcine amelogenin (rP172) alters the shape of calcite crystals by inhibiting the growth of the steps in the sides that are occluded in the crystals. These authors found out that, in samples with rP172-rhMMP-20, occlusion of the amelogenin in calcite crystals decreased drastically. The c-terminal end reduction decreased the affinity of the amelogenin crystals and therefore foresaw the occlusion, as found in studies by Uskokovic et al.,20 who by means of an in vitro model determined the ability of amelogenin to promote the nucleation and growth of crystals.
Concerning the process of mineralization and formation of mature enamel, Khan et al21 studied proteolysis of the kinetics of MMP-20 enzymes of full-length human recombinant amelogenin under different mineral compositions. They found out that MMP-20 was more efficient in high concentrations of calcium, but lower in high concentrations of phosphate and high concentrations of calcium and phosphate. These in vitro studies show that the chemistry of protein solutions can significantly alter the treatment of amelogenin by MMP-20, which can have significant effects in the matrix assembly in vivo and later in the mineralization of calcium phosphate.

En el 2010, Lee y colaboradores22 estudiaron los me-canismos moleculares responsables de la regulación de la MMP-20, encontrando que la fase secretora de la amelogénesis ODAM (proteína asociada a ameloblastos odontogénicos) se localizó en el núcleo y citoplasma de los ameloblastos, y la etapa de maduración se observó en el citoplasma y en la interfaz entre los ameloblastos y la capa de esmalte, pero no en el núcleo. Se sugiere que Runx2 regula la expresión de ODAM nuclear que tiene una función reguladora importante en la mineralización del esmalte a través de la regulación de la MMP-20. Es-tos resultados son confirmados nuevamente en el 2012 por Lee y colaboradores.23 Estos autores investigaron la expresión de ODAM en ameloblastos, odontoblastos y varios tipos de células cancerígenas, y encontraron que ODAM se localizó en estas células tanto in vivo como in vitro. Estos resultados sugieren que el patrón de expre-sión y la localización subcelular de ODAM es altamente variable y dependiente de los tipos celulares, sus esta-dos de diferenciación y las correlaciones funcionales existentes entre ODAM y MMP-20.
Por otro lado, Gao y colaboradores24 exploraron el rol del factor-2C potenciador de miocito (MEF2C) y el factor de crecimiento transformante beta (TGFβ1) en la expresión genética de MMP-20. Estos autores concluyeron que el TGFβ1 induce la expresión de MMP-20 en ALCs (Línea celular de ameloblastos).
Estudios en animales
Con relación a los estudios realizados en modelos muri-nos, Yamakoshi y colaboradores25 caracterizaron ratones knockout para MMP-20 y KLK4, y doble nulo de MMP-20/KLKL-4. Estudiaron las fases secretoras y de madu-ración por medio de histología, zimografía y Western blot. Solo se encontraron amelogeninas y ameloblasti-nas intactas en la fase secretoria en el ratón knockout para MMP-20, mientras que en la matriz perteneciente a

In 2010, Lee et al,22 studied the molecular mechanisms responsible for the regulation of MMP-20, finding out that the secretory phase of amelogenesis ODAM (odontogenic- ameloblast-associated protein) was located in the nucleus and cytoplasm of ameloblasts, and maturation stage was observed in the cytoplasm and at the interface between the ameloblasts and enamel coating, but not in the nucleus. Concerning studies in mouse models, Yamakoshi et al25 characterized knockout mice for MMP-20, KLK4, and double null MMP-20/KLKL-4. They studied the secretory and maturation phases by histology, zymography, and Western blot. They only found intact amelogenins and ameloblastins in the secretory phase in knockout mouse for MMP-20, while the matrix belonging to the secretory

la fase secretoria para el ratón nulo de KLK-4 fue idéntico a lo encontrado en el ratón silvestre.
Más residuos en la matriz se observaron en el ratón doble nulo comparado con los ratones knockout de MMP-20 o KLK-4. Esto lo corroboraron Bartlett y co-laboradores26 y Shin y colaboradores27 donde, con un modelo de ratón knockout para MMP-20, demostraron la importancia de MMP-20 como mediador necesario para el mantenimiento de la superficie de esmalte, una unión amelo-dentinal fuerte y para el establecimiento del patrón de varillas de esmalte. Estos resultados validan lo encontrado en modelos in vitro por Nagano y colabora-dores14 y Chun y colaboradores15.
Estudios en humanos
Con relación a los estudios en humanos, estos men-cionan la importancia que tiene la MMP-20 en la caries dental, el grosor del esmalte y la susceptibilidad a caries por radiación y agenesias dentales.
En primer lugar, Shimada y colaboradores28 estudiaron, por inmunomarcación enzimática, la participación de MMP-2,-8,-9 y 20 en lesiones de caries dentinal en ter-ceros molares humanos de pacientes entre 32-59 años, y hallaron que la MMP-2 estaba distribuida en la denti-na normal y la dentina cariada, los niveles de MMP-8 y MMP-9 estaban significativamente disminuidos en la ca-ries dentinal interna comparada con la dentina normal y la MMP-20 era la más alta en la dentina normal, y decre-ció significativamente hacia la región externa de caries.
Por otro lado, Tannure y colaboradores29 evaluaron la asociación entre MMP-20 y la experiencia de caries en los niños brasileros. De 388 sujetos, 161 fueron niños libres de caries. Estos autores no encontraron diferencias entre los niveles de caries y la distribución genotípica en la cohorte total. Sin embargo, hubo diferencias en la distribución genotípica, observadas en los niños libres de caries vs. niños con caries en caucásicos (p=0,03). Estos resultados sugieren que el desarrollo de la caries es multifactorial y puede estar asociado a genotipos de MMP-20 en mayor proporción

More matrix remnants were observed in double null mice compared with knockout mice of MMP-20 or KLK-4. This was supported by Bartlett et al26 and Shin et al,27 who by means of a knockout mouse model for MMP-20 showed the importance of MMP-20 as a necessary mediator for the maintenance of enamel surface, a strong dentin-enamel junction, and the establishment of enamel rods pattern. These results validate the findings in in vitro models by Nagano et al14 and Chun et al.15
studies in humans
With regard to studies in humans, they mention the importance of MMP-20 in dental caries, enamel thickness, and susceptibility to caries by radiation and dental agenesis.
By means of immunolabeling enzyme, Shimada et al28 studied the participation of MMP-2, -8, -9, and -20 in dentinal caries lesions in third molars of human patients aged 32 to 59 years, finding out that the MMP-2 was distributed in normal dentin and carious dentin, MMP-8 and MMP-9 levels were significantly decreased in inner dentinal caries compared to normal dentin, and MMP-20 was the highest in normal dentin, significantly decreasing towards the outer region of caries.
On the other hand, Tannure et al29 evaluated the association of MMP-20 and caries history in Brazilian children. 161 out of 388 subjects were caries-free kids. The authors found no differences between the levels of tooth decay and genotypic distribution in the total sample. However, there were differences in genotypic distribution. Differences in genotypic distribution were observed in caries-free children vs. children with caries, in Caucasians (p = 0.03). These results suggest that caries development is multifactorial and may be associated to MMP-20 genotypes, in higher

proportions among Caucasian individuals with poor oral hygiene habits.
In 2014, McGuire et al,30 by means of mass spectrometry techniques, immunolabeling, and gel zymography, studied the teeth of patients with oral cancer treated with radiotherapy, comparing them against the teeth of healthy individuals. They concluded that MMP-20 is the most abundant matrix metalloproteinase in irradiated and incubated in vitro extracts of mature dental crowns, as in control crowns. In their in vitro studies, they found out that the 23 kDa fragment of MMP-20 is resistant to radiation of mature dental crowns, which indicates that this component can degrade the protein components of the enamel-dentin interface and thus contribute to the pathological delamination observed in oral radiotherapy, which is also associated with high doses of radiotherapy.
In the same year, Horvath et al31 found strong evidence for a positive selection in the 5’ and 3’ regions of MMP-20 and ENAM regions along the lineage of humans, and both 5’ and 3’ regions of MMP-20 along the lineage leading chimpanzees. Non-coding changes and their potential differential regulation by known transcription factors regulate tooth development and can provide insight into the mechanisms that allow quick evolutionary changes in enamel thickness among closely related species.
On the other hand, Kuchler et al32 studied the associations of MMP-1, MMP-3 and MMP-20, and their relations to dental agenesis in 167 nuclear families of two different populations, 116 from Brazil and 51 from Turkey. The participants had at least one missing tooth. These authors found associations among dental agenesis, MMP-1 (p = 0.007) and MMP-20 (p = 0.03) in Brazilian families. These results disagree with those reported by Antunes et al,33 who studied the association of polymorphisms in human genes for MMP-2, MMP-9 and MMP-13 with dental agenesis, in 285 non-related individuals

En el 2014, McGuire y colaboradores,30 por técnicas de espectrometría de masa, inmunomarcaje y zimografía en gel, estudiaron dientes de pacientes con cáncer oral tratados con radioterapia, y se compararon con dien-tes de individuos sanos. Concluyeron que la MMP-20 es la metaloproteinasa de matriz más abundante en extractos in vitro irradiados e incubados de las coro-nas dentales maduras, como con las coronas control. En los estudios in vitro encontraron que el fragmento de 23 kDa de MMP-20 es resistente a la radiación de las coronas dentales maduras, lo que indica que este componente puede degradar los componentes protei-cos de la interfase esmalte-dentina y contribuir así a la delaminación patológica del esmalte observado en la radioterapia oral, lo cual también se asocia a las do-sis altas de radioterapia.
En el mismo año, Horvath y colaboradores31 encontraron una fuerte evidencia para selección positiva en las re-giones 5’ y 3’ de las regiones de MMP-20 y ENAM, a lo largo del linaje de los humanos, y en ambos el extremo 5’ y 3’ de las regiones de MMP-20 a lo largo del linaje que lidera a los chimpancés. Cambios no codificantes y su potencial para la regulación diferencial por factores de transcripción conocidos regulan el desarrollo del diente y pueden ofrecer una visión a los mecanismos que permi-ten cambios evolutivos rápidos en el grosor del esmalte entre especies relacionadas cercanamente.

(202 controles sin agenesia dental y 83 casos con agenesia dental), donde no encontraron asociación significativa para el genotipo MMP-2, y encontraron una relación significativa de agenesias dentales asociadas a los genotipos de MMP-9 y MMP-13.
conclUsión
Los resultados de esta revisión proveen un soporte sobre la importancia que tiene la MMP-20 para la degradación de diversas proteínas de la matriz orgánica, como ame-logeninas y ameloblastinas en la fase secretora de la for-mación de esmalte. Además de la participación de otras proteasas como la KLK4 que, a diferencia de la MMP-20, participa en la fase de maduración de la amelogénesis. Los modelos in vitro permiten concluir que la MMP-20 tiene sitios específicos de escisión para las proteínas de matriz y que estos difieren para la KLK4. Así mismo, este proceso puede ser alterado por la composición química, iones y la presencia de hidroxiapatita.
Con los modelos knockout se concluye que alteraciones en MMP-20 modifican completamente la morfología del esmalte, a diferencia de lo hallado en el ratón nulo para KLK4. Estos modelos permiten entender las diversas va-riaciones que se presentan en el desarrollo del esmalte, como por ejemplo la amelogénesis imperfecta.
En los estudios realizados en humanos, se ha relaciona-do la MMP-20 en primer lugar; con una mayor suscep-tibilidad de caries dental, en segundo lugar; una relación evolutiva entre humanos y chimpancés y su asociación con el grosor del esmalte, en tercer lugar; una mayor presencia de MMP-20 en la unión amelo-dentinal en dientes sometidos a radioterapia, lo cual se vincula con la delaminación del esmalte. Y, por último, una asocia-ción entre MMP-20 y agenesias dentales.

conclUsion
The results of this review highlight the importance of MMP-20 in the degradation of various proteins of organic matrix, such as amelogenins and ameloblastins in the secretory phase of enamel formation, in addition to the involvement of other proteases such as KLK4 which, unlike MMP-20, takes part in the maturation stage of amelogenesis. The in vitro models allow concluding that MMP-20 has specific sites for excision of matrix proteins and that these differ to KLK4. Similarly, this process may be altered by chemical composition, ions, and the presence of hydroxyapatite.
The knockout models allow concluding that alterations in MMP-20 completely alter enamel morphology, unlike the findings in null mice for KLK4. These models allow us to understand the different variations occurring during enamel development, such as amelogenesis imperfecta.
In human studies, MMP-20 has been related to a) increased susceptibility to tooth decay, b) an evolutionary relationship between humans and chimpanzees, as well as an association with enamel thickness, and c) a greater presence of MMP-20 in the amelodentinal junction in teeth subjected to radiation therapy, which is linked to enamel delamination. Finally, there is an association between MMP-20 and dental agenesis.
