可攜捐款表格上的條碼，到任何一間 7-eleven 或 OK 便利店捐款至本會戶口。捐款金額需為港幣一百元至五千元。請保留銀

行收據及此表格一併以傳真或電郵交回本會辦事處。Please bring along the donation barcode in this form when making

donation at 7-eleven or Circle K. You can make donations between HK$100 and HK$5000. Please keep a copy of the deposit slip

and send it together with this sponsorship form to our office by e-mail or fax.

助養指定動物 Nominate sponsored animal：

本會的助養計劃可以指定會內任何一隻動物，並會定期按你所指定助養的動物以 WhatsApp 或電郵發出牠們的近照和近況。

(如沒有指定動物則由本會職員代為選擇)You can nominate any animal in our shelter for sponsorship. Update picture of your

sponsored animal will be sent by WhatsApp regularly. (We will choose the animal for you if you do not state your preference)

You will be notified once your chosen sponsored animal is adopted or has passed away.

1. 助養滿一年或以上，本會均會頒發感謝卡以表揚閣下對本會的慷慨捐助。A certificate of appreciation will be issued upon

completion of one year’s sponsorship as acknowledgement of your generous support to SAA.

2. 所有超逾港幣$100 的捐款可作申請扣稅之用。Donations over HK$100 are tax deductible.

3. 如欲中途終止捐款，請以郵寄或電郵書面通知。To termination sponsorship please inform us by post or e-mail.
