Reactions 1480, p16 - 30 Nov 2013 S Deferoxamine Acute interstitial pneumonia: case report A 43-year-old man developed acute interstitial pneumonia while receiving deferoxamine. The man, who had myelodysplastic syndrome, started thalidomide and stanozolol. He then started an IV infusion of deferoxamine 25 mg/kg/day for five consecutive days a week. On the 16th day of treatment, he began to develop a hacking cough and coughed up a small amount of rust-coloured sputum. He also developed shortness of breath and a progressive dyspnoea. He had cyanotic lips, coarse rales and fine crackles over his lower lung lobes. The man was considered to have pulmonary infections, and received cefodizime and supplemental oxygen. However, his dyspnoea and lung crackles worsened, and he was coughing more frequently. A chest CT showed diffuse and interstitial infiltration in both lungs with frosted-glass-like shadows and decreased transmittance. His breathing difficulty became severe and his mobility was limited. Bronchoscopy showed a large amount of tissue and a small number of neutrophils in his lung cavity. His alveolar epithelium showed signs of proliferation, fusion, degeneration and shredding. He was diagnosed with acute interstitial pneumonia. Since deferoxamine was considered to be the cause of the interstitial pneumonia, the drug was withdrawn, and he started methylprednisolone. Three days later, his coughing and dyspnoea was significantly alleviated. A follow-up CT scan showed great improvement. Subsequently, methylprednisolone was reduced, and he fully recovered from dyspnoea and coughing.
