This booklet contains information you will need to enroll your child/children in the 2015 MPSD Summer School program.

If your child’s teacher has recommended that your child take math and/or reading homeroom during summer school, we strongly recommend that you take advantage of this opportunity. With the schedule and structure of summer school, you can enroll your son/daughter in courses designed to boost academic skills and enrichment classes as well.

This catalog attempts to answer many of your questions. However, please feel free to call with any questions you still have after reading through the enclosed information.

Important

Please read through this booklet and keep it as a reference until summer school is completed. Summer School Office Location –Although the first two days of summer school

registration (April 9 & 10) will take place at the Administration Office located on 2902 Lindbergh Drive, the Summer School Office will be located at Madison Elementary School –

701 North 4th Street. Registration after April 10 will take place at Madison Elementary School.

Summer School Office Hours are 8:00 am to 3:30 pm April – June 12, and 8:00 am to 12:30 pm June 15 – July 17

Jackson Drop Off and Pick Up – Parents can drop off and pick up students on the west side of North 18 Street or east of the staff parking lot on Menasha Avenue. Buses will drop off and pick up students on the east side of North 18 Street; please do not stop or pick up your child on this side of the road since it is a no stopping/parking zone and you could be ticketed.

Medical History Information (located on the back of the Registration Form) - You MUST complete this form if your child does NOT attend the MPSD. If your child is a student of the MPSD we have the health information on file; please only complete this portion if there are changes to your child’s health record.

Consent to Administer Medication - If your child requires medication to be administered to them while at summer school, please call to have a consent form mailed to your home. All medication must be in the proper containers and labeled.

Participants – Summer School is open to all students who live within the boundaries of the Manitowoc Public School District (public or private school students, including home schooled students) and students open-enrolled into the district.

Class Locations – Class locations are listed by building in the Session 1, 2 & Special Class tables. Classes offered at the various buildings are located directly below the heading. Room numbers and directions to the classrooms will be posted at the main entrance of each building on the first day of each session.

Registration Fee / Cancellation – Fees to help defray the cost of supplies are included in the session tables. All fees must accompany the registration and are NON-REFUNDABLE unless a course is cancelled due to insufficient enrollment or the class is full. Checks should be made payable to the MPSD.

o Fees will be held until registrations or cancellations are confirmed. Refunds will be mailed once the registration process is complete.

o Additional fees will apply for non-residential students. The MPSD will charge an additional $2.00, per hour, per day – not to exceed $25.00.

During the first two days of registration, students are enrolled in summer school classes based on the DATE registration forms are received – not time of day registration forms are turned in. All registrations received on a date, regardless of time of day received, will be given equal consideration for enrollment. A lottery system is used if there are more registrants than space available. All checks should be made payable to the MPSD. After the initial registration perio0d, students are enrolled on first come, first served basis.

Please be sure to enter a second class choice (if applicable) on the registration forms, as many classes will be filled on the first day. In the event your child’s 1st course choices are full, we will make every attempt to place your child in their 2nd course choice options, if they are still available.

When paying registration fees, please pay for your 1 st course choices only. Any difference in registration fees will be

refunded or billed at a later date.

If you have siblings that need to be registered together due to transportation/child care issues, please note this on the registration forms for both students. You can indicate that they must be registered together. Although we will do our best to make accommodations for this, we cannot make any guarantees.

SchedulesWe understand that many people need to know their children’s schedule as soon as possible to make plans for the summer. We will send schedules out at the earliest possible date . Please do not call to find out if your child is registered for classes; this will simply slow the registration process.

KEEP THIS BOOK AS A REFERENCE for dates, times & locations of classes. In the event a course is changed, you will be notified by the instructor or summer school secretary.

You may MAIL your registration to our office. All registrations received prior to the first day of registration will be processed as of April 9, 2015. Early ReleaseIf a child needs to be released from a class early, the parent or guardian must come to the office to sign the child out.

Progress ReportsStudents enrolled in Reading and Math Homeroom will receive a progress report on the final day of their class.

BicyclesBicycles are to be placed in the bicycle racks at school and must be locked. We discourage bringing expensive bicycles to school.

TransportationIn an effort to allow more children to attend the program, the district will provide a free shuttle bus service. Please see attached page at the end of this booklet for more details.

If you are interested in having your child ride the shuttle bus, please complete the shuttle busing form on page 35.

READING & MATH HOMEROOMS

(4 week classes)

Reading Homeroom Grades 2-7Students will have the opportunity to improve their reading and writing skills. This course is also for the student who wants to continue his/her studies throughout the summer months. Students may sign up for Period 1 (8:00am – 10:00am) or Period 2 (10:05am – 12:05pm). Reading Homeroom will be held for 4 weeks (June 15 – 26 AND July 6 – 17). Your child’s teacher may recommend that your child take this class.

Math Homeroom Grades 2-7Students will have the opportunity to practice and improve math skills. This course is also for the student who wants to continue his/her studies throughout the summer months. Students may sign up for Period 1 (8:00am – 10:00am) or Period 2 (10:05am – 12:05pm). Math Homeroom will be held for 4 weeks (June 15 – 26 AND July 6 – 17). Your child’s teacher may recommend that your child take this class.

“Tri” GolfThis class is designed for 3 days to introduce students to 3 types of golf: Traditional Golf, Disc Golf, and an up and coming sport, Foot Golf. Part of the class will be held at Fairview Golf Course in Two Rivers along with attending Silver Creek and Indian Creek Park Disc Golf Course.
