บทท 1 บทน ำ
ความชกของโรคภมแพตางๆทวโลกเพมสงขน โดยขอมลจากการส ารวจใน 30 ประเทศทวโลก1 ดวยประชากรกวา 1,200 ลานคน พบมผปวยถงรอยละ 22 หรอกวา 250 ลานคน ซงก าลงประสบปญหาโรคภมแพ สวนของประเทศไทยความชกของโรคหดในเดกในเขตกรงเทพมห านครมการเพมขนจากรอยละ 4 ในปพทธศกราช 2528 เปนรอยละ 11 ในปพทธศกราช 2538 และเปนรอยละ 15 ในปพทธศกราช 25442-5 ในเขตจงหวดเชยงใหม มการเพมขนจากรอยละ 5.5 ในปพทธศกราช 2538 เปนรอยละ 7.8 ในปพทธศกราช 25442,3,6
การดแลผปวยเดกโรคหดประกอบดวยการรกษาดวยยาทเหมาะสม ซงปจจบนยงเ ปนยาทมราคาแพง และยาบางชนดเปนยาแบบสดพนซงตองบรหารยาโดยอาศยการดแลจากคนในครอบครว การดแลสภาพแวดลอมและการหลกเลยงสารกอภมแพ ตลอดจนการดแลสขภาพของผปวยใหแขงแรง แตในสภาพเศรษฐกจและสงคมไทยปจจบน การมเดกทเจบปวยดวยโรคหดเปนภาระทท าใหเ กดความยากล าบากกบครอบครว รวมกบสภาพมลภาวะเปนพษในอากาศตามเมองใหญ ทอาจมผลกระตนอาการของผปวยเดกโรคหดใหแยลง สงผลกระทบตอสขภาพของผปวยในระยะยาว และคาใชจายในการรกษาพยาบาลทเพมขน รวมถงผลกระทบในดานตางๆตอครอบครวของผปวย คณภาพชวตของผปวยและครอบครวทแยลง รวมถงการสญเสยทางเศรษฐกจของครอบครวผปวยและประเทศชาต
ปจจบนยงไมมการศกษาเกยวกบการดแลผปวยเ ดกโรคหดในประเทศไทยอยางเ ปนระบบ ทงในดานการดแลรกษาดวยยาทเหมาะสม และการประเมนถงคาใชจายในการรกษา ตามสภาพของโรคในผปวยเดกแตละราย การประเมนถงการสญเสยทางเศรษฐกจของครอบครวกบการเ จบปวยจากโรคหด ผลกระทบตอคณภาพชวตของผปวยเดกโรคหด และ ผลการด าเนนโรคในระยะยาวของผปวยเดกโรคหด ปจจยจากสภาพสงแวดลอมและมลพษในอากาศทมผลตอการเ จบปวยจากโรคหดในเดก ท าใหไมมขอมลเกยวกบโรคนในประเทศไทย พอทจะแสดงใหเหนถงความรนแรงของปญหาโรคหดในประเทศไทย ผลของการรกษาโรค ผลการด าเนนโรคในระยะยาว การพยากรณโรค ตลอดจนผลกระทบของโรคหดตอคณภาพชวตของผปวยเดกโรคหด เพอใหความรและค าแนะน าแกผปวยและหนวยงานทเกยวของ ขอมลความรจากตางประเทศ อาจมความแตกตางจากสภาพความเปนจรงในประเทศไทย และตองมการปรบใชตามความเหมาะสม โดยเฉพาะกบระบบเศรษฐกจแบบพอเพยงของประเทศไทย

บทท 2 กำรทบทวนวรรณกรรมทเกยวของ
ความชกของโรคภมแพตางๆทวโลกเพมสงขน โดยขอมลจากการส ารวจใน 30 ประเทศทว
โลก1 ดวยประชากรกวา 1,200 ลานคน พบมผปวยถงรอยละ 22 หรอกวา 250 ลานคน ซงก าลงประสบปญหาโรคภมแพ และจากการศกษาใน ISAAC (International Study of Asthma and Allergies in Childhood) ความชกของโรคหดในหลายประเทศอยระหวางรอยละ 3 – 30 2 ,3 สวนของประเทศไทยความชกของโรคหดในเดกในเขตกรงเทพมหานครมการเพมขนจากรอยละ 4 ในปพทธศกราช 2528 เปนรอยละ 11 ในปพทธศกราช 2538 และเปนรอยละ 15 ในปพทธศกราช 2544 2-5 ในเขตจงหวดเชยงใหม มการเพมขนจากรอยละ 5.5 ในปพทธศกราช 2538 เปนรอยละ7.8 ในปพทธศกราช 2544 2,3,6
ผลการด าเนนโรคในระยะยาวของผปวยเ ดกโรคหดจากการศกษาในตางประเทศ อาท การศกษาในประเทศออสเตรเ ลย Melbourne Epidemiological Study of Childhood Asthma 7,8 ซ งตดตามผปวยเดกโรคหดตงแตอาย 7 ป จ านวน 291 คน และผปวยเ ดกโรคหดทมความรนแรงมากอาย 10 ป จ านวน 83 คน ตดตามและประเมนผลทก 7 ปไปนานจนถงอาย 42 ป พบวาในกลมผปวยทมอาการโรคหดเปนครงคราวในตอนแรก รอยละ 60 จะหายจากโรคและไมมอาการของโรคเลยในชวง 3 ปหลง รอยละ 33 จะยงมอาการของโรค และในกลมผปวยทมอาการของโรคหดรนแรงและตลอดเวลาในตอนแรก รอยละ 21 จะหายจากโรคและรอยละ 6 ยงมอาการของโรคอย การศกษาในประเทศนวซแลนด 9 ในผปวยเดกโรคหด 613 คน พบวาผปวยเดกรอยละ 14.5 ทมอาการของโรคหดตงแตเ ดก จะยงมอาการของโรคเ มอตดตามจนถงอาย 26 ป ผปวยรอยละ 12.4 มอาการของโรคทหายไปแลวกลบมามอาการของโรคอกทตอนอาย 26 ป ผปวยรอยละ 15 ไมมอาการของโรคหดอกตอนอาย 26 ปเลย โดยมปจจยทเกยวของกบการยงมอาการโรคหดทอาย 26 ป คอ การแพไรฝน การมภาวะหลอดลมไวตอสงกระตน การศกษาในประเทศเนเธอรแลนด 10 ตดตามผปวยจ านวน 119 คนเปนระยะเวลานานเฉลย 30 ป พบวา ผปวยรอยละ 22 หายจากโรคหด (Complete remission) รอยละ 30 ไมมอาการของโรค (Clinical remission)โดยท รอยละ 57 ในกลม Clinical remission จะ มอาการของ bronchial hyperresponsiveness และ/หรอ low lung function โดยทมปจจยคอ การม FEV1 ทสงกวาในตอนเ รมมอาการในวยเดก และการเพมขนของ FEV1 จากชวงอาย 5-14 ป ถง ชวงอาย 21-33ป เ ปนตวก าหนด Complete remission และ Clinical remission การ ศกษาจาก Tuscon Children’s Respiratory Study ประเทศสหรฐอเมรกา 11,12 ตดตามผปวยเดกโรคหดตงแตอาย 3 ป จ านวน 277 คน พบวา มผปวยเ ดก 113 คนทยงมอาการของโรคหดทอาย 6 ป และตดตามผปวยกลมน ตอไปทอาย 8 , 11 , 13 และ 16 ปพบวา จะมผปวยทยงมอาการของโรคหดอยรอยละ 67.5, 63.6, 54.4 และ 50.0 ตามล าดบ

1. ตนทนของกำรเจบปวย (Cost of illness) จากสถานการณททรพยากรในโลกนมอยอยางจ ากด สงคมตองการใชทรพยากร ใหเ กดประโยชนสงสด เมอมทางเ ลอกทใชทรพยากร หรอตนทนในการด าเนนการแตกตางกน และใหผลลพธทแตกตางกน ในสถานการณเชนนจ าเปนตองใชวธการทางเศรษฐศาสตรชวยวเคราะหขอมลเพอการตดสนใจในการวางแผนการใชทรพยากรใหเกดประโยชนสงสด จงไดเ กด วชาเศรษฐศาสตรสาธารณสข (Health economics) ซงเปนสาขาวชาทท าการวเคราะหดานเศรษฐศาสตรของสขภาพและการดแลสขภาพ โดยทฤษฎดานเศรษฐศาสตรและการแพทย โดยทวไปจะเนนททรพยากรทใชไปหรอตนทน และผลตอเน องทเ กดขนหรอผลลพธ ของมาตรการทางดานส ขภาพ 13 ชอสาขาวช าเศรษฐศาสตรสาธารณสข อาจเ รยกวา เศรษฐศาสตรเภสชกรรม (Pharmacoeconomics) หากก าหนดกรอบมาตรการทางสขภาพทศกษา เพยงยาและการบรการเภสชกรรม โดยก าหนดค าจ ากดความไววาเปนการ จ าแนก วดและเปรยบเทยบตนทนของยาและบรการทางเภสชกรรม กบผลท เ กดขน ไดแกผลลพธดานคลนก ผลลพธดานเศรษฐศาสตร และผลลพธดานมนษยนยม (Clinical, economic and humanistic outcomes) 14
การศกษาทางเศรษฐศาสตรรปแบบหนงไดแก การศกษาตนทนของการเจบปวย (Cost of illness) หมายถง ภาระทางเศรษฐกจของโรคหรอการเ จบปวยทเกดขนแกสงคม ผลการศกษานสามารถน าไปใชประโยชนการบรหารจดการไดดงน
1. กำรจดล ำดบควำมส ำคญของกำรเจบปวยประเภทตำงๆ เพอใหทราบขนาดของผลกระทบทางเศรษฐศาสตรของโรคหรอการเจบปวยชนดตางๆ แลวตดสนก าหนดความส าคญในการสนบสนนงบประมาณในการแกปญหา หรอในการวจย
2. กำรปรบปรงประสทธภำพกำรจดกำรบรกำรของหนวยงำน ไดแกการน าผลการวเคราะหไปเปนขอมลหรอเครองมอในการปรบปรงพฤตกรรมการท างานใหใชทรพยากรทค านวณออกมาเปนตนทนตอหนวยผลงานทผลตลดลง ซงอาจเกดจากการผลตในปรมาณเทาเดมแตเ กดการประหยดท าใหตนทนลดลง หรอใชทรพยากรเทาเดมแตผลตผลงานไดมากขน
3. กำรจดท ำงบประมำณหรอเจรจำตอรองอตรำกำรจำยคำบรกำร ไดแกการวเคราะหตนทนตามกรอบงบประมาณหรอกรอบการจายคาบรการ แลวน าเสนอหนวยงานทพจารณางบประมาณหรอจายคาบรการเพอปรบปรงอตราการเบกจาย
4. กำรประเมนทำงเลอกของกำรใชทรพยำกรทคมคำ เ ปนการประเมนทางเศรษฐศาสตร (Economic evaluation) ซงเปนการวเคราะหเปรยบเทยบทางเลอกของการด าเนนงานทงในดานตนทนและผลลพธทเกดขน
2. รปแบบกำรค ำนวณ การศกษาตนทนของการเ จบปวยท าได 2 รปแบบดงน
2.1 กำรค ำนวณบนพนฐำนของควำมชกของโรค (Prevalence-based approach)

เปนการคดตนทนทเกดขนกบทกคนทไดรบการวนจฉยวาเปนโรคในชวงเวลาของการศกษาทก าหนด ซงโดยทวไปก าหนดใหเปน 1 ป โดยรวมทงผทเรมปวยกอนหนาและในชวงเวลาทก าหนด รปแบบนเหมาะส าหรบโรคทเหนผลการรกษาในระยะสน ไมเรอรง ตวเลขทไดเปนตนทนเฉลยของโรค (ทมความหลากหลายของลกษณะตามระยะอาการของการเกดโรค) ทเกดขนในเวลา 1 ป
2.2 กำรค ำนวณบนพนฐำนของอบตกำรณของโรค (Incidence-based approach) เปนการคดตนทนของกลมผปวยเฉพาะรายใหมทเรมปวยในชวงเวลาทก าหนดในการศกษา ซงมกก าหนด 1 ป เชนเ ดยวกน แลวตดตามรวบรวมตนทนไปจนหายปวยหรอเสยชวต รปแบบนเหมาะส าหรบโรคทเรอรง เชน เบาหวาน ความดนโลหตสง แตยากในการรวบรวมขอมลกวารปแบบแรก ตวเลขทไดเปนตนทนของโรคตงแตเ รมจนสนสด (Lifetime disease costs)ในดานการประเมนตนทนจ าเปนตองก าหนดองคประกอบ ซงจะตองสอดคลองกบมมมองในการวเคราะห ตนทนสามารถแบงออกเปน 3 กลม 15 ดงน
2.2.1 ตนทนทำงตรงดำนกำรแพทย (Direct medical costs) ไดแก ตนทนทางการแพทย เ รมตงแตการตรวจวนจฉย การ รกษาเ บองตน การรกษาตอเนอง การฟนฟสมรรถภาพ และการรกษาระยะสดทาย (Terminal care) คอ ตนทนในการรกษาพยาบาลในโรงพยาบาล รวมถงบรการอน เชน การดแลทบาน (Home care) การดแลดานการพยาบาล (Nursing care) 2.2.2 ตนทนทำงตรงทไมเกยวกบกำรแพทย (Direct non-medical costs) คอ ตนทนทเกดขนจากสาเหตการเจบปวย การรกษาแตไมไดเปนสวนหนงของการรกษา เชน
การบรการทางสงคม เชน การชวยเหลอท างานบาน การบรการอาหารสงถงบาน คาเดนทางไปรบการรกษา อปกรณการอ านวยความสะดวก เชน การปรบปรงบานพกใหผปวยขนลงได
สะดวก อปกรณชวยการขบรถของผพการ การดแลอยางไมเปนทางการ (Informal care) เชน การดแลโดยญาต เพอนบาน
(บางทอาจจดอยในกลม ตนทนทางออม) 2.2.3 ตนทนทำงออม (Indirect costs) มความหมายเ ดยวกบตนทนดานผลตผล (Productivity cost) ไ ดแก ผลผลตทางสง คมทขาดหายไปเน องจากการลา ปวย การลดประสทธภาพการท างาน การทตองเกษยณกอนอายปกต การพการถาวร ซงท างานไดนอยลง หรอท างานไมได การเสยชวตในวยท างานในสวนของมมมอง (Perspective) ในการวเคราะหเปน การพจารณาวาภาระตนทนทเ กดพจารณาจากจดยนของใคร โดยทวไปอาจก าหนดมมมองเปน 5 แบบ 16 ไดแก

1) มมมองของสงคม (Societal perspective) เปนมมมองทกวางทสด เ ปนการรวมตนทนของทงผใหบรการ ผรบบรการ (ซงรวมของญาตดวย) ทงของภาครฐและเอกชน
2) มมมองของภำครฐ (Public sector perspective) มมมองนอาจก าหนดเฉพาะเปนมมมองระดบกระทรวง เชน กระทรวงสาธารณสข กระทรวงพฒนาสงคมและความมนคงของมนษย หรอตนทนรวมของทกหนวยงานราชการเปนตนทนของรฐบาล
3) ผจำยเงน (Payer perspective) ผจายเงนโดยสวนใหญ หมายถง หนวยงานทซอบรการจาก ผใหบรการ ห รอสถานพยาบาล เพอใหจดบรการแ กผรบบรการหรอผปวยทหนวยงานนนตองรบผดชอบ เชน บรษทประกนภย ส านกงานประกนสงคม ส านกงานหลกประกนสขภาพแหงชาต
4) ผใหบรกำร (Provider perspective) เ ปนตนทนทเ กดขนกบผใหบรการ เชน โรงพยาบาลสถานอนามย สถานสงเคราะห หรอฟนฟสมรรถภาพผปวย ในการผลตบรการ
5) ผรบบรกำร (Patient or consumer perspective) เปนการรวมตนทนทเ ปนภาระของผปวยและครอบครวในการแลกกบบรการทไดรบในประเทศไทย มการศกษาทโรงพยาบาลศรราช 17 ระหวางป 2543-2546 พบจ านวนผปวยเดกโรคหดทตองรกษาในโรงพยาบาลเพมขนจาก 113 คน เ ปน 147 คน และ 176 คน ในจ านวนนเปนผปวยเดกทมอายนอยกวา 5 ป ถง รอยละ 72 คดเปนคาใชจาย 3,236 บาทตอคนตอการนอนรกษาในโรงพยาบาล หรอ 998 บาทตอวนการศกษาของโรงพยาบาลมหาราช นครเชยงใหม 18 พบมผปวยโรคหดทตองรกษาในโรงพยาบาลจ านวน 201 คน ซงเ ปนผปวยเ ดก 32 คน มคาใชจายโดยเฉลย 8,009 บาทตอคนตอป โดยเปนคาใชจายจากคายาและบรการทางการแพทยถงรอยละ 83.9
3. กำรศกษำคณภำพชวตดำนสขภำพ ปจจบนคณภาพชวตจดเปน Humanistic outcome หนง ทส าคญในการบงชความส าเ รจของ
การรกษานอกเหนอไปจาก Outcome ทางดาน คลนกและ Outcome ทางดานเศรษฐศาสตร จากการศกษาพบวา คณภาพชวตสามารถใชในการท านาย Health service utilization และ Mortality ของผปวยได 19, 20

คณภำพชวตดำนสขภำพ หมายถง “คณคาทใหแกชวงเวลาของชวตทเปลยนแปลงเนองมาจากความเจบปวย สภาพของการท างานของรางกาย ความคดเหนและโอกาสทางสงคมทไดรบอทธพลจากโรค การบาดเจบ การรกษาพยาบาลและนโยบายสขภาพ ” ในความหมายดงกลาวครอบคลมสาระ 4 ประเดนดงน 1. อาการของความเจบปวย(Symptoms) ขอบงชโรค(Signs) และการบนตอความ
เจบปวย (Complaints) 2. ผลของการวนจฉยโรคทเ ปนอย 3. สถานะการท างานของรางกายในดานกายภาพ จต สงคม และความทรงจ า รวมทง
ความจ ากดของการด าเนนกจวตรประจ าวน และสมรรถภาพทางรางกาย 4. ความคดเหนหรอความพงพอใจตอสขภาพ
3.1 ดชนคณภำพชวตดำนสขภำพ แบงออกเปน 2 ประเภทไดแก ดชนแบบทวไป เ ปนเครองมอทใชวดความเ จบปวยทวไปไมเฉพาะโรค ขอดคอ
สามารถน าผลทไดไปเปรยบเทยบและวางแผนส าหรบการเจบปวยทแตกตางกนได ขอเ สยคออาจไมสามารถแสดงความแตกตางทเฉพาะโรคนนๆได ตวอยางเครองมอ ไดแก EuroQol-5D (EQ-5D), SF-36, PedsQol
ดชนแบบเฉพำะโรค เปนเครองมอทมการพฒนาใหใชจ าเพาะโรค จงใหความละเอยด แตไมสามารถน าไปใชกบโรคอนและผลทวดไดกไมสามารถเปรยบเทยบกบโรคอน เชน The Arthritis Impact Measuring Scales (AIMS), Barthel Index, Pediatric Asthma Quality of Life Questionnaire (PAQLQ)
3.2 กำรวดคณภำพชวตในเดก เนองจากคณภาพชวตขนกบการรบรของแตละคน21 ดงนน การวดคณภาพชวตในเดกจ าเปนตองใชเครองมอทเฉพาะเจาะจงกบเดกโดยเฉพาะ ทงนจ าเปนตองมการวดทงในตวเ ดก (Child self-report) และ ในผปกครอง (Proxy-report) เพอใหไดครอบคลมในทกมมมอง 22
4. กำรพฒนำแบบสอบถำมจำกภำษำตำงประเทศเปนภำษำไทย จากการทบทวนวรรณกรรม23 พบวา ขนตอนการแปลแบบสอบถามจากภาษาตางประเทศเปนภาษาไทยนนประกอบดวยขนตอนตางๆ ดงตอไปน
1. แปลจากตนฉบบภาษาองกฤษ เปนภาษาไทย (Forward translation) 2. แปลจากภาษาไทยในขอ1เปนภาษาองกฤษโดยผแปลอกคนหนง ทไมเคยเหนตนฉบบ ภาษาองกฤษ (Backward translation)

3. น าสองฉบบทเปนภาษาองกฤษในขอ 1 และ 2 มาเปรยบเทยบกน และปรบเปลยน ส านวนใหเปนภาษาทเหมาะสมจนกระทงไมมความแตกตางระหวาง 2 ฉบบ และท าการบนทกปญหาตางๆ ทเกดขนจากการแปลในทกขนตอน รวมทงปญหาทางดานความแตกตางของภาษาและวฒนธรรม
4. ท าการทดสอบแบบสอบถามทแปลขนได 4.1 ท าการทดสอบขนตนในกลมเ ลกๆ โดยมวตถประสงคเพอคนหาปญหาทอาจ
เกดขนจากความเขาใจ และภาษาทใชในแบบสอบถาม ทงนกลมตวอยางควรมลกษณะทางประชากรและสงคมทแตกตางกนเพอจะไดมนใจวาแบบสอบถามทสรางขนเหมาะสมกบประชากรทกกลม 23

5. เครองมอทมกำรพฒนำเปนภำษำไทย เครองมอวดคณภาพชวตแบบทวไปทนยมใชกนอยางแพรหลายและมการพฒนาเปน
ภาษาไทย ไดแก 5.1 แบบวด คณภำพชว ต EuroQol-5D (EQ-5D) เ ปนแบบวดคณภาพชวตแบบทวไป
(Generic quality of life instrument) โดยไมขนอยกบอาย เพศ หรอสภาวะโรคทมอย นอกจากจะใชวดคณภาพชวตทางดานสขภาพแลว ยงสามารถน ามาใชเพอประเมนความคมคาทางดานเศรษฐศาสตรของการใหบรการทางดานสขภาพไดอกดวยเนองจากแบบวดคณภาพชวต EQ-5D สามารถใหคะแนนทเปนตวเลขเดยวๆทเ รยกวา อรรถประโยชน (Utility) ซงมคะแนนอยในชวง 0-1 โดยสามารถน าคะแนนอรรถประโยชนนมาค านวนหาจ านวนปทมชวตทปรบดวยคณภาพชวต (Quality Adjusted Life Years หรอ QALYs) ซง QALYs เ ปนผลลพธทส าคญและเปนทนยมใชกนมากของการประเมนตนทน-ประสทธผล (Cost-effectiveness analysis) ทางดานสขภาพ 24,25 แบบวดคณภาพชวต EQ-5D หรอชอเตมวา EuroQoL มการพฒนาขนทประเทศองกฤษ 26 โดยถามถงภาวะสขภาพในวนนของผตอบในมตทางดานสขภาพทงหมด 5 ดานคอ การเคลอนไหว (Mobility) การดแลตนเอง (Self-care) การท ากจกรรมในชวตประจ าวนตามปกต (Usual activities) ความเ จบปวดหรอความร สกไมสบาย (Pain/discomfort) และความวตกกงวลห รอความซมเศร า (Anxiety/depression) และในมตแตละดานมระดบความรนแรงของภาวะสขภาพอย 3 ระดบคอ ไมมปญหา มปญหาปานกลาง หรอ มปญหารนแรง

ส าหรบอรรถประโยชนสามารถค านวณไดจากสตรทถกสรางขน ในประเทศองกฤษ จากการท าส ารวจผใหญ จ านวนประมาณ 3,000 คน27 และส าหรบประเทศไทยในขณะนก าลงมการสรางสตรค านวณส าหรบคนไทย โดยทมงานของกระทรวงสาธารณสข โดยคาดวาจะแลวเสรจในป 2551 แบบวดคณภาพชวต EQ-5D นนผตอบแบบสอบถามสามารถทจะท าไดดวยตนเอง (Self-administered) เวลาทใชประมาณ 1-2 นาท และมคณสมบตของการวดทดทงคณสมบตทางดานความเทยงตรง (Reliability), ความถกตอง (Validity), การตอบสนองตอการเปลยนแปลงของสขภาพในอนาคต (Responsiveness) 28-31 อยางไรกตาม EQ-5D อาจจะมปญหาในเ รองของการม Ceiling effects (คะแนนสงสดของเครองมอ) ทสง 32-34 แตโดยภาพรวมดงทไดกลาวมาตงแตตนวา EQ-5D เ ปนเครองวดคณภาพชวตแบบทวไปทดใหประโยชนทงกา รวดคณภาพชวตและการน ามาใชในการประเมนทางดานเศรษฐศาสตร, สน และงายเมอเทยบกบหลายเครองมอทมอย ดงนนในปจจบนจงไดมการน า EQ-5D มาใชกนอยางแพรหลายในหลายประเทศทวโลก25 ใน สวนของประ เ ทศไทยได มการทดสอบเ ครอง มอ EQ-5D ในแง ของ การยอมรบ (Acceptability) และความถกตอง ในผปวยทตดเชอเอชไอว35 พบวาผตอบสวนใหญเหนวาเครองมอนเขาใจงายและไมมผใดปฎเสธการไดรบสมภาษณ เวลาทใชในการสมภาษณโดยเฉลย 1.1 (SD = 0.4) นาทและคะแนนอรรถประโยชนของ EQ-5D มความสมพนธในระดบทด (Spearman’s rho = 0.66) อยางมนยส าคญทางสถต (p < 0.01) กบอาการทเ กยวของกบการตดเชอเอชไอว และในอนาคตอนใกลนประเทศไทยนาจะมการน า EQ-5D มาใชกนมากขน เนองจากเปนเครองมอทไดรบการสนบสนนใหมการใชในการหาอรรถประโยชนและอกไมนานกจะมสตรค านวณอรรถประโยชนส าหรบคนไทยโดยตรง มการศกษาทน า EQ-5D มาใชในการวดคณภาพชวตในเดกบาง 36-39 โดยสวนใหญจะใชตวแทน (Proxy) เชน ผปกครอง ถาเดกอายนอยกวา 7 ปมกจะใหผปกครองเปนคนท าแบบสอบถามให ถาอายระหวาง 7-12 ป เดกสามารถท าเองได (หรอดวยความชวยเหลอของผปกครอง ) ถามากกกวา 12 ป สามารถท าดวยตนเองได 6. กำรศกษำภำระของกำรเจบปวยและคณภำพชวตดำนสขภำพของผปวยโรคหดในเดก Pediatric Asthma Quality of Life Questionnaire(PAQLQ) เ ปนเครองมอทมการพฒนาโดย Juniper และคณะ40 ใชจ าเพาะส าหรบผปวยเดกโรคหด ใชประเมนในเดกอาย 7-17 ป40 ซงงายตอบการตอบและเขาใจไดอยางถกตอง ประกอบดวย 23 หวขอ ในมตทางดานสขภาพ 3 ดานคอ การมขอจ ากดในการท ากจกรรมในชวตประจ าวน (Activity limitation domain) อาการของโรคหด (Symptoms domain) และอารมณ (Emotional functions domain) โดย Activity limitation domain ประกอบดวย 5 หวขอทเกยวของกบการวงเลน การเ ลนกฬา และกจกรรมตางๆ ใน Symptoms domain ประกอบดวย 10 หวขอ เชน อาการไอ อาการหายใจมเสยงวด การมอาการของโรคหดจนตองตนขนตอนกลางคน

เปนตน Emotional function domain ประกอบดวย 8 หวขอ เชน อารมณตนตระหนก อารมณกระวนกระวาย อารมณวตกกงวล เปนตน สามารถใหคะแนนทเปนตวเลขจาก 1 (มผลกระทบหรอสญเสยมากทสด) จนถง 7 (ไมมผลกระทบหรอสญเสยเลย) จงใหความละเอยดและจ าเพาะแกโรค แตไมสามารถน าไปใชกบโรคอน และผลทวดไดกไมสามารถเปรยบเทยบกบโรคอน ใชไดในทกความรนแรงของอาการของโรคหด และไวตอการเปลยนแปลงทเกดขนเพยงเลกนอย 41 มการศกษาถงผลกระทบตอคณภาพชวตดานสขภาพในผปวยเ ดกโรคหดทโรงพยาบาลศรราช 42 ผปวยเ ดกจ านวน 51 คน อายระหวา ง 7-17 ป โดยใชแบบวดคณภาพชวต PAQLQ ฉบบภาษาไทย พบวาม High internal consistency ส าหรบคะแนนในทง 3 -coefficient = 0.83–0.95) ในผปวยเ ดกโรคหดทอาการของโรคสงบ มความเ ทยงตรงของแบบวดคณภาพชวต PAQLQ ทด ส าหรบการจ าแนกอาการ (Intra-class correlation coefficient – ICC = 0.84) และส าหรบคะแนนโดยรวมทงหมด บอกถงความถกตองและเ ทยงตรงของแบบวดคณภาพชวต PAQLQ ส าหรบใชในคนไทยได ปจจบนยงไมมการศกษาเกยวกบการดแลผปวยเ ดกโรคหดในประเทศไทยอยางเ ปนระบบ ทงในดานการดแลรกษาดวยยาทเหมาะสม และการประเมนถงคาใชจายในการรกษา ตามสภาพของโรคในผปวยเดกแตละราย การประเมนถงการสญเสยทางเศรษฐกจของครอบครวกบการเ จบปวยจากโรคหด ผลกระทบตอคณภาพชวตของผปวยเดกโรคหด และ ผลการด าเนนโรคในระยะยาวของผปวยเดกโรคหด ปจจยจากสภาพสงแวดลอมและมลพษในอากาศทมผลตอการเ จบปวยจากโรคหดในเดก ท าใหไมมขอมลเกยวกบโรคนในประเทศไทย พอทจะแสดงใหเหนถงความรนแรงของปญหาโรคหดในประเทศไทย ผลของการรกษาโรค ผลการด าเนนโรคใ นระยะยาว การพยากรณโรค ตลอดจนผลกระทบของโรคหดตอคณภาพชวตของผปวยเดกโรคหด เพอใหความรและค าแนะน าแกผปวยและหนวยงานทเกยวของ ขอมลความรจากตางประเทศ อาจมความแตกตางจากสภาพความเปนจรงในประเทศไทย และตองมการปรบใชตามความเหมาะสม โดยเฉพาะกบระบบเศรษฐกจแบบพอเพยงของประเทศไทย และเนองจากโรคหดมความหลากหลายสงทงในดานสาเหต การแสดงออกและความรนแรงของโรค แนวทางการรกษาและผลลพธจงมความแตกตางกน การศกษาแบบสหสถาบนจะชวยใหไดขอมลและสะทอนภาพรวมของโรคหดในประเทศไทยมากขน โครงการวจ ยน จง เ ปนการศกษาเบองตนเพอใหทราบถงระบาดวทยาของโรคหดในเดกในประเทศไทย การดแล คาใชจาย และผลกระทบตอเศรษฐกจตอครอบครวของผปวย ปจจยของสงแวดลอมทมผลตอโรค ดวยความรวมมอของหนวยโรคภมแพและอมมโนวทยา ของสถาบนการแพทยทงในสวนกลาง และสวนภมภาค อนจะเปนพนฐานตอการปรบรปแบบการดแลผปวยเดกโรคหดทเหมาะสมกบประเทศไทย ตลอดจนการวจยในเชงลกตอไปในอนาคต

3.2 วธกำรคดเลอกผปวยเขำโครงกำร เกบผปวยเดกโรคหดทกราย ตามเกณฑการวนจฉยในแตละชวงอาย ทไดรบการรกษาทหนวยโรคภมแพและวทยาภมคมกนในเดก จากสถาบนทรวมในโครงการทง 13 แหง ในชวงระยะเวลาตงแตวนท 1 กนยายน 2550 ถง วนท 30 มถนายน 2551 โดยมผปกครองทเปนผดแลผปวย ทสามารถพาผปวยมาตดตามการรกษาตามนดไดอยางสม าเสมอ ถกคดเ ลอกเขาในโครงการ โดยมแพทยหรอพยาบาลทอยในทมผดแลผปวยและเปนผวจยรวมของโครงการฯในสถาบนนน เปนผของความยนยอมจากผปกครองของผปวย ตามในใบขอความยนยอมเขารวมในโครงการโดยความสมครใจ

2) ตนทนทางตรงทไมเกยวกบการแพทย เชน คาใชจายในการเดนทาง คาจางผดแล 3) ตนทนทางออมไดแกตนทนทเกดจากการทผปวยหรอผดแล จากการขาดงานหรอ ประสทธภาพทลดลงของผปวย และการเสยชวตกอนวยอนควร
ตวแปรอสระ (Independent variables) ประกอบดวย 1) อาย 2) ชนดของโรค 3) โรคแทรกซอน 4) ฐานะทางเศรษฐกจและการศกษา 5) ประเภทของผปวยตามสทธทางการจายเงนในการรบรกษา 6) ปจจยดานสถานพยาบาล 7) ประเภทสถานพยาบาล
มมมองหรอทรรศนะของกำรศกษำ (Perspective) การศกษาครงนครอบคลมทงของผปวย และสงคม 3 Humanistic outcomes คะแนนทไดจาก แบบสมภาษณคณภาพชวต EQ-5D, PAQLQ - วธกำรเกบขอมล
1. การ คดลอกขอมลคลนกและ การรกษาจากบนทกการรกษาของผปวย โดยเจาหนาทเกบขอมลจากนนท าการตรวจสอบและสรปตามลกษณะตวแปรทตองการโดยนกวจย
2. สมภาษณ ผปวยและ /ห รอญา ต ส าหรบขอมล ตนทนทางตรง ทไมเ กยวกบ การแพทยและตนทนทางออม ท าการสมภาษณผปวยทกครงทนดมาพบแพทย ซงครอบคลมในกรณทผปวยไปรบการรกษาทสถานบรการอนๆและคาใชจายในการรกษาดวย แตละครงมแบบสอบถาม 1 ชด และแบบเกบการบรการทางการแพทย 1 ชด

ระยะเวลา 5 ป ใช Multiple regression ในการวเคราะหความสมพนธระหวางตวแปรทางคลนกตางๆกบระดบความรนแรงของโรค และการตอบสนองตอการรกษา ขอมลดานตนทนการเจบปวย ตวแปรการรกษาตางๆ ตามระดบความรนแรง ของโรคในผปวย ใช Multiple regression ในการวเคราะหแบบจ าลองสมการตนทน ทสามารถประมาณการตนทนของผปวยลกษณะตางๆ และมการวเคราะหความออนไหวของผลการศกษาจากการเปลยนคาของตวแปรทเ กยวของกบตนทนในการรกษาพยาบาล ไดแก ราคายา ขอมลการศกษาคณภาพชวตดานสขภาพ (Quality of life) ทไดจากแบบสมภาษณคณภาพชวต EQ-5D และ PAQLQ มการวดซ าทก 6 เดอนและวเคราะหการเปลยนแปลงโดย EQ-5D จะแปรผลจาก Domain scores และ Utility scores และใชสตรค านวณส าหรบคนไทยทไดจากการศกษาน ารองของกระทรวงสาธารณสข ในการค านวณจ านวนปทมชวตทปรบดวยคณภาพชวต (Quality Adjusted Life Years หรอ QALYs) ซงใชในการค านวณการประเมนตนทน -ประสทธผล (Cost-effectiveness analysis) ทางดานสขภาพ และค านวณ Sensitivity analysis
7. ผลกำรด ำเนนงำนทไดด ำเนนกำรไปแลว การเกบขอมลผปวยในโครงการฯตอเนองปท 2 ของสถาบนตางๆในโครงการมผปวยเ ขารวมโครงการ ปท 2 จ านวน 425-485 ราย คดเปนรอยละ 87.1-90.5 จากจ าน วน ผปวยท รวมโครงการวจยเดม ขอมล CRFs ทไดถกสงมาท าการบนทกขอมลในคอมพวเตอร ทหนวยบรหาร จดการขอมล กลมงานระบาดวทยาคลนกและชวสถตคณะแพทยศาสตร โรงพยาบาลรามาธบด โดยการขนสงขอมลทางไปรษณยสวนใหญ ขอมลทเขาส DMU ถกเชคและตรวจสอบความเปนไปไดตลอดจนความครบถวนของขอมล โดย นพ.สวทย เลศขจรสน ซงถาหากมความไมชดเจนไดตดตอกลบไปยงสถาบนรวม เพอแกไขขอมลใหถกตองกอนด าเนนการบนทกขอมลลงคอมพวเตอร

บทท 4 ผลกำรศกษำ
ขอมลพนฐำนของผปวยในโครงกำร ขอมลพนฐานของผปวยในโครงการ เปนผปวยเดกโรคหดจ านวนทงสน 485 ราย แบงเ ปนเพศชาย 304 คน และเพศหญง 181 คน อายเฉลยของผปวยในโครงการคอ 11 ป โดยอายทผปวยเ รมมอาการของโรคหดเฉลย 24 เดอน (range 1-177 เดอน) และไดรบการวนจฉยวาเ ปนโรคหดทอายเฉลย 60 เดอน (range 2-179 เดอน) มคาดชนมวลรางกายเฉลย 19 กโลกรม/ตารางเมตร ในผปวยเดกโรคหดทศกษามโรคภมแพอนๆรวมดวยคอ โรคเ ยอบจมกอกเสบจากภมแพ รอยละ 81.2 โรคผนภมแพทางผวหนง รอยละ 19 และโรคแพอาหาร รอยละ 13.4 มผปกครองทเปนผดแลหลกคอ มารดารอยละ 79.4 บดารอยละ 11.1 และญาตหรอคนในบานอกรอยละ 9 โดยมคนสบบหรในบานสงถงรอยละ 30.9 และมการเ ลยงสตวเ ลยงในบานรอยละ 37.9 ปจจยทางพนธกรรมของโรคภมแพในครอบครว พบวา ผปวยมบดาหรอมารดาเปนโรคหดรวมดวยรอยละ 22.7 และมพนองของผปวยเปนโรคหดรวมดวยรอยละ 10.5 สวนโรคภมแพอนๆ ดงแสดงในตารางท 4.1 ขอมลกำรรกษำดวยยำของผปวยเดกโรคหดในโครงกำร การรกษาดวยยาของผปวยเดกโรคหดในโครงการฯ มการรกษาดวยยาคมอาการและปองกน (controllers) โดยไดรบยาสดชนดสเตยรอยดจ านวน 224 คน คอยา budesonide 209 คน , fluticasone 8 คน และ beclomethasone 7 คน ซงสามารถแบงตามขนาดยาทผปวยไดรบคอ มผปวยรอยละ 18 ทก าลงใชยาขนาด low dose inhaled corticosteroids (< 200 mcg/day) ผปวยรอยละ 34 ใชยาขนาด medium dose inhaled corticosteroids (200-400 mcg/day) และผปวยอกรอยละ 48 ใชยาในขนาด high dose inhaled corticosteroids (> 400 mcg/day) มผปวยจ านวน 74 คนทไดรบยาในกลม leukotriene modifier (montelukast) และมผปวยอกจ าน วน 1 7 8 คน ทไ ดรบยา combined inhaled corticosteroids + long-acting beta-2 agonist คอ fluticasone/salmeterol จ านวน 140 คน และ budesonide/formoterol จ านวน 38 คน ดงแสดงในตารางท 3 การประเมนผลการรกษาโดยดจากการควบคมไดของโรคหด (level of asthma control) ในผปวยรายเกาจ านวน 437 คน โดยมอาการของโรคหดทอยในระดบ controlled จ านวน 284 คน , ในระดบ partly-controlled จ านวน 120 คน และ ในระดบ uncontrolled อก 33 คน สวนในผปวยเ ดกโรคหดรายใหมทเขารวมในโครงการฯ มจ านวน 46 คน ซงแบงความรนแรงของโรคเปน mild intermittent

asthma จ านวน 7 คน , mild persistent asthma จ านวน 23 คน , moderate persistent asthma จ านวน 16 คน และไมมผปวยเดกโรคหดในระดบ severe persistent asthma เลย ส าหรบภาวะการแพตอสารกอภมแพทส าคญในผปวยเ ดกโรคหดของการศกษาครงน พบวา มผปวยเดกโรคหดทแพตอไรฝนบานทงชนด Dermatophagoides pteronyssinus (Der p) รอยละ 69.9 และ Dermatophagoides farina (Der f) รอยละ 67.4 สวนสารกอภมแพอนๆทพบรองลงมาคอ แมลงสาบ รงแคแมว และหญาแพรก ดงตารางท 4.3

Asthma remission ของผปวยเดกโรคหดในโครงกำร
ไดท าการวเคราะหขอมลในสวนของผปวยเดกโรคหดจ านวน 454 รายทมาตดตามการรกษาตอเนองในระยะเวลาทคามธยฐาน 1.5 ป (0.13-2.4 ป)
ภาวะ Asthma remission ของผปวยเดกโรคหดในการศกษาน ซงนยามโดย ตองมอาการของโรคห ดคมได (Clinical asthma controlled) เ ปน ระยะเวลาอย างน อย 12 เ ดอน และไมม asthma exacerbation เลย พบวา ในจ านวนผปวยทมาตดตามการรกษา 577.5 ครง -ป อตราการเ กด asthma remission คอ 76.5 ตอผปวยเดกโรคหด 100 คนตอป โดยมคามธยฐานของระยะเวลาทจะเ ขาส asthma remission 1.38 ป (95% confidence interval 1.32-1.42 ป) หรออาจบอกไดวา รอยละ 50 ของผปวยเ ดกโรคหดจะม asthma remission ทระยะเวลา 1.38 ปหรอมากกวา
การวเคราะหปจจยทมผลตอการเ กด asthma remission โดยวธ univariate analysis พบวาม 10 ปจจยทมแนวโนมทจะมผลตอการม asthma remission (p value < 0.15) คอ อาย, คาดชนมวลรางกาย (body mass index, BMI), ผดแลผปวยทบาน , การศกษาของผปกครอง , โรคภมแพรวมของผปวย คอ โรคจมกอกเสบจากภมแพ หรอ โรคผนภมแพผวหนง , การมพอ-แมเ ปนโรคหด, การมพ-นองเปนโรคหด หรอโรคผนภมแพผวหนง และประวตการไดรบวคซนปองกนไขหวดใหญ (ตารางท 4.4)
หลง จากว เคราะห เพ มเ ตมตอ ดวย multivariate Cox regression โดยปรบดวย ตวแปร confounding พบวา ดชนมวลรางกาย, การศกษาของผปกครอง และการมประวตพ -นองเปนโรคภมแพ เปนปจจยทมผลตอการเกด asthma remission คอ ผปวยเ ดกโรคหดทมดชนมวลรางกายทมากกวา 18 ก.ก./ม.2 จะมโอกาส 1.4 เทาทจะม asthma remission มากกวาผปวยเ ดกโรคหดทมดชนมวลรางกายทนอยกวา 18ก.ก./ม.2 ผปวยเดกโรคหดทไมมประวตพ-นองเปนโรคภมแพ จะมโอกาสรอยละ 80 ทจะม asthma remission มากกวาผปวยเ ดกโรคหดทมพ -นองเปนโรคภมแพ และผปวยเ ดกทมพอ -แมจบการศกษาระดบปรญญาตรขนไปจะมโอกาสรอยละ 34 ทจะม asthma remission มากกวาผปวยเ ดกทมพอ-แมจบการศกษาระดบต ากวาปรญญาตร (ตารางท 4.5)

โรคหด ทใชวธการเกบขอมลจากการตอบแบบสอบถามโดยผปกครอง เปนผตอบแบบสอบถาม มปญหาเกยวกบขอมลในแบบสอบถามทมกจะไดไมครบ หรอบางครงเกดการสญหายของแบสอบถามในระหวางนดตดตามอาการทก 3 เดอน ดงนนทางทมผวจยและทม DMU ไดปรกษากน เหนดวยทจะไมน าขอมลทไดจากแบบสอบถามมาวเคราะห แตไดหาวธการทจะไดขอมลในสวนของคาใชจายในการดแลผปวยเดกโรคหด เพอทจะไดน ามาแสดงใหเหนถงผลกระทบของโรคหดตอครอบครว รวมถงสงคม และประเทศ จงพจารณาวเคราะหขอมลคาใชจายในสวนของ คาใชจายโดยตรงจากยาทรกษาโรคหด (Direct medical cost) โดยพจารณาเกบรวบรวม และวเคราะหขอมลจากสถาบนฯ โรงพยาบาลรามาธบด เปนแหงแรกกอน ซงสามารถทจะเกบขอมลผปวยไดครบถวน และมฐานขอมลในโรงพยาบาล ทสามารถน ามาวเคราะหได โดยมทปรกษา คอ ดร .ภญ.อรลกษณ พฒนาประทป นกวชาการเวชสารสนเทศ ฝายเวชสารสนเทศ โรงพยาบาลรามาธบด ซงมความช านาญในดานHealth economics เปนอยางด รวมวเคราะหขอมลดวย
คาใชจายทางการแพทยจากยารกษาโรคหดในผปวยเดกโรคหด ทรกษาในโรงพยาบาลรามาธบด ทรวมในโครงการ จ านวน 59 คน โดยมคามธยฐาน (median) ของคาใชจายจากยารกษาโรคหด 6,571 บาทตอคนตอป ซงมผปวยจ านวน 1 รายทมอาการหอบตองมารกษาทหนวยฉกเฉนและตองรบการรกษาในโรงพยาบาล โดยมคาใชจายทหองฉกเฉน (ER) 602.50 บาท และคาใชจายจากการนอนรกษาตวในโรงพยาบาล 15,391.25 บาท
ในผปวยเดกโรคหดทรกษาทโรงพยาบาลรามาธบด มจ านวนครงของการมาตดตามอาการทงหมด 277 ครง ซงเปนการมาตรวจตดตามอาการตามปกตทหนวยผปวยนอก (OPD.) 272 ครง และมาตรวจดวยอาการ asthma exacerbation ท OPD. จ านวน 4 ครง และท ER. อก 1 ครง โดยเปรยบเทยบคาใชจายในผปวยทง 2 กลมน ในดานคาใชจายทงหมด (รปท 1) คาใชจายจากการรกษาเฉพาะท OPD. (รปท 2) และคาใชจายจากกลมยาเฉพาะ คอ ยาสด inhaled corticosteroids (ICS) (รปท 3) , ยาตานสาร leukotr ienes (LTRA) ( รปท 4) , ยาสดขยายหลอดลม short-acting beta2 agonists (SABA) ( รปท 5) และยาปฏชวนะ (Antibiotics, ATB) ( รปท 6) ซงไมพบความแตกตางทางสถตระหวางกลมแตอยางใด

บทท 6 สรปผลกำรศกษำและขอเสนอแนะ
การศกษาผลลพธทางคลนก ทางคณภาพชวต และทางเศรษฐศาสตรของการรกษาระยะยาวในผปวยเดกโรคหดในประเทศไทยครงน ซง เ ปนการศกษาแบบสหสถาบนครงแรกในประเดนคงกลาว พบวาในผปวยเดกโรคหดของไทย โดยสวนใหญมกมอาการของโรคหด หลงจากไดรบการรกษาอยางเหมาะสมแลว อ ยในภาวะทสามารถควบคมอาการของโรคไดเ ปนอยาง ด (Clinical asthma well controlled) ถงรอยละ 64.5 โดยทจะมผปวยเดกโรคหดเพยงรอยละ 7.5 ทยงคมอาการของโรคหดไมได (Clinical asthma uncontrolled)
ในการศกษาตดตามผปวยเดกโรคหดกลมนเปนระยะเวลาประมาณ 2 ป พบวา มอตราการเ กดการสงบไดของโรคหดในเดก (Asthma remission) ซงไมมการก าเ รบของการจบหดเฉยบพลนและมอาการของโรคหดทควบคมไดอยางนอย 12 เดอน คอ 76.5 ตอผปวยเ ดกโรคหด 100 คนตอป โดยมคามธยฐานของระยะเวลาทจะเขาส asthma remission 1.38 ป หรออาจบอกไดวา รอยละ 50 ของผปวยเ ดกโรคหดจะม asthma remission ทระยะเวลาประมาณ 1.4 ปหรอมากกวา เปนขอมลทบอกไดถงพยากรณของโรคหดในเดกทดมาก โดยเฉพาะอยางยงหากไดรบการรกษาดวยยาควบคมอาการของโรคอยางเหมาะสม ตามเกณฑมาตรฐานการรกษาโรคหดในเดก โดยแพทยผเชยวชาญทางดานโรคหดและโรคภมแพในเดก ซงสะทอนโดยการศกษาในครงนทท าในสถาบนฯทเปนผเชยวชาญในการดแลผปวยโรคหดในเดก

ถาประมาณการจ านวนประชากรในประเทศไทยในป พ .ศ. 2552 มจ านวนประชากรทงสน 63,396,000 คน มจ านวนประชากรเดกอายระหวาง 5-21 ป ประมาณ 15,192,000 คน และมความชกของ

โรคหดอยรอยละ 10 จะมผปวยเดกโรคหดประมาณ 1.519 ลานคน และใชตวเลขคาใชจายจากยารกษาโรคหดประมาณ 6,500 บาทตอคนตอป เราสามารถประเมนคาใชจายจากยาในการรกษาโรคหดในเดก ถงปละ 9,874 ลานบาท ซงเปนการสญเสยทางเศรษฐกจของประเทศไทยอยางยง
การศกษาถงผลกระทบของโรคหดในผปวยเ ดก และครอบครว ของประเทศไทย ในเชงลกตอไป เพอศกษาถงสาเหตการเ กดโรคหด ปจจยเสรมทกระตนใหอาการของผปวยเ ดกโรคหดแยลง การศกษาถง ชนดของยารกษาโรคหด ทงในเชงลก ทางดานเภสชพนธกรรมศาสตร (Pharmacogenetic) ตลอดจนในดาน cost effectiveness ของการรกษาโรคหดดวยยาทเหมาะสมกบผปวยเ ดกไทย จะชวย ในเชงนโยบายประกนสขภาพของโรคหด ทเ ปนโรคในเ รอรงในกลม non-communicable diseases ทเหมาะสมตอไป

INTRODUCTION
Asthma is a chronic inflammatory disorder of respiratory system. It leads to recurrent
episodes of wheezing, breathlessness, tightness of chest, and coughing, particularly at
night or in the early morning. It is usually diagnosed based on clinical manifestations (1).
Prevalence of asthma in children varies according to countries and various criteria used for
diagnosis, approximately 300 million people around the world are affected (1). For
instance, the prevalence was 8 to 26% in school children aged 6-7 and 13-14 reported by
the international study of asthma and allergies in childhood (ISAAC) (2, 3) identified
asthma using a standardized questionnaire(4), and it was 14.5% in Thailand, (5).
Once there is a member of family is diagnosed as asthma, it is socio-economic burden to
the family, the community, and also the country. It required 9.3 million physician-office-
visits, 500,000 hospitalizations, and 5,500 deaths (1-6.5 per 10,000) each year in the
United States (6, 7). The cost of treatment varies between countries according to disease
severity, numbers of hospitalizations and emergency-room(ER) visit, medicines, etc. For
instance, estimated annual cost in1998 exceed US$ 2,500 per patient in the USA(6), £381
and £108 for exacerbated and non-exacerbated patients in the UK (8), 3,236.20 baht per
one admission or 998.60 baht per hospital day in Thailand(9).
For social burden, there were 6.3 million school-absence days in the USA which resulted
in loss of parental productivity about 719.1 million US$ (7). In addition, it also affected
on disability-adjusted life years (DALYs) lost, which accounted about 1% (25th rank) of
all reported worldwide DALYs lost (currently reported about 15 million per year)(1).

Furthermore, it was a cause of worldwide deaths with approximately 1 in every 250 deaths
(10).
The goal of management of asthma is disease remission. Knowing prognostic factors that
associate with disease remission or exacerbation should be able to encourage patients to
reach the goal. Two large studies (11, 12) in the USA had identified risk factors of
exacerbation outcomes (i.e., hospitalization and ER-visits) in children, such as age, oral
steroid intakes, and number of physicians, or medication co-payment. Since these studies
were based on data registry of insurance companies, representativeness of cases was
questionable since these studies were focused on only patients whose could effort for
private insurances. In addition, some clinical or behavior data (e.g., allergy, passive
smoking, exercise, treatments) were not considered.
Some studies had reported prognostic factors (e.g. percent FEV1, eosinophilia, house-dust
mite sensitization, smoking, airway hyperresponsiveness, and sex) of disease remission
(13),(14),(15),(16). The remission rates were varied from 15 to 30% (13),(14),(15),(16).
These studies had followed-up patients for 6 to 30 years and temporal relationship
between prognostic factors and disease remission could be therefore assessed according to
the study designs. However, diagnosis of disease remission was mostly based on
interviewing without approving by medical and physical examination data. Ascertainment
bias might be thus present. In addition, some other variables might influence to
exacerbation or remission rate (e.g., breastfeeding (17),(18), physical activity and
exercise(19), and influenza immunization(20) have not been considered in these studies.
The goals of asthmatic management are not only improvements of health but also quality
of life of patients and parents as for the definition of health(21). Health related quality of
life (HRQoL) has been defined(22) as “the individual’s subjective perception of the
impact of their health status, including disease and treatment on physical, psychological
and social functioning”. Assessing the HRQoL can be used for monitoring results of

treatments, compare different treatment regimens, and economics evaluations. It is
classified as generic and specific HRQoL. The former type is able to apply to general
population regardless age, gender, or health status and it is used to compare across
interventions, conditions, and other diseases. Conversely, the later type HRQoL is more
specific in particular condition or disease/s that affect/s on patient’s perceptions.
The HRQoL questionnaires can be used for economics evaluations if their scores are
indexed or utilized (i.e., net impacts of HRQoL) to be between 0 and 1. Well known
generic HRQoL scores that are commonly used in medical research are EQ-5D (EuroQol)
(23) and SF-36. Both EQ-5D and SF-36 are profiles scores that have been translated and
validated to be Thai versions (24). The SF-36 is later modified to be SF-6D which can be
transformed to utility score as the EQ-5D by select 11 items from 6 dimensions (i.e.,
physical functioning, role participation, social functioning, bodily pain, mental health, and
vitality). It had a number of sets of prevalence weight that differ between countries but
there is no preference weight for Thailand (25),(26),(27). Although some studies showed
that SF-6D had good discriminatory power (28),(29). in classifying health status in
patients with some conditions (e.g. low back pain, diabetes), the EQ-5D which consists of
5 domains and 5 items with a scale of 3 levels, is required less concentration and less time
consume than the SF-6D. As a result, the EQ-5D should be more suitable for applying in
children than the SF-6D.
The pediatric asthma quality of life questionnaire (PAQLQ) had been developed and used
for assessing specific HRQoL (30). This questionnaire has also been widely used in
children aged 7 years or older and it has been translated and validated in Thai patients
(31). The assessment can reflect only patient’s perception of asthma but cannot use for
economics evaluations as the EQ-5D can.

As for our knowledge, not many prognostic studies of disease remission were available in
literatures across the world and even there is no evidence in Thai asthmatic-children.
Within those available studies, some prognostic factors have not been considered whereas
some were considered but inadequate measurements. In addition, different environmental
factors between Thailand and aboard may result in different effects on clinical outcomes.
Furthermore, HRQoL has never been studied and estimated in Thai asthmatic children and
thus we still lack of information for another outcome management. We therefore conduct a
cohort study with following research questions and objectives.
OBJECTIVES
Primary objectives
- To estimate incidence of disease remission in asthmatic children.
- To determine predictive factors of disease remission.
Secondary Objectives
- To estimate HRQoL scores using specific and generic questionnaires and determine
factors associate with the HRQoL.
- To construct an equation uses to convert specific HRQoL (by PAQLQ with EQ-5D)
to generic HRQoL (EQ-5D).
- To describe cost of treatment in asthmatic cohort.

METHODS
Study design and setting
This study is a collaborative-prospective cohort study of 13 tertiary care hospitals, i.e.,
Siriraj Hospital, Mahidol University; Ramathibodi Hospital, Mahidol University; King
Chulalongkorn’s Memorial Hospital, Chulalongkorn University; Maharaj Nakorn Chiang
Mai Hospital, Chiang Mai University; Srinagarind Hospital, Khon Kaen University;
Children’s Hospital, Queen Sirikit National Institute of Child Health; Phramongkutklao
Hospital; Vajira Hospital, Navamindradhiraj University; Naresuan University Hospital;
Thammasat University Hospital; Bhumibol Adulyadej Hospital; Songklanagarind
Hospital, Prince of Songkla University; and HRH Princess Maha Chakri Sirindhorn
Medical Center, Srinakharinwirot University. Patients will be followed-up every 3 months
for 5 years in order to assess their disease progressions.
Study Population
All asthmatic children who are consecutively treated at Allergy clinic in these studied
hospitals between 1st June 2009 and 31st October 2009 are invited to participate with this
study. Patients will be eligible if they meet following criteria.
Inclusion criteria
Subjects would be enrolled if they met following criteria
- Age < 18 years
- Have been diagnosed asthma with following criteria
For age< 5 years
- Having at least one of following clinical symptoms
- Frequent episodes wheeze > 1 time/month
- Activity-induced cough or wheeze

- Nocturnal cough without viral infection
- No seasonal variation in wheeze
- these symptoms that persist after age 3 years
- Having predictive clinical index that consist of following two conditions
• Three or more episodes of wheezing in the past year
• Diagnosed atopic dermatitis or Parental history of asthma or
- Having at least two of the following conditions
• diagnosed allergic rhinitis
• Wheezing apart from colds
• Peripheral blood eosinophilia (≥ 4%)
• Skin test positive to any allergens
- Symptom is improved with short acting bronchodilator and inhaled
glucocorticosteroids, and deterioration when treatment is stopped.
For patients ages 5 -18 years, have at least 1 of the following
- Nocturnal multiple episodes of cough, chest discomfort or wheezing are
resolvable with or without bronchodilator.
- These symptoms usually be stimulated by respiratory infection, exercise,
smoking, irritant, seasonal change or allergens.
- These symptom usually be associated with atopic dermatitis, allergic rhinitis
- Have family history of atopy especially asthma
- Have clinical signs of respiratory system such as wheeze, cyanosis, tachycardia,
chest deformity
- Respiratory function test abnormality
- Abnormality one spirometry test
• Increase FEV1 more than 12% or 200 ml with bronchodilator
• FEV1/FVC less than 0.75

- Abnormality of peak expiratory flow rate or variability
• Peak expiratory flow(PEF) increase more than 20% with bronchodilator
• PEF variability >20%; PEF variability =
• Participants and parents are willing to participate and sign a consent form
Patients would be excluded if they had other chronic respiratory diseases (e.g.
cystic fibrosis, COPD, cor pulmonale, etc.), or other co-morbidity (e.g., cancer,
hematologic disease, congenital immunodeficiency or cerebral palsy, etc), or
currently participated in other studies that determine effect of medications
especially asthmatic medication or intervention such as immunotherapy
Data Collection
Data record forms (DRF) consist of 6 categories, which were screening form, enrollment
form, follow-up form (M03), general HRQoL, specific HRQoL, and care-giver
information. Details of each form are as follows:
- The screening form included symptomatic data, diagnostic criteria, and
eligibility and exclusion criteria.
- The enrollment form contained data at enrollment, which are demographic
data, history of allergic diseases, family history of illness, severity and level of disease
control, environmental factor, and treatments.
- The follow-up forms consisted of a level of disease control, environmental
factors, and uses of treatments

- Health related quality of life questionnaire consisted of 5 items (EQ-5D) and
visual analog scale.
- The PAQLQ was a specific health related quality of life questionnaire, which
consisted of 21items with level scale of 1-7 in each items. Both EQ-5D and PAQLQ were
used for only patients aged older than 7 year.
- Care-giver information considered familial and parental socioeconomic
status, insurance, and quality of life (EQ-5D) of patient in parental perspective.
All forms were used at enrolment and follow-up period, except the follow-up form was not
used at the enrollment, and the screening and enrollment forms were not used at the
follow-up.
Training
A meeting for orientation of data collection was organized for all collaborative
investigators and nurses who involved in the study. A standard operating procedure (SOP)
of data collection was developed and this had been distributed to collaborative sites.
Collaborative investigator and nurse collected data following the SOP and any problem
occurred due to unclear SOP or data collection forms would lead to a meeting to revise
them properly. Regular meeting of investigators was organized once a year.
Variables and measurements
Primary outcome
The primary outcome is time to disease remission, which was defined according to the
GINA guideline (1) as asthmatic controlled executively for 1 year with or without use of
medications with following conditions: having daytime symptom or need reliever ≤ 2

times/week, no limitation of daily activity, no nocturnal symptom, no exacerbation, and
normal or near normal lung function
Secondary outcome
The secondary outcomes are severe asthma-related events, asthma control, and HRQoL.
The severe asthma-related events are number of hospitalizations and visiting emergency
medicine unit. These could be identified by interview and review a medical record in each
visit. Common criteria for hospitalization consists of one of the following conditions:
require more than 2 doses of beta-agonist, not improve with systemic corticosteroid, need
oxygen supplementation (O2 saturation less than 90%), a history of rapid deteriorated
exacerbation, poor adherence with medical regimen, and psychosocial concern. Patients
will be recorded as having visited ER if patient seek for unscheduled medical treatment
and be prescribed for systemic corticosteroid.
Asthma control was assessed at baseline using following criteria:
- No (twice or less/week) day time symptoms
- No limitations of daily activities, including exercise
- No nocturnal symptoms or awakening because of asthma
- No (twice or less/week) need for reliever treatment
- Normal or near-normal pulmonary function
- No exacerbations
EQ-5D consisted of 5 items for motility, self-care, usual activity, pain/discomfort, and
anxiety/depression. Each item is graded from 1 to 3, 1 was the best and 3 was the worst
score. The proxy EQ-5D, measured patient quality of life in a perception of a care-giver
or parents. The utility score can be calculated by regression equation defined in (32).
PAQLQ consists of 3 domains with 23 items, i.e. 5 items for activity limitation domain, 10
items for symptoms domain, and 8 items for emotional function domain. Each item

contains 1 to 7 rating scale, 1 means the worst and 7 refers to the best. Domain and total
scores are calculated by summing up item scores for that domain and all items,
respectively.
Independent variables
Independent variables include clinical, environmental, behavioral, and treatment factors
that may be associated with the clinical outcomes. Some of these factors are fixed while
some factors are varied or changed over time. Fixed variables include gender, age at
asthma onset & diagnosis, allergy (allergic rhinitis, atopic dermatitis and food allergy),
skin tests. Time-varying include treatments for asthma, weight, height, environmental in
household (e.g., number of smokers, numbers of pets). In addition, family history of
asthma, atopic, and allergy were also recorded.
Data management
All data record forms were printed and sent off from the central site, i.e., data management
unit (DMU), Ramathibodi Hospital. All data were delivered to the central DMU for
performing data computerization. EpiData software version 3.1 was used for constructing
databases. Data checking files were created for quality control in the data entry process.
There were 3 databases, i.e., logbook, enrollment, and follow-up databases. The
enrollment and follow-up databases were also split into medical, quality of life, and
economic databases. Confidential data (e.g., name, birth date, and address) were kept in
the logbook database and were linked with enrollment and follow-up databases with
assigned identity numbers. All data were securely encrypted. Two staffs independently
performed data entry and the data were then validated. Clean and check data were
regularly performed every 3 months. Unclear or missing data was inquired from the
central DMU.

Statistical Analysis
i) Analysis for baseline data
The outcomes of analyses were asthma control and HRQoL. For asthma control, chi-
square and t-tests were applied to assess association between factors and asthma control
for categorical and continuous data, respectively. Multiple logistic regression analysis was
applied to assess factors associated with asthma control adjusting confounding factors.
Variables which P-value in univariate analysis less than 0.20 would be considered
simultaneously including in the multiple logistic model. Odds ratio along with 95%
confidence interval (CI) were estimated and reported.
For HRQoL, the EQ-5D and PAQLQ total scores were described using mean (SD) or
median (range) where appropriate. Simple linear regression was applied to assess factors
correlate with the HRQoL. Variables which P-values less than 0.20 were simultaneously
considered including in the multiple linear regression model.
ii) Analysis for follow-up data
Disease remission was the primary outcome of interest for the follow-up data, which was
clearly described previously. A rate of disease remission along with 95% CI was estimated
and reported. Kaplan-Meier method was used to estimate a median time and rate of
disease remission. For univariate analysis, a simple Cox regression was applied to assess
prognostic factors of disease remission. A multivariate Cox regression was then applied by
simultaneously including prognostic factors with P-value from the univariate analysis of
less than 0.15. A hazard ratio (HR) by exponential of regression coefficients along with
95% CI were estimated and reported.

All analyses were performed using STATA version 12.0. P value less than 0.05 was
considered as statistically significant.
RESULTS
a) Baseline cohort General characteristics
Four-hundred eighty-five asthmatic children have been enrolled in our cohort. Their
characteristics at enrollment were described in Table 1. Their mean age was 11.2 (SD =
2.6), 62.7% were males, mean BMI was 19.0 (SD = 4.9). These children had main care
givers as mothers (79.4%) and a median income of their family income was 20,000 Baht
(range = 2,000-428,035). Environmental household was recorded, 150 (30.9%) reported
having at least one person smoked at home and 184 (37.9%) had pets at home.
History of illness
Mean age at onset of asthma was 24 months (range = 1-177) and the mean age at
diagnosis was 60 (range = 2-179) months. As for history of other allergic diseases, 394
(81.2%) had allergic rhinitis, 92 (19.0%) had atopic dermatitis, and 65 (13.4%) had food
allergies.
As for family history of illness, 110 (22.7%) and 51 (10.5%) children had parents and
siblings diagnosed of asthma. Allergic rhinitis was reported approximately 42.1% in
parents and 42.1% in siblings, respectively. Atopic dermatitis and food allergy were not
common in parents and sibling, approximately 5% for both in parents and 2% for both in
siblings.

Type of cases
Among 485 children, 439 (90.5%) were old cases and only 46 (9.5%) were newly
diagnosed cases. Level of asthmatic control was assessed for all 439 children and found
that 284 (64.7%) were completely controlled, 120 (27.3%) were partly-controlled, and 33
(7.5%) were uncontrolled. Asthma severity was also assessed for those new cases,
majority of cases were mild persistent (23, 50.0%) followed by moderate persistent (16,
34.8%). There was none severe persistent.
Pulmonary function test was performed in children who aged older than 7 years. Among
241 (49.7) children performed PEFR test, mean PEFR was 277.6 (SD = 84.5) liter/min.
One-hundred sixty-one children had performed spirometry tests 161 (33.2%) with the
median FEV1 and FEV1/FCV ratio of 1.8 (0.7-5.6) liters and 89.1% (0.7-99.7),
respectively.
Treatments
Nearly a half of children (224, 45.8%) were prescribed inhaled corticosteroids (ICS), in
which budesonide was the most common (93.3%) follow by fluticasone (3.6%), and
beclomethasone (3.1%). Most children received ICS with dosage of > 400 mcg/day
(48.2%), some received 200-400 mcg/day (33.5%), and the rest received < 200 mcg/day.
Combination of ICS and long-acting beta2-agonist was also prescribed in 178 (36.7%)
children. Among them, fluticasone/salmeterol was the most frequent prescription (78.6%)
and the rest was budesonide/formoterol. Most children (46.6%) received ICS+LABA with
ICS dosages of 200-400 mcg/day, follow by < 200 mcg/day (39.3%), and only few > 400
mcg/day (12.6%). Bronchodilators were also commonly prescribed in 223 (45.7%)
children, in which salbutamol was the most common (81%). Leukotriene modifier (LTRA)

and intranasal steroid were prescribed in 76 (15.6%) and 208 (42.5%) children,
respectively.
Skin test
Three-hundred and fifteen children had performed at least one skin test and their results
were described in Table 2. The most frequent positive test was dust mite-Der p (69.9%),
follow by dust mite-Der f (67.4%), German or American cockroach (45.9%-47.1%), and
Bermuda grass or cat dander (~22%).
Factors associated with asthmatic controls
Factors associated with level of asthmatic controls at baseline were explored in 439 old
cases, see Table 4. Among 439 children, 285 (64.5%) whose symptoms were completely
controlled, 122 (27.6%) were partly-controlled, and 33 (7.5%) were uncontrolled. These
two later groups were combined and compared with the completely-controlled group in
further analysis. The study factors were treatments, which were: use of ICS, ICS+LABA,
and LTRA. Results of univariate analysis suggested that only the use of ICS+LABA
significantly associated with completely-controlled with the odds ratio (OR) of 0.65 (95%
CI: 0.44, 0.97), whereas other two treatments were quite null associations. Other factors in
which the univariate analysis suggested a trend of association (p-value < 0.25) were age at
asthma onset, and having 1st or 2nd degree of siblings with asthma. Children with age at
asthma onset at 24 months or older had approximately 27% (OR = 1.27, 95% CI: 0.86,
1.87) higher chance of completely-controlled than children with age at asthma onset
younger than 24 months, although this was not statistically significant. Children whose 1st
or 2nd degree of siblings with asthma had 35% (OR = 0.65, 95% CI: 0.35, 1.20) lower
chance of having completely-controlled than children who 1st or 2nd degree of siblings

were not asthma. Multiple logistic regression was applied by including treatment, age at
asthma onset, and of the 1st degree of siblings with asthma in the model, see Table 5. After
adjusting for confounding variables and other treatments, children who received ICS plus
LABA were 0.42 (95% CI: 0.22, 0.78) times less likely to have completely-controlled than
children who did not receive this combined treatments. This could be interpreted that
children who were treated with ICS plus LABA had 58% lower chance of having
complete asthmatic control. Children had been treated with ICS alone was also 0.54 (95%
CI: 0.29, 1.01) less likely to have complete asthmatic control compared with children who
had not although this was borderline insignificant. Children had been treated with
leukotriene modifier (LTRA) were 14% (OR = 0.86, 95% CI: 0.49, 1.49) lower chance of
having complete asthmatic control than children who had not been treated with LTRA.
However, this did not reach to statistically significant. Children with age at asthma onsets
were 24 mother or older were 1.30 (95% CI: 0.87, 1.95) times more likely but not
significant to have complete asthma control than children with age at asthma onsets were
younger than 24 months. Children who had 1st degree of siblings with asthma were 0.64
(95% CI: 0.34, 1.19) times less likely but not significant to have complete asthmatic
control than children who did not have any sibling with asthma.
Quality of life
Specific and general quality of life (QoL) scores were assessed in children aged 7 years or
older. Results were described as follows.
PAQLQ
PAQLQ consisted of 3 domains (i.e., activity, symptom, and emotion) was used for
assessing specific QoL on 264 children. The average score for each domain ranged from 0
20
to 7, higher score reflects better QoL. As described in Table 6, the mean activity,
symptom, and emotion scores were respectively 5.6 (SD = 1.2), 5.6 (SD = 1.2), and 5.9
(SD =1.1) with the overall mean total scores of 5.7 (SD = 1.1).
Factors associated with overall mean PAQLQ score were assessed, see Table 7. These
factors included level of asthma control, use of medications, demographic data, and
environmental factors in households. Results of univariate analysis suggested that level of
asthmatic control was significantly associated with PAQLQ, i.e., children with asthmatic
control had 0.5 (95% CI: 0.26, 0.78) higher PAQLQ score than children with asthmatic
un-control. Using LTRA showed the trend of association whereas other factors did not.
Multiple regression analysis was applied by fitting asthmatic control and treatments into
the model, Table 9. This suggested that after adjusting for treatments, children with
asthmatic control had significantly better quality of life than children with uncontrolled
asthma with mean different score of 0.5 (95% CI: 0.2, 0.7). Using LTRA was also
significantly associated with quality of life, i.e., children who were prescribed with LTRA
had 0.4 (95% CI: -0.8, -0.03) PAQLQ score lower than children who were not. Other
treatments did not significantly associate with quality of life.
EQ-5D
The general quality of life was assessed based on children and proxy (i.e., parents or care
givers) using EQ-5D score. Mean EQ-5Ds were 0.87 (SD = 0.16) and 0.85 (0.15) for self-
report and proxy, respectively. These two means were statistically significant with the
difference of -0.02 (95% CI: -0.03, -0.01).
Although these were statistically significant different, the magnitude was very small and
thus self-report would be used for further analyses.

Factors associated with general quality of life were explored, see Table 9. This reviewed
that asthmatic control, smoking in household, and skin test were significantly associated
with EQ-5D utility score whereas treatments and other factors were not. Multiple
regression analysis was applied by fitting significant variables plus treatments and age
(p<0.25) simultaneously, see Table 10. After adjusting for other variables, children with
controlled asthma had 0.04 (95% CI: 0.00, 0.09) higher utility score than children with
uncontrolled asthma. This was however borderline statistically significant (p = 0.047).
Having smokers in household was also significantly associated with the utility score with
the coefficient of 0.05 (95% CI: .01, 0.09).
b) Follow-up results
Disease remission
Four-hundred and fifty-four patients were enrolled and followed up with a median time of
1.5 (range = 0.13, 2.4) year. Among 577.5 person-year, the incidence of disease remission
was 76.5 (95%CI: 69.7, 84.0)/100/year. The estimated median of disease remission was
1.38 (95%CI: 1.32, 1.42), i.e., 50% of patients would reach to disease remission at 1.38
years or longer.
The univariate analysis was performed to assess prognostic factors of disease remission.
As describe in Table 11, this suggested that 10 prognostic factors had a trend of
associations with the disease remission (P < 0.15), which were age, BMI, main caregiver,
parental education, allergic rhinitis, atopic dermatitis, parental asthma, sibling asthma,
sibling atopic, and influenza immunization. These prognostic factors were then
simultaneously included in the multivariate Cox regression. After adjusting for
confounding effects, BMI, parental education, and sibling’s history of atopic were
significantly associated with disease remission, see Table 12. Children with BMI > 18

kg/m2 were approximately 1.4 times significantly higher chance to reach to disease
remission than children with BMI < 18 kg/m2. Children who had sibling without history of
atopic were 80% significantly higher chance of disease remission than children who had
sibling with history of atopic. In addition, children with parent’s education of Bachelor’s
degree or higher were 34% significantly higher chance of disease remission than children
whose parent’s education were lower than Bachelor’s degree.
Quality of life
Thai utility scores and PAQLQ scores were measured at every 3 months during the follow
up period, see Table 14. After baseline enrollment, mean Thai utility scores changed from
0.87 (SD = 0.16) to 0.91 (SD = 0.16). Applying a mixed regression analysis suggested that
Thai utility scores significantly increased from baseline (P = 0.006). This trend was also
observed in PAQLQ scores, in which mean PAQLQ significantly increased from 5.91
(1.03) to 6.41 (SD = 0.95, p <0.001). Factors associated with Thai utility scores were
assessed and suggested that only asthmatic control were significantly associated with Thai
utility scores, see Table 16. After adjusting for confounders, children with completely and
partially controlled asthma had 0.095 (95%CI: 0.059, 0.130) and 0.080 (95%CI: 0.043,
0.117) significantly higher Thai utility scores than children with uncontrolled asthma;
whereas other covariables, i.e., BMI, skin test, and use of leukotriene modifier had a trend
of association but were not significant, see Table 16. The trend of association was also
observed in the PAQLQ, see Table 17. After adjusting for confounding effects, children
with well and partial controlled asthma had 0.88 and 0.56 significantly higher PAQLQ
scores than children with uncontrolled asthma, see Table 18. In addition, BMI was also
significantly associated with PAQLQ, in which children with BMI > 18 kg/m2 had 0.16
significantly higher score than children with BMI < 18 kg/m2.

Cost of treatments
Data for direct medical costs were retrieved and reviewed for 59 asthmatic children at
Ramathibodi Hospital. These children had 277 visits, (272 as scheduled OPD, 4
unexpected visits with exacerbation at OPD, and 1 at emergency unit). An overall median
direct medical cost was 6,571 Baht (range= 304, 45,248.50). The median cost for children
with exacerbation was 8473 (range = 2,345, 18,512) Baht which was higher but not
significant when compared with children without exacerbation (median = 6,417; range =
3,648, 14,191 Baht), see Figure 1. The costs of these 2 groups were very similar if
considering only OPD visits, see Figure 2. In addition, cost by specific medications i.e.
corticosteroids (ICS), leukotriene modifier (LTRA), short-acting beta2 agonist (SABA)
and antibiotics (ATB) were also compared, see Figure 3-7. We found that cost from LTRA
was the highest, followed by ATB and ICS, Figure 7. Although ATB was not a specific
treatment but its cost was as high as ICS’s cost.
DISCUSSION
We had conducted a Thai asthmatic cohort study with 2 years follow up. Our results
suggested that disease remission was as high as 76.5 per 100 persons per year. Fifty
percent of these children could reach to disease remission at approximately 1.4 years.
Three prognostic factors (BMI, parental education, and no sibling with history of atopy)
were associated with the disease remission. Children with higher BMI, higher educated
parents, and sibling without history of atopic diseases had approximately 30% to 80%
higher chance to reach to disease remission. Children with well or partly-controlled
asthma were better in general and specific quality of life scores than children with
uncontrolled asthma.

Clinical asthma controlled was positively associated with quality of life, better asthma
controlled resulted in better quality of life. This relationship was consistent in both general
and specific quality of quality of life. For general quality of life, children with partly- to
well-controlled asthma were approximately 1 unit higher quality of life score than patients
with uncontrolled asthma. The PAQLQ and utility scores were 0.88 and 0.56 higher in
children with well and partly-controlled compared with children with uncontrolled asthma,
respectively. Juniper et al (33, 34) suggested that the overall and domain (activities,
emotions, symptoms) asthma-specific quality of life scores were classified as a minimal,
moderate, and large important differences if the differences were 0.5, 1.0, and 1.5,
respectively. Our general and specific quality of life scores after receiving proper
treatments could therefore reach to moderate improvement. However, a long term
improvement should be followed up.
In summary, our cohort study suggested that children with well controlled could reach to
disease remission at approximately 1.4 years. Well controlled asthma could also
moderately improve the quality of life. However, long term outcome should be further
studied and assessed.
