Reversible cerebral vasoconstriction syndrome withischaemic stroke: case report
A 32-year-old woman developed reversible cerebralvasoconstriction syndrome (RCVS) with ischaemic stroke,while receiving bromocriptine.
The woman had started receiving a 7-day course of oralbromocriptine 2.5mg twice a day to suppress lactation; shehad given birth about 1 year earlier. After 7 days ofbromocriptine, she was admitted to a hospital with transientleft-sided weakness, dysarthria and a severe headache.Transient ischaemic attack with right middle cerebral arterystenosis was suspected following MRI and MRA investigation.
The woman started receiving low-dose aspirin. She wasadmitted to a different hospital 9 days after the onset of hersymptoms, with severe, recurrent headache which worsenedon her second day in hospital. She developed left-sidedweakness and dysarthria. RCVS was suspected and shereceived nimodipine for 14 days; her headache, dysarthria andhemiparesis gradually improved. At follow-up 6 months later,her symptoms had completely resolved.
