RESUMO
A endotelina, um potente vasoconstrictordescrito em 1988 por Yanagisawa, contribuiu
para normal regulação da funçãocardiovascular. No entanto, quando emdesequilíbrio, concorre para o processo
patológico de várias doenças cardiovascularescomo sejam o enfarte do miocárdio, areestenose, a hipertensão pulmonar, a
hipertensão arterial e até mesmo a doença deChagas. Têm sido desenvolvidos vários
esforços na tentativa de bloquear esta via. Osresultados obtidos com inibidores dos
receptores foram desanimadores, no entanto,tem-se verificado um interesse crescente no
bloqueio da sua formação. Estão descritosinibidores selectivos e não selectivos da
enzima de conversão da endotelina (ECE),estes últimos com a possibilidade da inibição
associada da enzima de conversão daangiotensina e da endopeptidase neutral
permitindo o bloqueio simultâneo de váriasvias envolvidas na fisiopatologia destas
doenças.Nesta revisão são apresentados os
diferentes tipos de inibidores da ECE comreferência às características farmacológicas,dando particular ênfase à potencialidade da
sua aplicação clínica nas diversas patologiascardiovasculares.

ABSTRACT
Endothelin-converting Enzyme Inhibitors:their Application in CardiovascularDiseases
Endothelin, a potent vasoconstrictor firstdescribed in 1988 by Yanagisawa, is animportant regulator of cardiovascular function.Hyperactivation of the endothelin system hasbeen implicated in the pathogenesis of variouscardiovascular disorders including myocardialinfarction, restenosis, hypertension, heartfailure and Chagas cardiopathy. Variousattempts have been made to suppress this axis.Although promising, the results of clinicaltrials on endothelin receptor antagonists havebeen disappointing.There is growing interest in blockade ofendothelin formation. Several selective andnon-selective endothelin-converting enzyme(ECE) inhibitors have been developed, thelatter with the possibility of simultaneouslyblocking angiotensin-converting enzyme andneutral endopeptidase, combining inhibition ofmore than one axis.This article reviews the different ECEinhibitors, with particular emphasis on theirpotential clinical application in cardiovasculardiseases.

INTRODUÇÃO
Em 1988, Masashi Yanagisawa descreveu umpeptídeo com potente acção vasoconstritora
que denominou de endotelina (1). Desde entãohouve grande desenvolvimento no conhecimentodesta substância revelando-se a sua actividadesobre diversos órgãos e sistemas, sendo hojesobejamente conhecidas muitas das suas acçõesna função cardíaca.
A endotelina é um importante regulador dafunção cardiovascular, contribuindo para aregulação da homeostasia como um factorparácrino (para revisão ver (2)). Paralelamente ao queacontece com outros mediadores neurohumorais,quando em desequilíbrio (hiperactivação dosistema da endotelina), pode concorrer para oprocesso fisiopatológico de várias doençascardiovasculares como sejam a hipertensãopulmonar, a hipertensão arterial, a insuficiênciacardíaca, o enfarte do miocárdio, a reestenosee até mesmo a doença de Chagas (3) (para revisão ver (4; 5)).
Estão caracterizados e descritos dois tipos dereceptores para a endotelina, o receptor ETA eo receptor ETB. Estes receptores fazem parteda família dos receptores acoplados às proteínasG, estando distribuídos por uma variedadede células e tecidos em diferentes proporções (6).Leite-Moreira e Brás-Silva sugeriram ainda aexistência de dois subtipos de receptores ETBno coração: ETB1 (localizado no endotélioendocárdico) e ETB2, (localizado noscardiomiócitos) (7), à semelhança do que já haviasido descrito para a vasculatura (8).
Da família da endotelina fazem parte quatroisoformas codificadas por diferentes genes: ET-1,a isoforma predominante, ET-2, ET-3 (9) e ET-4 (10).A partir do produto inicial do gene de cadaendotelina, uma cascata de eventos permiteformar o peptídeo na sua forma activa.
A enzima de conversão da endotelina (ECE) éuma protease transmembranar sensível aoFosforamidon (11) que cataliza a fase final doprocessamento pós-translacional da endotelina (12)
clivando a big ET(1-38) na ligação Trp21-Val/Ile22
para formar a ET(1-21) (Figura 1). A ECE tambémpode hidrolizar outros peptídeos biologicamenteactivos como a bradicinina, a substância P, aangiotensina I e a insulina, embora com menoreficácia (Para revisão ver (13)). Esta enzima integra a famíliaM13 das metaloendopeptidases de zincojuntamente com a endopeptidase neutral 24.11
INTRODUCTION
In 1988, Masashi Yanagisawa described apotent vasoconstrictor peptide, and called it
endothelin (1). Since then there have beenconsiderable advances in knowledge of thissubstance and its action in various organs andsystems, particularly its effect on cardiacfunction.
Endothelin is an important regulator ofcardiovascular function, contributing tohomeostasis as a paracrine mediator (for review see (2)).As with other neurohumoral modulators,hyperactivation of the endothelin system maybe implicated in the pathogenesis ofseveral cardiovascular disorders such aspulmonary and arterial hypertension, heartfailure (HF), myocardial infarction, restenosisand Chagas cardiopathy (3) (for review see (4, 5)).
Two types of endothelin receptor have beencharacterized, ETA and ETB. These belong to thefamily of G-protein-coupled receptors and arefound in a range of cells and tissues in varyingproportions (6). Leite-Moreira and Brás-Silva havefurther suggested that there are two subtypes ofETB receptors in the heart: ETB1 (in endocardialendothelium) and ETB2 (in cardiomyocytes) (7), ashad already been described for the vasculature (8).
There are four isopeptides in the endothelinfamily encoded by different genes: ET-1, thepredominant isoform, ET-2, ET-3 (9) and ET-4 (10).From the initial gene product of each endothelin,a cascade of events leads to formation of theactive peptide.
Endothelin-converting enzyme (ECE) is amembrane-bound phosphoramidon-sensitiveprotease (11) that catalyzes the final stage of post-translational modification of endothelin (12),cleaving big ET(1-38) at the Trp21-Val/Ile22 bond toform ET(1-21) (Figure 1). ECE can also hydrolyzeother biologically active peptides such asbradykinin, substance P, angiotensin I andinsulin, although less efficaciously (for review see (13)).This enzyme belongs to the M13 family of zincmetalloendopeptidases, together with neutralendopeptidase 24.11 (NEP), Kell blood groupprotein, the PHEX gene product (for review see (14)),soluble secreted endopeptidase (SSE) (15) andECE-like 1 (ECEL-1) (16-18). The members of thisfamily characteristically contain a highlyconserved pentameric consensus sequence of aC-terminal zinc-binding motif (HEXXH) (15).386

(EPN), a proteína do grupo sanguíneo Kell, oproduto do gene PHEX (para revisão ver (14), aendopeptidase solúvel secretada (ESS) (15) e aenzima tipo ECE (ECEL-1) (16-18).Caracteristicamente, os membros desta famíliacontêm no domínio C-terminal uma sequênciade consenso pentamérica de ligação ao zinco(HEXXH) fortemente conservada (15).
Estão descritas diferentes formas de ECE:ECE-1 (19), ECE-2 (20), e ECE-3 (21) com diferentesespecificidades para as isoformas de big ET.
Além disso, foram identificadas quatroisoformas para a ECE-1: ECE1a/ß, ECE-1b (22),ECE-1c/α (23) e ECE-1d (24), derivadas de um únicogene por splicing alternativo (22; 24), localizado nabanda 1p23 do genoma humano (25). Estas enzimasdiferem no domínio N-terminal e apresentamdiferentes localizações subcelulares (22). Àsemelhança do que aconteceu para a ECE-1identificaram-se as seguintes isoformas deECE-2: ECE-2a-1, ECE-2a-b, ECE-2b-1 eECE-2b-2 (26), também com diferenteslocalizações subcelulares e gene localizado nocromossoma 3 do genoma humano: 3q28-q29 (27).
A ECE-3 é uma enzima com especificidadepara a big ET-3 (21).

Three forms of ECE have been described:ECE-1 (19), ECE-2 (20), and ECE-3 (21), with differentspecificities for the isoforms of big ET.
Four isoforms of ECE-1 have also beenidentified: ECE-1a/ß, ECE-1b(22), ECE-1c/α (23) andECE-1d (24), derived by alternative splicing from asingle gene (22, 24), located at 1p23 of chromosome 1of the human genome (25). These enzymes differ inthe N-terminal portion and have differentsubcellular localizations (22). As with ECE-1, fourdifferent isoforms of ECE-2 have been identi-fied: ECE-2a-1, ECE-2a-b, ECE-2b-1 andECE-2b-2 (26), also with different subcellularlocalizations, the gene being located onchromosome 3 at 3q28-q29 (27).
ECE-3 is specific for big ET-3 (21).Studies on disruption of the ECE-1 gene (28),
together with the fact that ECE-1 is found in awider range of tissues and expressed in greaterquantities than ECE-2, indicate that ECE-1 is themain enzyme responsible for the transformationof big ET into ET (27).
As was the case with the angiotensin pathway,ECE inhibitors have been developed extendingthe possibilities of therapeutic intervention in theendothelin pathway beyond receptor antagonism.

assim como a distribuição por um maior númerode tecidos e expressão em níveis mais elevadosdesta enzima relativamente à ECE-2, indicamque a ECE-1 é a principal enzima responsávelpela transformação da big ET em ET (27).
À semelhança do que aconteceu com a via daangiotensina, foram desenvolvidos inibidores daECE, o que permite alargar as potencialidadesterapêuticas da intervenção na via da endotelinajá iniciada com o antagonismo dos receptores.
Nesta revisão serão abordados diferentesinibidores da enzima de conversão da endotelinadando particular ênfase à sua aplicação clínicanas diversas patologias cardiovasculares comreferência a estudos animais e humanos quandodescritos.
TIPOS DE INIBIDORES DA ECE
Há três tipos de inibidores conhecidos:inibidores selectivos ECE-1, inibidores ECE-1/EPN e inibidores ECE-1/EPN/ECA (Quadro I).O termo “selectivo ECE-1” refere-se apenas àselectividade entre diferentes tipos de enzimascomo a EPN, e não entre diferentes formas ouisoformas de ECE, uma vez que, além doFosforamidom e do FR901533 (20), ainda não foidescrita actividade destes compostos para aECE-2 ou para a ECE-3.
A ECE-1 humana partilha 37% da sequênciade aminoácidos com a EPN (29) enzimaresponsável pela degradação proteolítica dopeptídeo natriurético auricular (30), e dabradicinina (31), entre outros (para revisão ver (14)). Assim,não é surpreendente que a maioria dos inibidoresda ECE possua também actividade inibitória paraa EPN.
A ECE, a EPN e a ECA são metaloproteasesde zinco, pelo que podem ser inibidassimultaneamente por moléculas que possuamum grupo quelante de metal apropriado, (32) oque permite que um único composto tenhacapacidade inibitória tripla.
Inibidores selectivos ECE-1Estes inibidores podem ser de origem natural
ou sintética. São exemplos dos primeiros, oWS79089A, B e C, isolados da fermentação deStreptosporangium roseum No.79089, oFR901533, sal sódico de WS79089B (33; 34), oWS75624A e B isolados da fermentação de

TYPES OF ECE INHIBITORS
There are three known types of inhibitors:selective ECE-1, ECE-1/NEP and ECE-1/NEP/ACE inhibitors (Table I). The term“selective ECE-1” serves merely to differentiateit from other enzymes such as NEP, and notbetween different ECE forms or isoforms, since,besides phosphoramidon and FR901533 (20), it isnot yet known whether these compounds areactive against ECE-2 or ECE-3.
Human ECE-1 shares 37% of its amino acidsequence with NEP (29), the enzyme responsiblefor the proteolysis of atrial natriuretic peptide(ANP) (30) and bradykinin (31), among others (for review
see (14)). It is thus not surprising that most ECEinhibitors also inhibit NEP.
ECE, NEP and ACE are zincmetalloproteases, and can therefore be inhibitedsimultaneously by molecules that possess anappropriate metal-chelating group (32), enabling asingle compound to provide triple inhibition.
Selective ECE-1 inhibitorsThese inhibitors can be natural or synthetic.
Examples of the former are WS79089A, B and C,isolated from fermentation of Streptosporangiumroseum No. 79089; FR901533, the sodium salt ofWS79089B (33 34); WS75624A and B, isolated fromfermentation of Saccharothrix sp. No. 75624 (34);and B90063, isolated from fermentationof Blastobacter sp. SANK 71894 (35). Thesecompounds have IC50 values for ECE of0.73 µM, 0.14 µM and 3.42 µM (WS79089A,B and C), 0.03 g/ml (WS75624A and B), and 1µM (B90063), and for NEP of >100 µM(WS79089A B and C), 1 µg/ml (WS75624A andB), and 66 µM (B90063) (33-35).
Synthetic inhibitors include RO 67-7447(IC50 1.2 nM for ECE, >100 µM for NEP, and >10µM for ACE) (36), CGS 35066 (IC50 22 nM forECE, 2.3 µM for NEP) (37), and PD 069185 andSM-19712, with different IC50 values in differentexperimental models; the latter has practicallyno inhibiting effect on ACE and NEP (38 39). RO68-7629 and CGS 35339 are prodrugs of RO67-7447 (36) and CGS 35066 (37) respectively.388

Saccharothrix sp No 75624 (34) e o B90063 isoladoda fermentação de Blastobacter sp SANK71894 (35). Estes compostos apresentam IC50para a ECE de 0,73µM, 0,14µM, 3,42µM(WS79089A, B e C), 0,03µg/ml (WS75624A e B)e 1µM (B90063) e IC50 para a EPN de >100µM(WS79089A B e C), 1µg/ml (WS75624A e B) e66µM (B90063) (33-35).
São inibidores de origem sintética, o RO 67-7447 (IC50 1,2 nM-ECE, >100µM-EPN e>10µM-ECA) (36), o CGS 35066 (IC50 22nM-ECE/IC50 2,3µM-EPN) (37), o PD 069185 e o SM-19712,com IC50 diferentes em modelos experimentaisdiferentes, este último praticamente sem efeitoinibitório ECA e EPN (38; 39). O RO 68-7629 e oCGS 35339 são pró-fármacos de RO 67-7447 (36)
e CGS 35066 (37), respectivamente.
Inibidores ECE-1/EPNO Fosforamidon, um dos primeiros inibidores
descritos (11), é um composto com IC50 de 3,5µMpara a ECE, 0,034 µM para a EPN e 78 µMpara a ECA (40), com pouca eficácia por via oral ecurta duração de acção. De forma a contornarestas características farmacodinâmicas foramdesenvolvidos a partir dele, o CGS 26303(IC50 1,1µM-ECE/ IC50 0,9nM-EPN) (41), oCGS 34043 (IC50 5,8 nM-ECE/ IC50110nM-EPN) (42), o CGS 34226 (IC50 11nM-ECE/IC50 4,6nM-EPN) (43) e o SLV-306 (potênciasemelhante ao Fosforamidon) (44).
A partir dos compostos CGS 26303, CGS34043 e CGS 34226, desenvolveram-se pró--fármacos com maior biodisponibilidade oral:CGS 26393 (45), CGS 34753 (42) e CGS 34225 (46),respectivamente. O SLV-306 (daglutril) édisponível por via oral sendo metabolizado emKC-12615, a molécula activa (47; 44).
Impulsionado pelos resultados promissoresobtidos em estudos pré-clínicos, nomeadamenteefeito diurético, anti-hipertensor e anti-hi-pertrófico do músculo cardíaco (47; 48), este últimofármaco tornou-se o primeiro inibidor da ECE aintegrar ensaios clínicos (49), estando a decorrerneste momento a fase II de um ensaio em doentescom insuficiência cardíaca e em doentes comhipertensão arterial (44).
Inibidores ECE-1/EPN/ECASão inibidores triplos ECE/EPN/ECA, o Sch
54470, um ácido fosfínico (com IC50 de 80nMpara a ECE, 90nM para a EPN e 2,5nM para a
ECE-1/NEP inhibitorsPhosphoramidon, one of the first inhibitors
described (11), has an IC50 of 3.5 µM for ECE,0.034 µM for NEP and 78 µM for ACE (40), withlittle efficacy when taken orally, as well as beingshort acting. In order to overcome thesepharmacodynamic characteristics, the followinghave been developed from it: CGS 26303 (IC501.1 µM for ECE and 0.9 nM for NEP) (41), CGS34043 (IC50 5.8 nM for ECE and 110 nM forNEP) (42), CGS 34226 (IC50 11 nM for ECE and4.6 nM for NEP) (43), and SLV-306 (with similarpotency to phosphoramidon) (44).
Based on the compounds CGS 26303, CGS34043 and CGS 34226, prodrugs have beendeveloped with greater oral bioavailability: CGS26393 (45), CGS 34753 (42) and CGS 34225 (46)
respectively. SLV-306 (daglutril) is orally active,being metabolized to KC-12615, the activemolecule (47, 44).
Following the promising results obtained inpreclinical studies, particularly its diuretic,antihypertensive and antihypertrophic effects oncardiac myocytes (47, 48), SLV 306 became the firstECE inhibitor to be included in clinical trials (49),and a phase II trial is currently in progress inpatients with heart failure and in others witharterial hypertension (44).
ECE-1/NEP/ACE inhibitorsTriple inhibitors of ECE/NEP/ACE include
SCH 54470, a phosphinic acid (with an IC50 of80 nM for ECE, 90 nM for NEP, and 2.5 nM forACE) (50), SA-6817 (IC50 910 nM for ECE and Ki4.4 nM for NEP and 15 nM for ACE) (51), CGS26582 (IC50 620 nM for ECE, 4 nM for NEP, and175 nM for ACE) (52), CGS 35601 (IC50 55 nM forECE, 2 nM for NEP, and 22 nM for ACE) andCGS 37808, its orally active prodrug (32).
Theoretically, non-selective inhibitors would beexpected to produce greater benefits by actingsimultaneously on various pathways involved in thepathophysiology of cardiovascular diseases. Theywould thus inhibit the harmful effects mediatedby endothelin and angiotensin by blocking theirformation, while strengthening the knownbeneficial effects of atrial natriuretic peptide andbradykinin by preventing their degradation.This question will be discussed below, makingreference to relevant published studies.

Conceptualmente, será de esperar que osinibidores não selectivos tenham efeitosbenéficos superiores pela acção simultânea emvárias vias envolvidas na fisiopatologia dasdoenças cardiovasculares. Assim, inibiriam asacções deletérias mediadas pela endotelina eangiotensina bloqueando a sua formação epotenciariam os efeitos favoráveis já conhecidosdo peptídeo natriurético auricular e dabradicinina evitando a sua degradação. Estaquestão será abordada ao longo do texto, comreferência aos estudos desenvolvidos nessesentido.
ESTUDOS EXPERIMENTAIS COMINIBIDORES DA ENZIMA DECONVERSÃO DA ENDOTELINA
Enfarte do miocárdioApesar de ainda haver poucos estudos, os
inibidores da ECE revelaram uma acçãobenéfica no enfarte do miocárdio, comresultados consistentes em várias espéciesanimais, diminuindo a área de enfarte emelhorando parâmetros de remodelagemventricular.
A administração de SM-19712 diminuiu aárea de enfarte do miocárdio em coelhos sujeitosa oclusão (30 min) e reperfusão (5h) coronária,(25,9mg/Kg i.v. 10 min antes da oclusão daartéria marginal seguido por infusão contínua de1,7mg/Kg/min i.v. até ao fim da experiência) (39).Verificou-se um efeito semelhante para oFosforamidon, em cães submetidos a isquemia(2h) e reperfusão (3h) do miocárdio,(0,08mg/Kg/min i.v., com início 5 minutos antes econtinuação durante toda a experiência),associando-se à diminuição da área de enfarte(40%), uma diminuição de 88% do número deextrassístoles ventriculares (53).
Martin e colaboradores verificaram não haverdiminuição da espessura telediastólica da paredeanterior, nem diminuição da espessura relativa da
EXPERIMENTAL STUDIES ONENDOTHELIN-CONVERTING ENZYMEINHIBITORS
Myocardial infarctionAlthough there have been few studies to date,
ECE inhibitors have shown beneficial effects inmyocardial infarction by reducing infarct size andimproving ventricular remodeling parameters,with consistent results in various animal species.
Administration of SM-19712 reduced infarctsize in rabbits subjected to coronary occlusion (30min) and reperfusion (5 h) (25.9 mg/kg iv 10 minbefore occlusion of the marginal artery, followedby continuous infusion of 1.7 mg/kg/min iv untilthe end of the experiment) (39). A similar effect wasobserved with phosphoramidon in dogs subjectedto myocardial ischemia (2 h) and reperfusion (3 h)(0.08 mg/kg/min iv, beginning 5 min before andmaintained throughout the experiment), with a40% reduction in infarct size and an 88%decrease in ventricular ectopic beats (53).
Martin et al. found no reduction in anteriorwall end-diastolic thickness, nor in relativeventricular wall thickness, after administrationof FR901533 (100 mg/kg/day beginning24 h following infarction and maintainedcontinuously for seven days) in Sprague-Dawley rats, in contrast to what occurred in thecontrol group. There was a tendency forimprovement in hemodynamic parameters(mean blood pressure, systolic pressure andleft ventricular end-diastolic pressure), althoughthis was not statistically significant (54).
At the molecular level, ECE inhibitorsdecreased levels of α-actin (a marker ofmyofibroblast activation) in smooth muscle inthe infarct zone (54) and of plasma creatinephosphokinase (39).
The few studies available do however open uppossibilities for the use of ECE inhibitors inmyocardial infarction and will encourage furtherstudies to evaluate the short- and long-termaction of these compounds.
RestenosisRestenosis (re-occlusion of the vessel lumen
due to neointimal proliferation) occurs in 30-50%of cases in the first six months followingangioplasty, which is an important limitation onthe procedure’s efficacy (55).

É de assinalar uma tendênciapara a melhoria dos parâmetros hemodinâmicos(pressão arterial média, pressão arterial sistólicae pressão telediastólica do ventrículo esquerdo),que, no entanto, não foi estatisticamentesignificativa (54).
A nível molecular, os inibidores da ECEdiminuíram os níveis de α-actina de músculo lisona zona de enfarte (marcador de activação domiofibroblastos) (54) e a actividade da creatinafosfocínase no plasma (39).
Há ainda poucos trabalhos descritos, abrindo,não obstante, as perspectivas da aplicação dosinibidores da ECE no enfarte do miocárdio eimpulsionando o desenvolvimento de novosestudos que avaliem a acção destes compostos acurto e a longo prazo.
ReestenoseA reestenose, proliferação da neoíntima
reocluindo o lúmen do vaso, ocorre em 30-50%das angioplastias nos primeiros 6 meses após oprocedimento, constituindo um importantefenómeno limitativo da sua eficácia (55).
Após angioplastia de balão da artéria carótidade rato, a administração de CGS 26303(10mg.Kg-1dia-1 i.v. via mini bomba osmótica comlibertação de 5µl/h durante 14 dias na veiajugular), reduziu a proliferação da neoíntima em35%. Outras formas de administração destefármaco, por via s.c., intraperitoneal, ou por viaoral através do pró-fármaco CGS 26393reduziram também a proliferação da neoíntima,em 19, 21 e 23% respectivamente, não seobservando qualquer melhoria com o inibidorselectivo EPN-CGS 24592 (55). Este efeito tambémse tinha verificado com o Fosforamidon que,administrado por mini-bomba osmótica de 10mg.Kg-1.min-1 s.c. durante 21 dias em ratossubmetidos a lesão da artéria carótida comumesquerda, reduziu a formação da neoíntima em50%, sem alterar o tamanho da média (56).
Em ambos os estudos não houve alteraçãoda morfometria da artéria contralateral, nãosujeita a angioplastia (56; 55).
A ausência de efeito com a administração deum inibidor selectivo EPN (55) sugere que ainibição selectiva ECE pode ser eficaz. É
carotid artery, administration of CGS 26303 (10mg/kg/day iv by osmotic minipump, releasing 5µl/h into the jugular vein for 14 days) reducedneointimal proliferation by 35%. Other methodsof administering the drug - subcutaneous,intraperitoneal, or orally with the prodrug CGS26393 - also reduced neointimal proliferation (by19, 21 and 23% respectively), although noimprovement was seen with the selective NEPinhibitor CGS 24592 (55). A similar effect wasfound following administration by osmoticminipump of 10 mg/kg/min sc of phosphoramidonfor 21 days in rats subjected to left commoncarotid artery injury, which reduced neointimalformation by 50%, without altering medialthickness (56).
In both these studies, there was no change inthe morphology of the contralateral artery that didnot undergo angioplasty (56, 55).
The absence of an effect with a selective NEPinhibitor (55) suggests that selective ECE inhibitionmay be efficacious. It is important to determinewhether the more selective and potent of theseECE inhibitors lead to better results.
The development of stents, together withadjuvant pharmacological and radiationtherapies, has led to considerable improvementsin angioplasty, with significant reductions in therestenosis rate (57). Despite advances in therapyand the good results obtained with new drugs,ways to prevent the process underlying restenosiscontinue to be vigorously investigated, and futurestudies will determine whether ECE inhibitorshave a therapeutic role to play in the stent era.
Pulmonary hypertensionAlthough pulmonary hypertension is the only
pathology for which there is general agreementconcerning the efficacy of inhibiting theendothelin pathway (2), with one receptorinhibitor already approved for clinical use,there have been few studies on ECE inhibitionin this context.
Takahashi et al. found that in rats withmonocrotaline-induced pulmonary hypertension,100 mg/kg/day of FR901533, administered24 h after injection of monocrotaline, protectedagainst medial thickening of pulmonary arteriesand development of right ventricular overload. Nodifferences in left ventricular systolic pressurewere observed between the control group and thepulmonary hypertension group (58). 393

O desenvolvimento de stents, terapêuticafarmacológica e radiação adjuvantes, teve grandeimpacto na melhoria da técnica diminuindosignificativamente a taxa de reestenose (57). Apesarda evolução terapêutica, e dos bons resultadosobtidos com os novos agentes, a estratégia paraevitar o processo subjacente à reestenosecontinua em profícuo desenvolvimento. Novosestudos dirão se os inibidores da ECE terão umlugar terapêutico na era dos stents.
Hipertensão pulmonarApesar desta patologia ser a única em que não
há grande controvérsia acerca da eficácia dainibição da via da endotelina (2), havendoinclusivamente um inibidor dos receptoresaprovado para uso clínico, foram desenvolvidospoucos estudos sobre a inibição da ECE.
Takahashi e colaboradores verificaram que,em ratos com hipertensão pulmonar induzidapela monocrotalina, o FR901533 (100mg/Kg/dia)administrado a partir das primeiras 24h apósinjecção de monocrotalina, protegia doespessamento da camada média das artériaspulmonares e do desenvolvimento de sobrecargado ventrículo direito. Não foram observadasdiferenças na pressão sistólica do ventrículoesquerdo entre o grupo controlo e o dehipertensão pulmonar (58).
O Fosforamidon reduziu o aumento da pressãopulmonar após o bypass cardiopulmonar eparagem cardiocirculatória da hipotermiaprofunda no porco. Este inibidor foi administrado15 minutos antes do bypass cardiopulmonarnuma dose de 30mg/kg através de um catéterintroduzido na artéria pulmonar, registando-seum valor final de pressão arterial pulmonar de22,5±1,3mmHg versus 32,1±1,1mmHg no grupocontrolo (59).
Foi descrito um aumento dos níveis de ET-1associado à hipertensão pulmonar (60; 61), assimcomo uma diminuição da expressão dosreceptores ETB na hipertensão pulmonarinduzida pela monocrotalina (62), e aumento daexpressão dos receptores ETB noutras formas dehipertensão pulmonar ((63); para revisão ver (64)).

Increased ET-1 levels have been associatedwith pulmonary hypertension (60, 61), as well asreduced ETB-receptor expression in monocro-taline-induced pulmonary hypertension (62) andincreased ETB-receptor expression in other formsof pulmonary hypertension (63) (for review see (64)).
ETB receptors have beneficial effects in thisdisease since they are involved in endothelinclearance, thus decreasing circulating levels, andreduce pulmonary artery pressure (65). However,harmful effects have also been described, withthe receptors modulating the proliferation (66) andcontraction (67, 68) of pulmonary artery smoothmuscle cells.
The beneficial effects appear to be modulatedby receptors located in the endothelium, withharmful ones being modulated by receptors insmooth muscle (64), suggesting that it may bepossible to develop a specific antagonist oftherapeutic value.
In the study cited above, Takahashi et al.observed an increase in the immunoreactivity ofETB receptors in the pulmonary artery and a fallin ET-1 levels in the group treated withFR901533. The latter effect was produced byECE inhibition and possibly by the increasednumber of ETB receptors (58).
Thus, while intuition suggests that a selectiveETA antagonist would have advantages over acombined ETA/ETB antagonist, leaving ETBreceptors free for ET clearance and pulmonaryvasodilation, on the other hand, blocking theproliferative and vasoconstricting actionmodulated by the latter receptors may also bebeneficial.
Bosentan, an ETA/ETB antagonist, was shownto be efficacious in the treatment of variousforms of pulmonary hypertension (69-74), and wasapproved by the US Food and DrugAdministration in 2001 for treatment of patientswith this disease in functional class III and IV.

seus níveis circulantes, e diminuem a pressão daartéria pulmonar (65). No entanto, também foramdescritas acções nocivas mediadas por estesreceptores como sejam a proliferação (66) e acontracção (67; 68) das células musculares lisas daartéria pulmonar.
As acções benéficas parecem ser mediadaspelos receptores localizados no endotélio e asnefastas pelos receptores localizados no músculoliso (64), sugerindo que será possível desenvolverum bloqueio específico com tradução terapêutica.
No estudo supracitado, Takahashi ecolaboradores, observaram um aumento daimunoreactividade dos receptores ETB na artériapulmonar e diminuição dos níveis de ET-1 nogrupo tratado com FR901533. Este último efeitofoi promovido pela acção inibitória sobre a ECE epossivelmente pelo aumento do número dereceptores ETB (58).
Assim, se por um lado é intuitivo pensar queum antagonista selectivo ETA teria vantagemsobre um antagonista misto ETA/ETB, deixandoos receptores ETB livres para o clearance da ETe vasodilação pulmonar, por outro, o bloqueio dasacções proliferativas e vasoconstritoras mediadaspor este receptor também poderá ter efeitobenéfico.
O Bosentan, antagonista ETA/ETB, mostrou--se eficaz no tratamento de várias formas dehipertensão pulmonar (69-74), sendo aprovado pelaFood and Drug Administration em 2001 para otratamento desta doença em classe funcional III eIV.
O Sitaxsentan (75-77) e o Ambrisentan (78) sãoinibidores selectivos ETB, com bons resultadosno tratamento da hipertensão pulmonar,aguardando a aprovação para uso clínico.
Até à data, apenas um estudo fez acomparação directa entre estes dois tipos deinibidores. Neste estudo, o LU13525250mg/Kg/dia (antagonista ETA selectivo) e oBSF420627 50mg/Kg/dia (antagonista ETA//ETB) foram administrados durante três semanasa ratos com hipertensão pulmonar induzida pelamonocrotalina. Apesar de haver uma tendênciafavorável ao inibidor misto, os resultados obtidosforam semelhantes nos dois grupos. É importantereferir que a biodisponibilidade e a semi-vida doBSF420627 são inferiores às do LU135252 (79).
Qual será então o papel dos inibidores daECE neste contexto? Se por um lado diminuem osníveis de ET, bloqueando o seu papel na
selective ETA receptor antagonists that haveshown good results in the treatment of pulmonaryhypertension, and approval for clinical use isawaited.
To date, only one study has directly comparedthe two types of antagonists. In this study, 50mg/kg/day of LU135252 (a selective ETAantagonist) or 50 mg/kg/day of BSF420627 (anETA/ETB antagonist) were administered to ratswith monocrotaline-induced pulmonary hyper-tension for three weeks. Although the combinedantagonist was slightly more effective, the resultswere similar in both groups. It should be notedthat the bioavailability and half-life ofBSF420627 are lower than LU135252 (79).
What then is the role of ECE inhibitors in thiscontext? While on the one hand they decrease ETlevels, blocking its effect in the pathophysiologyof the disease, they also decrease the likelihoodof ET binding to endothelial ETB receptors. Arethey better than receptor antagonists? In thestudy by Takahashi et al., the effect of FR901533was similar to that of the ETA receptor antagonistFR139317 (58). Would it be advantageous toassociate ECE inhibition with receptorantagonism, and if so, which receptor? Thebenefits of each of these blocking agents remainthe subject of debate and the results of furtherstudies are awaited.
Arterial hypertensionWith regard to the effects of ECE inhibitors on
arterial blood pressure (BP), different resultshave been obtained depending on theexperimental model used.
In anesthetized normotensive rats,phosphoramidon had no effect on baseline BP,but it did inhibit the pressor action induced byadministration of big ET-1 (80).
In conscious, spontaneously hypertensiverats, a fall in mean BP (mBP) was seen afteradministration of 10, 20 or 40 mg/kg/h ofphosphoramidon for five hours (81), but not after0.3 or 1 mg/kg/min for two hours (80). Theantihypertensive effect of the drug was againobserved in reduced BP levels in a model ofhypertension induced by occlusion of the renalartery and in rats with hypertension induced byadministration of deoxycorticosterone acetate(DOCA) and salt. Haverá vantagemna associação da inibição ECE à inibição dosreceptores? Se sim, a qual deles? Permanece acontrovérsia acerca do benefício de cada umdestes bloqueios aguardando-se o resultado denovos estudos.
Hipertensão arterialRelativamente à acção dos inibidores da ECE
sobre a pressão arterial, observaram-se diferentesresultados consoante o modelo experimental dedoença estudado.
Em ratos normotensos anestesiados, oFosforamidon não teve efeito sobre a pressãoarterial basal, no entanto, inibiu a acção pressorainduzida pela administração de big ET-1 (80).
Em ratos espontaneamente hipertensosconscientes, verificou-se a diminuição da pressãoarterial média (PAM) após administração de 10,20 e 40 mg/Kg/h de Fosforamidon durante 5horas (81), mas não após administração de 0,3 e 1mg/Kg/min durante 2 horas (80). Este fármacoreiterou o seu efeito anti-hipertensor ao diminuira pressão arterial, nos modelos de hipertensãopor oclusão da artéria renal e em ratoshipertensos pela administração de acetato dedesoxicorticosterona (DOCA) e sal. Nestesestudos, o Fosforamidon foi administrado emdoses de 10, 20 e 40 mg/kg/h durante 5 horas ede 0,01, 0,1 e 1mg/kg/h durante 2 horas,respectivamente, em ratos conscientes (81; 80).
Após estes estudos iniciais, a investigação foiretomada dando particular atenção à inibiçãosimultânea da enzima de conversão daangiotensina. O CGS 35066 (30mg/Kg/dia emadministração intra-arterial contínua durante3 dias) diminuiu a pressão arterial média11mmHg em ratos normotensos e 22mmHg emratos espontaneamente hipertensos. A adição debenazepril, inibidor da enzima de conversão daangiotensina, (30mg/kg/dia em administraçãointra-arterial contínua durante 3 dias simulta-neamente com o CGS 35066), condicionou umamaior diminuição da PAM em ratos normotensose tratou a hipertensão nos ratos hipertensos.Removendo posteriormente o CGS 35066

Following these initial studies, research thenfocused on simultaneous inhibition of ECE andangiotensin-converting enzyme. CGS 35066(continuous intra-arterial administration of 30mg/kg/day for three days) reduced mBP by 11mmHg in normotensive rats and by 22 mmHg inspontaneously hypertensive rats. The addition ofthe ACE inhibitor benazepril (continuousintra-arterial administration of 30 mg/kg/day forthree days simultaneously with CGS 35066) ledto a larger fall in mBP in normotensive ratsand controlled hypertension in the hypertensiverats. Subsequently discontinuing CGS 35066while maintaining benazepril led to a smallerdecrease in BP in normotensive rats, with nosignificant change in BP levels in spontaneouslyhypertensive animals (82).
Thus, at the above doses, ACE inhibitionwithout ECE inhibition was sufficient to controlhypertension, and a combination of the twoinhibitors had an additional effect only innormotensive rats.
Following this line of research came the workof Daull et al., who compared the effect ofselective ECE-1 (CGS 35066), ACE (benazepril)and NEP (CGS 24592) inhibitors with thetriple inhibitor CGS 35061 in spontaneouslyhypertensive 36-week-old rats. CGS 35601reduced mBP (156±4 mmHg) by between 0% and40% for the lowest (0.01 mg/kg/day) and highest(5 mg/kg/day) doses respectively. ACE inhibitionwith benazepril was four times less effective thanCGS 35601 at doses of 1 mg/kg/day for bothdrugs, and half as effective at doses of 5mg/kg/day. Combined ACE and NEP inhibition,and the combination of three inhibitors(benazepril, CGS 35066 and CGS 24592), had noadditional effect (83).
A subsequent study by the same group set outto determine the effects of this triple inhibitor ina different model of hypertension (low-renin,high-salt hypertension). Dahl salt-sensitive ratswith unrestricted salt intake were fed a normaldiet (group 1) or a salt-rich diet (groups 2 and 3)for six weeks, and then instrumented with acarotid artery catheter and placed in metaboliccages for 30 days. Hemodynamic, hematologicand biochemical parameters were assesseddaily. The treatment began seven days afterstabilization, with groups 1 and 2 receiving398

(mantendo o benazepril), observou-se uma menordiminuição da pressão arterial nos ratosnormotensos e não se verificou alteraçãosignificativa da pressão arterial nos ratosespontaneamente hipertensos (82).
Em suma, para as doses acima referidas, ainibição da ECA, não associada à inibição daECE, revelou-se suficiente para controlar ahipertensão arterial, sendo que a combinação dosdois inibidores teve um efeito aditivo apenas paraos ratos normotensos.
No seguimento desta linha de investigaçãosurgem os trabalhos de Daull e colaboradorescomparando o efeito dos inibidores selectivosECE-1 (CGS 35066), ECA (benazepril) e EPN(CGS 24592) com o inibidor triplo CGS 35061 emratos espontaneamente hipertensos (com idade de36 semanas). Com o CGS 35601 a PAM (156 ± 4mm Hg) diminuiu entre 0% e 40% para as dosesmais baixa (0,01mg/Kg/d) e mais alta (5mg/Kg/d),respectivamente. Com a inibição da ECA(benazepril) obteve-se um resultado quatro vezesmenor do que o observado com o CGS 35601(comparando doses de 1mg/Kg/d para os doisfármacos) e duas vezes menor comparando dosesde 5mg/Kg/d. A inibição combinada ECA e EPN,bem como a combinação dos três inibidores(benazepril, CGS 35066 e CGS 24592) não teveefeito adicional (83).
Num estudo subsequente, os mesmos autores,pretenderam determinar os efeitos deste inibidortriplo noutro modelo de hipertensão (hipertensãode renina baixa induzida por dieta rica em sódio).Ratos Dahl sensíveis ao sódio, não sujeitos arestrição salina (DSS) foram alimentados comdieta normal (grupo 1) ou com dieta rica em sal(grupos 2 e 3) durante 6 semanas eposteriormente instrumentados com um catétercarotídeo e colocados em gaiolas metabólicasdurante 30 dias. Os perfis hemodinâmico,hematológico e bioquímico foram avaliadosdiariamente. O tratamento começou 7 dias após operíodo de estabilização com os grupos 1 e 2 afazerem placebo e o grupo 3 a fazer CGS 35601em doses de 0,1, 1 e 5mg/Kg/dia durante 6 diaspara cada dose por infusão intra-arterialcontínua, seguido por um período de 5 dias dewashout. Nos ratos que receberam CGS 35601, aPAM diminuiu para os valores basais dos ratosDSS alimentados com dieta normal. A frequênciacardíaca não foi afectada e o perfil hemodinâmicovoltou ao normal durante o período de washout. O
placebo and group 3 receiving CGS 35601 indoses of 0.1, 1 and 5 mg/kg/day for six days ateach dosage by continuous intra-arterial infusion,followed by a five-day washout period. In the ratsreceiving CGS 35601, mBP fell to the baselinevalues of the rats fed a normal diet. Heart rate wasnot affected and the hemodynamic profilereturned to normal during the washout period.The effect was dose-dependent and the drug waswell tolerated (84).
The only drug included in clinical trials, SLV306, has also shown good preclinical results inarterial hypertension, decreasing systolic anddiastolic pressure by 18 mmHg and 4.7 mmHgrespectively, and increasing diuresis in DOCA-salt sensitive rats (47).
Although there would appear to be advantagesto administering ECE inhibitors in arterialhypertension, different degrees of efficacy arefound in various animal models of the disease.Several factors may be responsible for theinconsistencies in the results, including drugdosage, method of administration, age of theanimals, and the model itself.
Simultaneous inhibition of endothelin-converting enzyme, angiotensin-convertingenzyme and neutral endopeptidase constitutes anew approach to the treatment of hypertensionthat has the theoretical advantage of reducinglevels of two vasoconstrictors, endothelin andangiotensin, while increasing levels of bradykininand atrial natriuretic peptide, two importantvasodilators. Further studies will be required toexplore this advantage, as well as to compare theassociation of selective ECE-1 and ACEinhibitors with triple inhibition.
Human studiesSolvay Pharmaceuticals Inc. have published
the preliminary results of a phase II clinical trialof SLV 306 in hypertensive patients, reportingsignificant reductions in systolic and diastolicblood pressure and good tolerance to the drug (85).
Heart failureIn a canine model of heart failure induced by
rapid stimulation of the right ventricle,FR901533 was administered acutely andchronically.

O único fármaco a integrar ensaios clínicos, oSLV 306, também obteve bons resultadospré-clínicos na hipertensão arterial, diminuindo apressão arterial sistólica e diastólica 18mmHg e4,7mmHg, respectivamente, e aumentando adiurese em ratos sensíveis à administração deDOCA e sal (47).
Apesar de parecer haver vantagem naadministração dos inibidores da ECE notratamento da hipertensão arterial, observam-sediferentes graus de eficácia em vários modelosanimais de doença. Diversos factores podem serresponsáveis pela falta de equidade dosresultados como sejam a dose do fármaco, o tipode administração, a idade dos ratos, e o modeloem si.
A inibição simultânea da enzima de conversãoda endotelina, enzima de conversão daangiotensina e endopeptidade neutral constituiuma nova abordagem para o tratamento dahipertensão que encerra a vantagem teórica daredução dos níveis de dois vasoconstritores,endotelina e angiotensina associada ao aumentodos níveis de bradicinina e peptídeo natriuréticoauricular, dois importantes vasodilatadores. Umavantagem, a explorar em futuros estudos assimcomo a comparação da associação iECE-1selectivo e iECA com os inibidores triplos.
Estudos humanosA farmacêutica Solvay apresentou os
resultados preliminares do ensaio clínico de faseII com SLV 306 em doentes hipertensos,referindo uma redução significativa da pressãoarterial sistólica e diastólica, e boa tolerância aofármaco (85).
Insuficiência cardíacaNum modelo de insuficiência cardíaca (IC)
induzida por estimulação ventricular rápida doventrículo direito de cão, administrou-seFR901533 de forma aguda e crónica.
Após administração aguda (1 e 3mg/Kgseparadas por 30 min) verificou-se aumento dodébito cardíaco, diminuição da PAM, dasresistências vasculares (pulmonar e sistémica),da actividade da renina plasmática, dos níveis deangiotensina II, aldosterona e PNA, emanutenção dos níveis de noradrenalina. (86)
(pulmonary and systemic), plasma renin activity,and angiotensin II, aldosterone and ANP levels,with no change in noradrenaline levels (86).
Chronic administration (0.3 mg/kg/h for twoweeks) also led to increased cardiac output,together with reduced pulmonary capillary wedgepressure, but had no effect on blood pressure (87, 88).There was a fall in levels of ANP, aldosterone (88),angiotensin II and noradrenaline (87) andprevention of vascular remodeling (as assessed bycollagen deposition, medial thickness and lumendiameter) (87).
Similar results were obtained with chronicadministration of CGS 26303 (10 mg/kg/day for30 days) in rats with HF due to completeocclusion of the left coronary artery, with reducedcardiac remodeling, preload and afterload andincreased cardiac output (89).
Interestingly, Fontaine et al. reporteddepression of myocardial function with ECEinhibition. Phosphoramidon (10 µM infusion)reduced cardiac contractility in isolated Syrianhamster heart (with and without cardiomyopathy),with no change in coronary vascular tone (90).
Emoto et al. compared chronic inhibition ofECE/NEP (CGS 26303), NEP (CGS 24592) andACE (temocapril) in a model of transitionfrom left ventricular hypertrophy to HF inhypertensive rats. All of the drugs improved leftventricular diameter and ejection fraction. Bothtemocapril and CGS 26303 reduced perivascularfibrosis and overall cardiac hypertrophy.However, only CGS 26303 completely preventedright ventricular hypertrophy and led toregression, and was more effective than ACEinhibition in reducing perivascular fibrosis (91).
Following the same line of investigation,Mellin et al. set out to determine whether theassociation of an ECE/NEP inhibitor (CGS26303, 10 mg/kg/day) with an ACE inhibitor(benazepril, 2 mg/kg/day) was superior to theiruse in isolation. Benazepril and CGS 26303 wereadministered alone or in combination to rats withHF (subcutaneously for 28 days beginning on the7th day following coronary occlusion). The resultsof the association of the two inhibitors provedsuperior in reducing blood pressure, leftventricular diameter and end-diastolic pressure,as well as collagen deposition and the weight ofviable left ventricular myocardium. Cardiacoutput increased to a similar extent in thedifferent study groups (92).400

Após administração crónica (0,3 mg/Kg/hdurante duas semanas) verificou-se de igualmodo um aumento do débito cardíaco e umaredução da pressão capilar pulmonar, mas, noentanto, ausência de efeito na pressão arte-rial (87; 88). Os níveis de PNA, aldosterona (88),angiotensina II e noradrenalina diminuíram (87) epreveniu-se o desenvolvimento de remodelagemvascular (avaliada pelo depósito de colagénio,espessura da média e diâmetro do lúmen) (87).
Resultados semelhantes foram obtidos com oCGS 26303 (10 mg/Kg/dia por 30 dias)administrado cronicamente a ratos com IC poroclusão completa da artéria coronária esquerda.Nestes animais verificou-se diminuição daremodelagem cardíaca, da pré e da pós-carga eaumento do débito cardíaco (89).
Curiosamente, Fontaine e colaboradoresdescreveram depressão da função miocárdicacom um inibidor da enzima de conversão daendotelina. O Fosforamidon (10µM em infusão)diminuiu a contractilidade cardíaca em coraçãoisolado de hamster Sírio (com cardiomiopatia enormal), sem alterar o tónus vascular coronário (90).
Emoto e colaboradores compararam a inibiçãocrónica de ECE/EPN (CGS 26303), EPN (CGS24592) e ECA (termocapril) num modelo detransição de hipertrofia do ventrículo esquerdopara IC em ratos hipertensos. Todos os fármacosmelhoraram o aumento do diâmetro do ventrículoesquerdo e a diminuição da fracção de ejecção.Tanto o termocapril como o CGS 26303diminuiram a fibrose perivascular e reduziram ahipertrofia cardíaca globalmente. No entanto, só oCGS 26303 preveniu completamente e conseguiuregredir a hipertrofia do ventrículo direito sendomais eficaz que o iECA na diminuição da fibroseperivascular (91).
No seguimento da mesma linha, Mellin ecolaboradores pretenderam verificar se aassociação de um inibidor ECE/EPN(CGS26303, 10 mg/Kg/d) e um inibidor ECA(benazepril, 2 mg/Kg/d), era superior ao seu usoisolado. Benazepril e CGS 26303 foramadministrados sozinhos ou em combinação a ratoscom insuficiência cardíaca (subcutaneamentedurante 28 dias iniciados ao 7ºdia pós-oclusão dacoronária). Os resultados da associação dos doisinibidores mostraram a sua superioridade nadiminuição da pressão arterial, do diâmetro epressão telediastólica do ventrículo esquerdo etambém na diminuição da acumulação de
The good results obtained with SLV 306prompted its inclusion in a clinical trial.Administration of SLV 306 (30 mg/kg/day) ingenetically hypertensive rats with HF(SHHF/Mcc-facp) reduced left ventricular end-diastolic pressure, peripheral vascular resistance,cardiac hypertrophy, and renin and aldosteronesecretion, and increased urinary output (44). Thesame dose administered to HF rats with aorticstenosis reduced cardiac hypertrophy andpulmonary congestion (48). In dogs with HF, itincreased cardiac output, contractility, diuresisand natriuresis, and reduced left ventricular end-diastolic pressure (44).
An interesting finding on the impact of ECEinhibition on survival was reported by Muller etal. in a model of transgenic rats carrying thehuman gene for angiotensin and renin. Theanimals developed arterial hypertension, as wellas severe renal and cardiac damageindependently of increased BP, with death ataround seven weeks. In this model, RO0687629reduced cardiac damage and mortality by 20%(36).
Human studiesVarious neurohumoral modulators are
implicated in the pathophysiology of HF,including angiotensin, endothelin, noradrenaline,and aldosterone. Inhibitors of some of these areroutinely used in clinical practice, but this is notyet the case for endothelin.
The harmful effects of elevated ET levels areseen as an important potential therapeutic goal inHF treatment, not only because of thehemodynamic effects of endothelin itself, but alsodue to changes in levels of other humoralmodulators. Following the promising resultsobtained in animal models with both endothelinreceptor antagonists and endothelin-convertingenzyme inhibitors, various clinical trials wereperformed, particularly of the former. However,the results for these antagonists have beendisappointing, in some cases showing aworsening of cardiac dysfunction (93-95).
These findings are in agreement with what hasbeen suggested by various authors (for review see (2)), andas reported in Fontaine’s study on phospho-ramidon in a model of HF in the Syrianhamster (90), that endothelin helps maintaincardiac function even in the failing heart. Thissuggests a possible contraindication to use ofthese drugs, at least in patients with poor left 401

A administração de SLV 306 (30mg/kg/dia) a ratos geneticamente hipertensoscom IC (SHHF/Mcc-facp), diminuiu a pressãotelediastólica do ventrículo esquerdo, aresistência vascular periférica, a hipertrofiacardíaca, a secreção de renina e aldosterona eaumentou o débito urinário (44). A mesma doseadministrada a ratos com IC por estenose aórtica,reduziu a hipertrofia cardíaca e a congestãopulmonar (48). Em cães com IC, aumentou o débitocardíaco, a contractilidade, a diurese e anatriurese, e reduziu a pressão telediastólica doventrículo esquerdo (44).
Um ponto interessante refere-se ao impactosobre a sobrevida observado por Muller ecolaboradores num modelo de ratos transgénicosportadores do gene humano da angiotensina erenina. Estes animais desenvolvem hipertensãoarterial bem como lesão renal e cardíaca gravesindependentes do aumento da pressão arterial,com morte ocorrendo cerca das 7 semanas deidade. Neste modelo, o RO0687629 diminuiua lesão cardíaca e reduziu a mortalidade em20% (36).
Estudos humanosVários mediadores neurohumorais estão
implicados na fisiopatologia da IC, como aangiotensina, a endotelina, a noradrenalina, aaldosterona, entre outros. Inibidores de algunsdestes mediadores neurohumorais são de usocorrente na clínica, mas não ainda os inibidoresda via da endotelina.
Os efeitos deletérios do aumento dos níveis daET são encarados como um potencial eimportante alvo terapêutico no tratamento da IC,não só pelos efeitos hemodinâmicos próprios daendotelina, mas também pela alteração dos níveisde outros mediadores humorais. Após osresultados promissores obtidos em modelosanimais, quer com antagonistas dos receptores,quer com antagonistas da enzima de conversão daendotelina, desenvolveram-se vários ensaiosclínicos, especialmente para os primeiros. Noentanto, no primeiro caso, os resultados foramdesanimadores tendo-se inclusivamente verifi-
ventricular function.In patients with chronic HF undergoing
conventional therapy including ACE inhibitors,phosphoramidon (30 nM/min for 60 min infusedinto a brachial artery) caused increased bloodflow to the forearm (96).
SLV 306 (three single doses of 200 mg, 400mg and 800 mg) administered to patients withcongestive HF undergoing right catheterizationwas well tolerated, reducing pulmonary arteryand right atrial pressures, with no significanteffect on systemic arterial pressure, heart rate orcardiac output, and no clear evidence of a dose-dependent effect (97, 98). A clinical trial of thiscompound is in progress and final results areawaited.
There are as yet no large-scale clinicaltrials on ECE inhibitors; the SLV 306 studymentioned above is currently under way (98).Comparisons between ECE and ACE inhibitors,as well as their association, also merit furtherinvestigation in the light of the results obtainedby Emoto and Mellin (91, 92).
Chagas diseaseEndothelin-converting enzyme inhibitors
have also proved useful in the treatment of otherdiseases with cardiac involvement, such asinfection by Trypanosoma cruzi, also known asChagas disease. The disease is expressed inthe heart as acute myocarditis and chroniccardiomyopathy. Phosphoramidon administeredto infected rats at a dose of 10 mg/kg during thefirst 15 days postinfection reduced cardiacdamage and mortality, and improved prognosis (99).
FINAL COMMENTS
Significantly increased levels of ET-1 havebeen detected in studies of ECE knockout mice,suggesting that pathways other than ECE maybe involved in processing big-ET-1 (28, 100).Fernadez-Patron et al. have suggested that otherenzymes such as chymase and non-ECEmetalloproteinases are involved in ET-1formation (101). Although ECE is the main pathwayfor ET formation, further studies are required todetermine the function of these alternativepathways when ECE is blocked. Furthermore,various isoforms for the different ECEs have beendescribed, with different subcellular402

sido sugerido por vários autores (para revisão ver (2)) e como observado por Fontaine com o Fosforamidon, nomodelo de insuficiência cardíaca de rato hamsterSírio (90), de que a endotelina contribui para amanutenção da função cardíaca mesmo emdoentes. Estes dados indiciam uma possívelcontra-indicação do uso destes fármacos pelomenos em doentes com má função ventricularesquerda.
Em doentes com IC crónica em tratamentoconvencional, incluindo um inibidor da enzimade conversão da angiotensina, o Fosforamidon(30 nmol/min durante 60 min infundido na artériabraquial) provocou aumento do fluxo sanguíneodo antebraço (96).
O SLV 306 (três doses únicas 200 mg, 400mg e 800 mg) administrado a doentes cominsuficiência cardíaca congestiva submetidosa cateterismo direito foi bem tolerado,diminuindo a pressão arterial pulmonar e apressão da aurícula direita, não tendo efeitosignificativo na pressão arterial sistémica,frequência ou débito cardíacos e sem apresentarclara evidência de dose-efeito (97; 98). O ensaioclínico com este composto está a decorreraguardando-se a publicação dos resultados finais.
Para os iECE ainda não há ensaios clínicosem larga escala, estando a decorrer o estudo como SLV 306 (98). Além disso, a comparação entreiECE e iECA e mesmo a sua associação mereceser mais explorada atendendo aos resultadosobtidos por Emoto e Mellin (91; 92).
Doença de ChagasOs inibidores da enzima de conversão da
endotelina também se revelaram úteis notratamento de outras doenças com expressãocardíaca, como a infecção por Trypanossomacruzi, também conhecida como doença deChagas. No coração, esta doença expressa-se soba forma de miocardite aguda e cardiomiopatiacrónica. O Fosforamidon, administrado a ratosinfectados, numa dose de 10mg/Kg nos primeiros15 dias pós-infecção diminuiu a lesão cardíaca ea mortalidade, condicionando um melhorprognóstico (99).

The specific role of each of thesehas yet to be determined in the pathophysiologyof diseases in which endothelin plays a part, andparticular targeted therapies may be developed inthe future based on inhibition of each isoform. Inaddition, ECE-1 can also hydrolyze otherpeptides such as bradykinin (102, 103), substance P,angiotensin I and insulin (103), and the implicationsof this effect and the therapeutic applicationsarising from it remain to be discovered.
Barker et al. created an antisenseoligodeoxynucleotide to ECE-1c with the aimof studying the role of this enzyme in ETformation (104). Although this study was notperformed for pharmacological purposes, it hasopened the way for pharmacogenomics tointervene in the endothelin pathway.
To date, there has been less progress indeveloping ECE inhibitors than endothelinreceptor antagonists. Research into ECEinhibitors received little attention in thebeginning, perhaps due to the initially promisingresults obtained with receptor antagonists.However, there is now growing interest,particularly in the context of heart failureand arterial hypertension, with excellenthemodynamic results, good drug tolerance andreduced mortality. At the same time, advancesin understanding of the cellular dynamics of ECEand characterization of its isoforms havestimulated interest in these inhibitors, with thepossibility of developing more specific, targetedblockade. The results of the clinical trial inprogress are eagerly awaited, and will certainlybe an important step in the research anddevelopment of new therapeutic strategies usingendothelin-converting enzyme inhibitors.

CONSIDERAÇÕES FINAIS
Em estudos com ratinhos knockout para asECE, foram identificados níveis signifi-cativamente elevados de ET-1, sugerindo queoutras vias que não a da ECE podem existir parao processamento de big-ET-1 (28; 100). Fernadez--Patron e colaboradores aventaram outrasenzimas como a quimase e metaloproteinasesnão-ECE, como também participando naformação de ET-1 (101). Apesar da ECE ser aprincipal via para a formação de ET, é necessáriopromover estudos no sentido de determinar quala função destas vias alternativas quando a ECE ébloqueada. Além disso, foram descritas váriasisoformas para as diferentes ECE, com diferenteslocalizações subcelulares não estando aindaexploradas funções específicas para cada umadelas na fisiopatologia de doenças em que aendotelina tem um papel contributivo, podendo,no futuro, desenvolver-se terapêuticas maisdirecionadas, com inibidores específicos paracada isoforma. Por outro lado, a ECE-1 podetambém hidrolisar outros peptídeos como abradicinina (102; 103), a substância P, a angiotensina Ie a insulina (103) estando ainda a descoberto asreais implicações deste efeito e as particu-laridades terapêuticas que daí podem advir.
Barker e colaboradores criaram umoligodesoxinucleótido anti-sense para a ECE-1ccom o intuito de estudar o papel desta enzima naformação de ET (104). Apesar de não ter sidodesenvolvido com sentido farmacológico, ficaaberta a página da farmacogenómica naintervenção da via da endotelina.
Até à data, tem havido menos progresso nodesenvolvimento de fármacos inibidores da ECEdo que antagonistas dos receptores da endotelina.A investigação dos inibidores da ECE foipouco explorada inicialmente, talvez fruto dosresultados inicialmente auspiciosos obtidos comos antagonistas dos receptores. No entanto, temvindo a ser retomada cada vez com maisinteresse, particularmente na área dainsuficiência cardíaca e da hipertensão arterial,com excelentes resultados hemodinâmcos, boatolerância ao fármaco e diminuição damortalidade. Por outro lado, a evolução doconhecimento da dinâmica celular da ECE e acaracterização das suas isoformas, vem estimularo interesse por estes inibidores com apossibilidade de uma inibição mais específica edirecionada. Aguardam-se com expectativa osresultados do ensaio clínico em curso que,certamente, serão um passo importante para ainvestigação e desenvolvimento de novasestratégias terapêuticas com fármacos inibidoresda enzima de conversão da endotelina.
