• dagadt lábak! – az esetek 50%-a CHF akut decompensatioja, sokszor a beteg nem is veszi észre
• Pangás• aorta stenosis!!
• EKG – ACS? Egyéb? (pl. bardycardia)• Laboratórium:
• Ha kérdés a kardiális eredet – pro-BNP• Troponin – ACS (kicsit biztos pozitív, ha > 5x emelkedés –
gyanús ACS-re. Kérdés esetén ismétlés.

Atípusos megjelenés
• Nincs vagy enyhe fájdalom
• émelygés/hányás, légszomj, palpitáció, syncope
• 30%!
• Nők, diabetes, idősek
• Késlekedő terápia
• Lényegesen alacsonyabb reperfúzió, „guideline” kezelés!
• =MAGAS RIZIKÓJÚ CSOPORT!
Atípusos EKG
• BTSZB
• Pacelt ritmus
• Posterior infarctus
• Perzisztáló fájdalom, de nincs (még) típusos ST eleváció
• Izolált ST eleváció aVR-ben(kiterjedt inferolateralis ST depressioval főtörzs occlusio jele lehet!!!)
Szívelégtelenség/kardiogen shock –a teljes revascularisation túl
• Lélegeztetés (ne akkor kezdjük intubálni, amikor bradycardia lép fel a hypoxiától)
• Keringéstámogatás
– IABP
– ECMO:
• Állapot stabilizálódásáig (stunning)
• Vagy döntésig: kezelés befejezése vagy BIVAD�HTX
• Vesepótló kezelés – rel. liberális alkalmazása (nem mindig jó megvárni az akut dialysis klasszikus indikációit)
• Infekció/sepsis – korai észlelés és kezelés
ECMO
• elsőként választható eszköz mech. keringéstámogatásra• hasonló effektivitású, mint a többi keringétámogató th.• relatív olcsó• max. 30 nap• Veno-venosus (O2)• Veno-arteriosus (O2, perctérfogat)• Helyszínen

Short-Term Use of Inodilator Treatment- Milrinone
OPTIME-CHF investigators:- randomized 941 patients (mean age- 65 years) - patients with an exacerbation of systolic HF with NYHA
class III to IV HF and an LVEF <0.40 (mean LVEF, 0.23)
- studied the effect of short-term Milrinone infusion (48 to 72hrs) - primary outcome:
- cumulative days of hospitalization within 60 daysof randomization
- secondary outcomes:-adverse events and mortality

SURVIVE-W: Background
• Levosimendan, a calcium-sensitizing agent that has both vasodilator and inotropic effects, has been proposed as an alternative to the standard treatment of dobutamine
• Two small pilot trials, LIDO and CASINO, showed lower rates of mortality with levosimendan compared with dobutamine and placebo
• The goal of the trial is to compare treatment with levosimendan to treatment with dobutamine in patients with acute heart failure who are in need of intravenous inotropic support
• SURVIVE is the first prospective randomized mortality trial of IV drug therapy in acute decompensated heart failure

• Among patients with acute heart failure in need of intravenous inotropic
support, treatment with levosimendan did not significantly reduce all-cause
mortality compared with treatment with dobutamine
• There was no difference in all-cause mortality at 31 days or at 5 days though
in the subgroup of patients with a history of heart failure, mortality did trend
lower at 5 days
• BNP at 24 hours was significantly lower (p=<0.001) in the levosimendan
group compared with the dobutamine group. BNP dropped by almost 50%
after the infusion and stayed low for at least five days
• There were no differences in hypotension or ventricular tachycardia.
However, atrial fibrillation occurred more often in the levosimendan group
(9% vs 6%) while cardiac failure occurred less often in the levosimendan group
(12% vs 17%)
