This course is intended for general dentists and pedodontists who have little or no orthodontic experience and want to learn how to incorporate it into their practices or wish to progress from removable appliances to a comprehensive, fixed technique. This course also welcomes any dental specialists that desire to participate. The 12-session course is a comprehensive program, not a “short-weekend” program of listening and writing. This is a one-of-a-kind, hands-on course, an extended experience requiring a commitment of time and energy.

“Get Acquainted Session” - This optional session is an opportunity for potential course participants to meet the course instructor, the clinical instructor/ host dentist and visit the course clinical facility. The morning lecture will examine current ideas and trends in orthodontics and will allow the participant to view documented case presentations with a Q & A regarding the course curriculum and the Differential Straight -Arch® Technique (Tip-Edge® System). In the afternoon, there will be a clinical demonstration of the technique to be taught along with a continuation of the Q & A session. This will assist the participant in making an informed decision as to whether he/she would like to make the commitment to participate in the comprehensive 12 session hands-on program. This will also allow the instructor, the clinical instructor(s) and or host dentist the opportunity to meet and interview potential course participants.

The Comprehensive Clinical Orthodontic Course will meet one weekend every two months for a total of 12 sessions. In addition to treating patients, participants will develop a digital library by taking intra-oral photographs of hands-on patients and giving case presentations to the class during the Friday seminar session. There also will be time to go over case records brought from each participant’s own practice. Along with diagnosis and technique, the Friday seminar will also include the following lecture topics:

A course binder, a technique guide, technique/diagnosis videos with manual, lecture handouts, clinical instruments and hands-on patients will be provided. Each participant will bring pliers, and diagnostic material; (reference tab 3, pgs. 13-15 for specific supplies).

Membership includes a membership certificate, quarterly newsletter/journal, and patient reference binder.

The Academy of Gp Orthodontics is solely responsible for organizing, administrating, publicizing, presenting, and recordkeeping for its continuing education programs to include the following twelve session hands-on course, a two day refresher course;

Mini-Residency; a two day dental assistants’ course; and its annual meeting. The Academy assumes both the professional and fiscal liability for the conduct and quality of its programs. All 12 session hands-on courses sponsored by the Academy provide a primary course instructor, clinical instructor(s), host dentist, and clinical facility. It is the responsibility of the Academy to make sure that the primary instructor, the clinical instructor(s), host dentist, and the site meet the required standards and criteria that comply with each state’s Dental Practice Act.…………………………………………………………………………………………………………………Attendance

It is the responsibility of each participant to maintain course dates and to attend and participate in each lecture and clinical session. All session dates are provided prior to the course by the Academy and are located in the Course Brochure, Syllabus, and Course Binder. As a courtesy, prior to each session, the Academy will send a reminder. Please be sure to furnish the Academy with your current phone/fax/address/e-mail information. If there is a date change or if a session is rescheduled, each participant will be notified immediately by Academy Headquarters.

The Academy’s hands-on format with patient treatment requires the presence of each participant for ten (10) sessions to maintain eligibility to receive the Academy’s Associate Fellowship Award. By making the commitment to take the course, the participant is committed to the hands-on treatment of the patients he/she provides for the course or to those patients provided by the course host. The integrity of the course depends upon a team effort from the course instructor, the clinical instructor(s), the host, and the course participants. If circumstances arise, and the participant is unable to attend a session, the participant needs to advise the course instructor or Academy Headquarters immediately.

Following the missed session, course handout material will be mailed to the student, or the instructor will save the material and hand it out at the subsequent session. A makeup lecture may be held if prior arrangements are made with the instructor. Other options for makeup include monitoring either a session at another site or a subsequent course.

If a participant knows he/she will be missing more than two (2) sessions, prior to beginning the course, it is recommended that they delay participating in the course, until a commitment to the twenty-four (24) month program can be arranged.

The instructor is required to keep a record of reach participant’s attendance. At the discretion of each instructor, CDE hours can be withheld upon completion of the program. This can be done if the instructor feels the participant is deficient in any area and requires additional lecture or clinical hours for verification of the course’s CDE hours and their eligibility for the Academy’s Associate Fellowship Award.

The course participant will be required to present their class treatment cases during the Friday lectures and participate in a minimum of ten (10) clinical course sessions. Participants are cautioned in applying the Differential Straight-Arch Technique to these clinical patients unless under the direct supervision of the course instructor, the clinical instructor(s) and the host dentist. When beginning patient treatment in their practice, participants are cautioned to begin patient treatment only after consulting with the course instructor, and or the clinical instructor(s). There will be specific Friday consultation sessions for reviewing participant’s patient treatment records. In addition, the instructor may be contacted by the course participant for

interim session consultations. In the event the instructor or the clinical instructor(s) are unavailable, Academy Headquarters will assist the participant in contacting the course instructor, another Academy instructor, or a member of the Academy’s Educational Committee for immediate participant assistance.

..................Clinical Protocol

All participating doctors in the Academy of Gp Orthodontics hands-on orthodontic program will be required to arrive at the host dentist’s office 30 minutes prior to the beginning of clinical patient treatment. Participating doctors should arrive at each clinical session with the following armamentarium: required pliers and supplies as stated in this course syllabus. Participants will also be responsible for bringing their own safety goggles if needed, or a special brand or size of disposable glove. It is the responsibility of each participating doctor to notify Academy Headquarters a minimum of 48 hours prior to any scheduled clinical session that he/she will not be able to attend. It is then the responsibility of Academy Headquarters to notify the course instructor, clinical instructor(s), and host dentist of any participant not attending a scheduled clinical session. It is the responsibility of the instructor to notify Academy Headquarters within 48 hours following the clinical session if a course participant did not attend the scheduled session without prior notification.

Appropriate course participant attire: Scrubs, long pants or slacks, and dress shirts. Sleeveless shirts, open-toed shoes, or high heels are not considered appropriate clinical attire. The host will provide lab jackets for participants. The host dentist will provide chairside assistants to assist course participants in cleaning operatories, sterilization procedures and assist with patient seating and dismissal. Course participants will be expected to do the following: conduct themselves in a professional manner, adhere to HIPPA regulations, and be responsible in following infection control protocol, assist with operaortory and office area cleanup, and patient documentation.

For the safety of all patients participating in the hands-on program, each course participant will be provided with a copy of the Dental Practice Act for Texas. All clinical facilities will be under direct supervision with a licensed primary instructor, and/or licensed clinical instructor(s), and/or licensed host dentist. Each student will be informed by the course instructor of the risks and precautions that need to be taken when applying new orthodontic knowledge and skills on their patients in the clinical setting as well as patients seen in their own practices. Liability insurance is required from all parties directly involved in the hands-on program and copies of their malpractice insurance coverage will be kept on file at the Academy Headquarters throughout the twelve (12) session course. All patients participating in the hands-on program will be properly screened for the program by the primary instructor, clinical instructor(s) and/or host dentist. Patients will be informed of their rights as a clinical patient, the program's patient requirements and expectations, and the necessary procedures to be performed. Each patient will be required to sign a patient informed

consent form and will remain a patient of record of the host dentist or a patient of record of the course participant during active orthodontic treatment in the hands-on program.

The Academy of Gp Orthodontics will comply with all State and Federal Laws protecting an individual's participation in the hands-on program from discrimination with regard to race, color, religion, sex, age, or national origin. Failure to comply with this policy can result in removal from their position, termination, or the cancellation of any contract agreements upon the recommendations made by the Academy Educational Committee and with consensus from the Academy Officers, and Board of Directors. To report an act of discrimination, a complaint, or a grievance please contact the Academy’s Executive Director immediately at 1-800-634-2027. All allegations will be documented, investigated, and a full written report will be sent to the Academy Educational Chairman and Academy President. Confidentiality will be maintained.

The Academy of Gp Orthodontics is a nonprofit continuing education organization which teaches the Tip-Edge® System or Differential Straight-Arch® Technique. The bracket is patented and distributed solely by TP Orthodontics, Inc. The bracket will be referred to by its commercial name the Tip-Edge bracket during the course; however, as often as possible it will be referred to by its generic term – Differential Straight-Arch Technique. In the Tip-Edge Guide, published by TP Orthodontics, Inc. it references the bracket and the system interchangeably. Students participating in the course will be informed of other products and manufacturers that may be used in conjunction with this bracket or technique. The Tip-Edge Guide is provided by the Academy of Gp Orthodontics as part of the course syllabus.

TP Orthodontics, Inc., nor any other manufacturer, provides financial support to the Academy of Gp Orthodontics or its educational program. AGpO instructors sign a conflict of interest statement and thereby do not/and will not receive any financial support from any manufacturer or product presented during the course.

For Academy refresher courses and annual meetings, all guest speakers invited to participate sign a conflict of interest statement. If a speaker has financial ties with a particular manufacturer or product he/she then must inform the Academy of Gp Orthodontics and the audience, prior to his/her lecture so this information can be printed in the meeting program.

A failure to disclose or a false disclosure will require the immediate attention of the AGpO Board of Directors and a correction will be announced or sent to all course or meeting participants immediately. It will be at the discretion of the AGpO Board of Directors to reprimand or to ask for the immediate dismissal of an instructor or a speaker if compliance is withheld.

The Academy of Gp Orthodontics is an ADA CERP recognized provider and requires a copy of each participant's current dental license and proof of malpractice insurance

Associate Fellowship Award This award will be presented upon completion of an AGpO 12 Session Hands-On Course utilizing the Differential Straight-Arch® Technique or Tip-Edge® System. Award qualifications:

1. Minimum completion of 152 CDE hours in orthodontics, to include 4 hours of troubleshooting cases from the course or private patients; along with topics relating to orthodontics.

2. Minimum completion of 3 cases during the hands-on course. Cases can be patients provided in the hands-on program or cases completed from the participant’s practice (case presentation with documentation to the instructor is required).

3. Maintain 2 years of active membership in the Academy while enrolled in the hands-on course, and pass a written and oral examination administered by the course instructor.

4. It is recommended that the participant attend at least one annual meeting, preferably two meetings, during the course of the 12 session program. The first and second year hands-on course participant’s annual meeting registration will be ½ of the regular registration fee. Course participants must register on or before the early registration date or they will pay additional registration fees thereafter. If the registered hands-on course participant does not attend the annual meeting without 10 days prior cancellation notice they will pay full registration fees.

5. Associate Fellow applicants must submit:

a. Application for the AGpO Tier Advancement Program with session verification to Academy Headquarters. No fee is required for the12 Session Hands-On Course Associate Fellowship award. The written examination will be given at the 12th session or at a session designated by the site instructor. The applicant must arrange an appropriate time for the case presentations with the primary site instructor.

Upon successful completion the participant is recognized as an Associate Fellow of the Academy of Gp Orthodontics. This recipient is presented with an Associate Fellow Award at completion of the 12 session course, or

during the Annual Meeting at the Members’ Luncheon and receives a class photo.

Continuing education credits awarded for participation in this CDE activity may not apply towards license renewal in all states. It is the responsibility of each participant to verify the requirements of his/her state licensing board(s). Upon completion of this course, the course participant will be given a letter of CDE verification with ADA codes for each session. Participants should retain this document for their records. It is the responsibility of the participant to submit verification to any regulatory agency.

If you are submitting an application for AGD Fellowship or Mastership, and require CDE hours at the fiscal year end, please submit your request in writing to the Academy Headquarters no later than October 31st to assure your receipt by December 1st of the current year.
