การผาตด Anterior Cervical Spine Surgery

• ไดแก การผาตด ACDF, ACF ในการรกษา cervical disc herniation, cervical

radiculopathy หรอ cervical spondylotic myelopathy (CSM) เปนตน

• Preoperative care

– เตรยม Plain film AP & lateral view, MRI cervical spine

– ใบขอ intraop. X-ray

– ใบขออนมต implant

– เสยบใบ cervical check list เขาไวใน chart โดย fellow หรอ resident spine จะเปนผกรอกเอกสาร

• แพทยจะตดหมอนรอง กระดกสนหลง กระดกงอกออก •ใส bone graft หรอ กระดกเทยมอนๆ เขาไป แทน

•แพทยจะตดหมอนรองกระดกสนหลง กระดกงอก รวมท ง vertebral body ออก •ใส bone graft แลวยดดวย cervical plate Structural iliac bone graft

Postoperative Care การดแลผปวยหลงการผาตด anterior cervical spine

• หากมการดงรง soft tissue มาก แพทยจะให NPO ไวกอน • หากไมสามารถ extubate (off tube) ไดหรอไมแนใจวาหลงผาตดจะมคอบวม

มากจนอาจเกด upper respiratory tract (airway) obstruction

แพทยจะใหน าผปวยเขา ICU

• หากรสกตวด ไมเหนอย สามารถเขา PCT X-rays หลงผาตดทนท • ใส collar ชนดใดนน แลวแตกรณ • หากมการใส implant ให antibiotic ประมาณ 3 วน (แลวแตกรณ)

• หากหลงผาตดผปวยมการเปลยนแปลงทางระบบประสาททแยลง ใหรายงานอาจารยผผาตดทนท

• จะทราบไดอยางไรวา หลงผาตด การท างานของระบบประสาท แยลง ???

ผาแลว แยลง !!! จะรไดอยางไร ???

Manual Motor Testing การตรวจก าลงกลามเน อ (6 Grades)

• Grade 0: ไมมก าลงกลามเนอใดๆ ไมมการเคลอนไหวของกลามเนอหรอเอนกลามเนอทก าลงตรวจนนๆ

• Grade I: ไมสามารถตานแรงโนมถวงได เหนการเคลอนไหวของกลามเนอหรอเอนกลามเนอบาง แตไมเพยงพอทจะขยบขอตอได

• Grade II: ไมสามารถตานแรงโนมถวงได มการเคลอนไหวของกลามเนอหรอเอนกลามเนอทจะขยบขอตอได

• Grade III: ตานแรงโนมถวงได ไมสามารถตานแรงผตรวจไดเลย • Grade IV: ตานแรงโนมถวงได สามารถตานแรงผตรวจไดบาง • Grade V: ก าลงกลามเนอเปนปกต ตานแรงผตรวจไดเตมท

• Maintain skull traction พจารณาเปนรายๆ ไป – หากยงไม off skull traction หลงจากออก OR อาจเปนไปไดวา

• Fixation ไมแนนหนา แขงแรงพอ ตองดงน าหนกตออกสกพก • SOMI brace ยงไมได • ผปวยอาจไมคอยใหความรวมมอ

• หลงผาตด Laminoplasty

– ใส soft collar กเพยงพอ – ให routine postop. care ผปวยสามารถ PT ไดใน PO. Day 3

• SOMI brace หรอใช cervical collar พจารณาเปนรายๆ ไป

• Posterior cervical surgery มโอกาสตดเชอมากกวา anterior

• หากเปน case TB spine ตองให anti-TB drugs กอนการผาตด อยางนอย 2 สปดาห

• หากเปน case tumor ตองเตรยม G/M PRC ปรมาณมากกวาปกต อาจพจารณา preop. embolization หากเปน highly-vascularized

tumor เชน metastatic renal cell & thyroid carcinoma

• หากเปน AIS ตองนดแนะกบผปวยวาอาจตองท า wake-up test

Postoperative Care การดแลผปวยหลงการผาตด posterior lumbar spine

• ควรใชยาแกปวดทเพยงพอ (ถาได PCA, pain doctor จะมา off PCA เอง แพทยผผาตดจะไมสง morphine เพม)

• ให volume or blood replacement ควรสงเกต sign

ของ hypovolemic shock, check Hb./Hct.

• ให IV antibiotics อยางนอย 3 วนกรณทใช spinal
