• เปน special sense organ ททาใหมองเหน ประกอบดวยสวนสาคญหลก ไดแก cornea และ lens ทรวมแสง สงไปยง photoreceptors ทอยบน retina ซงกจะรบ stimuli กคอความเขม (intensity) และส (wavelength) ตาง ๆ ของแสง แลวสงสญญาณผาน optic nerve ไปยงสมอง

• ลกตาของคน แบงเปนชน (layers) ตาง ๆ ดงน• Corneoscleral coat (Fibrous tunic) เปน fibrous layer อยนอกสด ประกอบดวย sclera และ cornea

• Uvea (Vascular tunic) เปน vascular layer อยตรงกลาง ประกอบดวย choroid, ciliary body และ iris

• Retina (Neural tunic) อยในสด ประกอบดวยชน pigment epithelium ดานนอก และชน neural retina ดานใน

• Vitreous space อยระหวาง posterior surface ของ lens และ neural retina ภายในมเจลใส เรยก vitreous humor เปนสารประกอบพวก hyaluronicacid, collagen fibrils, proteins และ glycoproteins

• Anterior chamber อยระหวาง cornea และ iris• Posterior chamber อยระหวาง posterior surface ของ iris และ anterior

surface ของ lensภายใน anterior และ posterior chamber มของเหลวใส เรยก aqueous humor

1. The external Layer: Corneosclera

• Corneaเปนองคประกอบททาหนาท refractive element หลกของลกตา นอกเหนอจาก aqueous และ vitreous humor เปนเหมอนหนาตางบานแรกทแสงเขามา (มคา refractive index ~1.376)Cornea ม 5 ชน ไดแก

• Corneal epithelium เปน stratified squamous epithelium ทม turnover time ประมาณ 7 วน คอเซลลชนลางจะ proliferate ขนมาเปน squamous cells แทนทเซลลชนบนทตายไป เซลลทอยชนบนม 1-2 ชน เปน nonkeratinized cells ทยงม nucleus แตไมม organelles (ไมม metabolic activity) ในชนนจะพบ free nerve endings มากมาย

• Bowman’s membrane เปนชนทมลกษณะ homogeneous ม collagen fibrils บาง ๆ เรยงตวไมเปนระเบยบ ทาหนาทใหความแขงแรง และเปนกาแพงปองกนเชอโรคตาง ๆ ชนนไม regenerate ถาเกดเสยหายอาจทาใหตาบอดได

• Corneal stroma เปนชนทหนาทสด (90% ของความหนา cornea) ประกอบดวยชน (lamellae) บาง ๆ ซอนกนประมาณ 60 ชน แตละชนมมดของ collagen fibrils ทเรยงตวขนานกนไป ซงเชอวาคณสมบตนทาให cornea มลกษณะใส ระหวางชนม fibroblast รปรางแบนแทรกอย ground substance ของชนนเปนพวก proteoglycans และ sulfated glycoaminoglycans

• Descemet’s membrane คอ basal lamina ของ corneal endothelium ทหนาตวมากเปนพเศษ เปนชนท regenerate ไดถาไดรบบาดเจบ เมออายมากขนชนนจะหนา ทปลายรอยตอกบ sclera จะยนออกไปมลกษณะเปนตาขายยดกบ ciliary muscle เรยกวา pectinate ligament เชอวาเปนตวทชวยรกษาให cornea มรปรางโคงนนอยได

• Corneal endothelium เปนเซลลแบนบางชนเดยว ทาหนาทให metabolic exchange ระหวาง cornea กบ aqueous humor ภายในเซลลม mitochondria, vesicles, RER และ Golgi apparatus มาก ม endocytic activity และ active transport ทาหนาทสาคญควบคมการเขาออกของนา เพอรกษาความโปรงใส หากเสยหายจะทาใหเกด swelling และเกดการขนมวขน

• Scleraเปนชนทบแสงทเปน dense connective tissue ประกอบดวย collagen bundles เรยงตวหลายทศทางแตอยในระนาบทขนานกบ sclera surface ระหวาง collagen bundles ม elastic fibers, fibroblasts และ ground substance แทรกในชนนจะพบมเสนเลอดและเสนประสาทมาก รวมทง optic nerve กจะพบทชนน บรเวณรอยตอกบ cornea เรยกวา limbus

Sclera สามารถแบงออกเปน 3 ชน ไดแก • Episclera เปน loose connective tissue ทอยนอกสด• Sclera proper (Tenon’s capsule) คอชนของ dense network ของ collagen bundles

• Lamina fasca เปนชนในสดตดกบชน choroid ประกอบดวย pigment cells, collagen fibers เสนบางๆ และ elastic fibers

2. The middle layer: Uvea (Vascular coat)

• Iris

• Ciliary body

• Lens

• Choroid

• Irisอยดานหนาของ ciliary body ทาหนาทเปน contractile diaphragm อยดานหนา lens เพอควบคมแสงทจะเขาส lensชองวางตรงกลาง เรยกวา pupil (รมานตา)Iris ประกอบดวยชนตาง ๆ ไดแก

• ชนของ fibroblasts & melanocytes

• ชนของ loose connective tissue ทมเสนเลอดขนาดเลกมากมาย• ชนของ smooth muscles 2 ชนด คอ sphincter pupillae (circular) และ dilator pupillae (radial)

• ชน pigmented epithelium มเซลลอย 2 ชน

• Ciliary bodyเปนสวนทยนออกมาจากสวนฐานของ iris มโครงสรางคลาย iris ประกอบดวย loose connective tissue, melanocytes, เสนเลอด และ smooth muscle เรยก ciliary muscle

Ciliary muscle ม 3 functional groups ไดแก Tensor portion, Radial portion และ Circular portion ซงจะทางานรวมกนดงใหเกดการเปลยนรปรางของ lens

Ciliary processes คอสวนทยนออกมาจาก ciliary body คลายนวมอ ม chromatophores และ elastic fibers กระจายตวอย

Ciliary epithelium เปนเซลลรปราง columnar 2 ชน มหนาทหลก 3 อยาง - สราง aqueous humor- เปน blood-aqueous barrier (blood-ocular barrier)- สราง zonula fibers

Zonula fibers จะรวมตวเปน suspensory ligament of the lens

• Lensเปน crystalline, avascular, biconvex structure ทถกแขวนอยกบ ciliary body โดยการยดดวย suspensory ligament (zonule) ถา ligament ถกดงไว lens กจะมลกษณะแบน ถา ligament คลายตว lens กจะมลกษณะโคงนนปองขน เพอทจะปรบ focus ขนกบภาพทรบLens ประกอบดวยสวนตาง ๆ ดงน

• Lens capsule เปน basal lamina หนา สรางมาจาก lens cells• Subcapsular epithelium เปน cuboidal cells ชนเดยว พบเฉพาะ anterior surface ของ lens

• Lens fibers เปน fibers ทสรางมาจาก subcapsular cells

• Choroidเปน pigmented vascular sheet สนาตาลเขม ประกอบดวย loose connective tissue ทม fibroblasts จานวนมาก มเสนเลอดมาเลยงมาก สนาตาลเขมในชนนเกดจาก melanocytes แบงเปน 2 ชน ไดแก

• Choriocapillary layer เปน vascular layer อยดานใน ประกอบดวยเสนเลอดขนาดเลกจานวนมาก ทาหนาทใหอาหารแกชน retina

• Bruch’s membrane เปนชนบาง ๆ ของ amorphous hyaline membrane ทกน choroid กบ retina อาจเรยกอกชอวา lamina vitrea จากการศกษาดวย TEM จะแบงไดเปน 5 ชนทมโครงสรางของเซลลและ fibers แตกตางกน

3. The internal layer: Retina

• Neural retina

คอชนในทม photoreceptors สามารถแบงเปน 2 บรเวณทม function ตางกน ไดแก

• Retinal pigment epithelium

คอชนนอกสดทยดแนนกบชนของ choroid

Layers of the Neural Retina

Neural retina ประกอบดวยชนทงหมด 10 ชน ซงเกดจากการเรยงตวของเซลล และ cell processesเซลลชนดตาง ๆ ทเปนองคประกอบของ neural retina ไดแก

• Photoreceptors ไดแก rod cells และ cone cells• Conducting neurons ไดแก bipolar neurons และ ganglion cells• Association neurons ไดแก horizontal, centrifugal และ amacrine

cells• Supporting cells ไดแก Müller’s cells และ neuroglial cells

Ten Layers of the Retina1. Pigment epithelium

ชนนอยนอกสดและไมไดเปนสวนของ neural retina แตมความเชอมโยงกนทงโครงสรางและหนาท พบวาเซลลในชนนยนยาวขนไปปกคลมสวนของ rod และ cone cell processes

2. Layer of rods and cones เปนชนของ outer และ inner segments ของ photoreceptor cells

3. External (outer) limiting membraneเปนสวน apical ของ Müller’s cells

4. Outer nuclear layer เปนชน cell bodies (nuclei) ของ rod และ cone cells

5. Outer plexiform layerเปนชนของ rod และ cone cell processes รวมทง processes ของ horizontal, amacrine และ bipolar cells

6. Inner nuclear layer

เปนชน cell bodies (nuclei) ของ horizontal, amacrine, bipolar และ Müller’s cells

7. Inner plexiform layer

เปนชนของ processes ของ horizontal, amacrine, bipolar cells และของ ganglion cells ทอยตดกบแตละเซลล

8. Ganglion cell layer

เปนชน cell bodies (nuclei) ของ ganglion cells ซงเปน multipolar neurons ขนาดใหญ

9. Layer of optic nerve fibers

เปนชนของ processes ของ ganglion cells ทนากระแสประสาทจาก retina ไปยงสมอง

10. Internal (inner) limiting membrane

เปนชนของ basal lamina ของ Müller’s cells

Rod Cells

เปนเซลลท sensitive ตอแสงสลว (dim light) สามารถถกกระตนดวยแสงเพยง 1 photon และจะไมสามารถสงสญญาณไดถาแสงสวางมาก มรปรางยาว ประกอบดวย

• Outer segment คอดาน dendrite ภายในม membranous lamellae ซอนกนจานวนมาก ใน membrane นม rhodopsin (light-sensitive pigment)

• Inner segment ม mitochondria และ glycogen granules สรางพลงงานใชใน visual process

• Nuclear region บรเวณทอยของ nucleus• Synaptic region บรเวณทปลอย neurotransmitter ทอย

ใน synaptic vesicles ออกมา synapse กบ bipolar cells

Cone Cells

เปนเซลลทรบแสงจา (bright light) และส (color) ม 3 ชนด ขนกบชนดของ photopigments คอ iodopsin ทแตกตางกน มรปรางอวนคลายรปกรวย ประกอบดวยสวนตาง ๆ คลาย rod cells ไดแก outer segment, inner segment, nuclear region และ synaptic regionชนดของ iodopsin ขนกบ sensitivity ของการรบ color spectrum ทตางกน ไดแก ชนดทรบ red spectrum, ชนดทรบ green spectrum และชนดทรบ blue spectrum

Organ of hearing and balance

ประกอบดวย 3 chambers

• External ear

• Middle ear

(tympanic cavity)

• Inner ear

• หชนนอกจะรบคลนเสยง แลวแปลงเปนแรงสนสะเทอน ซงจะถกขยายสญญาณสนสะเทอนโดยชนกระดกในหสวนกลาง

• หลงจากนนสญญาณจะเขาสหนาตางรปไข (oval window) ถายทอดตอไปในตวกลางทเปนของเหลว (perilymph)ในหชนใน

• ภายในหชนในจะม vestibulocochlear apparatus ประกอบดวย cochlear portion รบแรงสนสะเทอน (เสยง) และ vestibular portion รกษาสมดล สงสญญาณตอไปยง vestibulocochlear nerve (CN VIII)

External Earหชนนอก ประกอบดวย • Auricle (pinna) เปนโครงสราง

ภายนอกทประกอบดวย irregular-shaped plate ของ elastic

cartilage ซงจะตอเนองกบสวนของ cartilage ของหชนถดไป ปกคลมดวยผวหนงบาง ๆ ทยดตดแนนกบตว cartilage

• external auditory meatus คอสวนของทอทยนจาก pinna เขามาใน temporal bone จนถง tympanic membrane ภายในม elastic cartilage ทตอเนองกบ pinna ภายในทอปกคลมดวยผวหนงทม hair follicles, sebaceous gland และ modified sweat glands ชอวา ceruminous glands สราง waxy material เรยกวา cerumen(earwax)

• Tympanic membrane (eardrum) ปดอยทสวนลกทสดของชน external auditory meatus สวนนอกสดของ tympanic membrane ปกคลมทงสองดานดวย epidermis บาง ๆ เปน simple

squamous หรอ simple cuboidal

epithelium ทมาจาก endoderm ตรงกลางเปนชนบาง ๆ ของ collagen fibers, elastic

fibers และ fibroblasts ทมาจาก mesoderm

Tympanic membrane ทาหนาทรบคลนเสยงทมาในอากาศ ผานชองแคบ ๆ ของ external auditory meatus เขามา แลวเปลยนคลนเสยง เปนการสนสะเทอน เปนการเปลยน sound waves ไปเปน mechanical energy จากนนจงสงไปยงหชนกลางตอไป

Middle Ear(Tympanic Cavity)

• Tympanic cavity บดวย simple

squamous epithelium (แตในบรเวณใกล ๆ auditory tube จะเปน psedostratified ciliated columnar epithelium) ชน lamina propria ยดแนนกบ temporal bone ในสวนทเปน cartilage ภายในชนนจะม mucous glands มาก

• เปนชองวาง (air-filled space) อยภายใน temporal bone เปนทอยของ bony ossicles 3 ชน ไดแก the malleus, the incus และ the stapes

• ชวงกลางของผนง tympanic cavity ม membrane ทกนชองเปดของกระดกอย 2 ชอง ไดแก oval window และ round window เปนโครงสรางทเชอม middle ear กบ inner ear

• มทอ auditory tube (eustachian tube) เปดถง pharynx

• ระหวางทมการกลนอาหาร, หาว หรอสงนามก รเปด (orifice) ของ auditory tube ทางดาน pharynx จะเปดออก ทาใหมการปรบความดนอากาศภายใน tympanic cavity และใน external auditory meatus ใหสมดล

• ชนกระดก malleus, incus และ stapes มขอตอทบดวย simple squamous epithelium เชอมกนทง 3 ชน โดยท malleus จะอยนอกสดตดกบ tympanic membrane ถดมาเปน incus และ stapes ซงอยตดกบ oval window ม skeletal muscles 2 มด ชวยในการเคลอนท คอ tensor tympani และ stapedius

• เมอเกด vibrations ท tympanic membrane จะทาใหกระดกทง 3 ชน เกดการเคลอนตว ในลกษณะแบบคานงด (lever action) ทาใหสญญาณสนถกขยายใหไปกระตนท oval window แรงขน

Inner Ear

หชนใน ประกอบดวย 2 สวน•• Bony labyrinthBony labyrinth เปนชองกระดกทฝงตวอยใน

temporal bone ประกอบดวย 3 สวน ไดแก • semicircular canals • vestibule• cochlea

•• Membranous labyrinthMembranous labyrinth อยใน bony labyrinth

Bony LabyrinthBony Labyrinth• ประกอบดวย semicircular canals, vestibule และ

cochlea• แยกกบ membranous labyrinth โดยม

perilymphatic space ซงภายในมของเหลวใส เรยกวา perilymph

• บรเวณตรงกลางของ bony labyrinth เปนบรเวณทเรยกวา vestibule ซงจะม oval window และ round window อย

• Semicircular canals ม 3 วง (superior, posterior และ lateral) ตงฉากกน ภายใน เปน semicircular ducts (membranous) ทโคนของแตละวงมลกษณะปองออก เรยกบรเวณนวา ampulla

• Cochlea เปนสวนทขดเปนกนหอยชขนดานบน ภายในกลวง

Membranous LabyrinthMembranous Labyrinth

• บดวย epithelium ทมาจาก embryonic ectoderm แทรกตวเขามาอยใน temporal bone เกดเปนทอ semicircular ducts, เปนทอ cochlear duct และเปนถงเลก ๆ 2 ถง คอ sacule และ utricle

• ภายในมของเหลว เรยกวา endolymph เปน viscous fluid ทมองคประกอบของ ion คลาย extracellular fluid (Na ตา, K สง)

Saccule and Utricle

• Saccule และ utricle เชอมตอกนดวยทอเลก ๆ (ductus utriculosacularis) ทอนจะไปรวมกนเปน endolymphatic ducts ยนขนไปสดท endolymphatic sac และมทอเลกอกทอ (ductus reuniens) เชอมระหวาง saccule กบ cochlea

• ม specialized regions ทเปน receptors สาหรบรบ orientation ของศรษะทสมพนธกบ gravity และ acceleration มชอเรยกวา macula of the saccule และ macula of

the utricle ซงตางกตงอยในตาแหนงทไดฉากกน• Macula of saccule จะอยทผนงดานขาง ทาหนาทรบ linear vertical

acceleration• Macula of utricle อยทพน ทาหนาทรบ linear horizontal acceleration

• Maculae (pl.) กคอ epithelium ทหนาตวขน ขนาด 2-3 mm ประกอบดวย neuroepithelial cells 2 ชนด ไดแก type I และ type II

hair cells นอกนนกเปน supporting cells ขางใตรองรบดวย basal lamina และม nerve endings จาก vestibulocochlearnerve มาท hair cells

• Cilia ของ hair cells ถกปกคลมดวย gelatinous glycoprotein เรยกวา otolithic

membrane บน surface ม calcium carbonate crystals ขนาดเลก เรยกวา otoliths จานวนมาก

• ในแตละทอ ตรงโคนมลกษณะปองขน เรยกวา ampulla ซงภายในม specialized receptors อย ทาหนาทรบ linear และ angular movement

• Specialized receptors ภายใน ampulla มชอวา cristae ampullares มโครงสรางเปนสนนนขน (ridge) บดวย sensory epithelium ทประกอบดวย neuroepithelial hair cells และ supporting cells ทอยบน basal lamina เหนอ epithelial surface กม gelatinous glycoprotein ปกคลม เรยกวา cupula

Cochlear Duct

• ภายในม membranes แบงเกดเปนชอง 3 ชอง ตรงกลางคอ scala media

(cochlear duct) ซงมดานหนงกนดวย vestibular membrane อกดานกนดวย basilar membrane ชองทอยเหนอ vestibular membrane คอ scala

vestibuli สวนชองทอยใต basilar membrane คอ scala tympani ตรงยอดสดของ cochlea เปนบรเวณททง 2 ชองนเชอมกน เรยกวา helicotremahelicotrema

• บน basilar membrane ภายใน cochlear duct (scala media) มโครงสรางทเรยกวา organ of Corti ทง cochlear duct และ organ of Corti นทาหนาทในกระบวนการไดยน

ตางกมลกษณะเหมอนกนตรงท apical surface ของเซลลขนมาถง free surface ดานบน และใน cytoplasm ประกอบดวย microtubules และ microfilaments

• ในสวนทแคบทสดของ cochlear duct มโครงสรางยนออกมาจากสวนทอยของ acoustic nerve (spiral lamina) ภายในม interdental cells ซงสราง tectorial membrane ออกมา มลกษณะเปน proteoglycan-rich gelatin ทม keratin-like filaments อยดวย tectorial membrane นปกคลมทบอยบน stereocilia ของ hair cells ของ organ of Corti
