KUESIONER
PETUNJUK PENGISIAN :
1. Kuesioner ini ditunjukkan hanya kepada supervisor dan staff departemen
keuangan dan akuntansi perusahaan yang dalam melaksanakan pekerjaannya di
perusahaan Bapak/Ibu sudah biasa/dapat/mampu/familier dalam menggunakan sistem
informasi berbasis komputer.
2. Jawaban yang disampaikan kepada peneliti merupakan suatu perwujudan dari
intensitas perusahaan atas penggunaan sistem informasi berbasis komputer, sehingga
untuk mendapatkan hasil penelitian yang maksimal, diharapkan Bapak/Ibu dapat
menjawab pertanyaan dengan benar dan jujur.
3. Apabila perusahaan tidak menggunakan sistem informasi yang berbasis komputer
dalam operasional bisnis, Bapak/Ibu tidak perlu merespon kuesioner ini.
