Laboratory of Health Care Associated Infection • The Infection Control Unit at Colindale is able to provide the following

Laboratory of HealthCare Associated Infection • Reference and Diagnostic Testing Services • Epidemiological Typing Unit (ETU)
• LHCAI's Reference and Diagnostic Services are provided by the
Epidemiological Typing Unit (ETU). ETU is the national centre for the phenotypic and molecular epidemiological type identification of bacterial species associated with infections acquired in hospitals. The main services provided are typing of Acinetobacter spp., Burkholderia spp., Enterobacter spp., Enterococcus spp., Klebsiella spp., Pseudomonas spp., Serratia spp. and Staphylococcus spp., and Stenotrophomonas maltophilia. The Unit also undertakes the molecular comparison of epidemiological linked isolates of other gram negative and gram positive species implicated in hospital outbreaks.
• The Unit is comprised of two typing sections, Staphylococcus Reference and Opportunist Pathogens. Both sections provide specialist reference functions, give advice on microbiological aspects of outbreak investigations and monitor national trends of distribution of strains.
• Serodiagnosis Section offering serodiagnostic tests for staphylococcal and streptococcal infections as well as for P. aeruginosa and B. pseudomallei (melioidosis).
• In the year ending March 2002 reports were issued on c. 22,500 specimens.

ทมาของโครงการ การใชนายาทาลายเชอเชดบนพนผวของสงแวดลอมรอบตวผปวยทตรวจพบเชอดอยาเปนมาตรการทสาคญในการลดการแพรกระจายของเชอดงกลาวในโรงพยาบาล เนองจากเชอแบคทเรย
กอโรคทปนเปอนในหอผปวยมหลายชนด และสภาพแวดลอมบรเวณดงกลาวประกอบดวยพนผวหลายประเภท เชน ผา, กระจก, โลหะ, ไม, ฯลฯ ประสทธภาพในการทาลายเชอของนายาแตละชนดอาจมความแตกตางกน
วธด าเนนการ 1. คณะผวจยไดตรวจสอบคณสมบตในการท าลายเชอดวยน ายา 6 วธ เพอลดปรมาณเชอกอโรค 4 ชนด ซงถกท าใหปนเปอนบนพนผวของวสด 4 ประเภท ไดแก ผา กระจก โลหะ และไม ในหองปฏบตการ รวมทงสน 96 ครง
2. เชอทใชทดสอบ เปนเชอกอโรค 4 ชนดซงแยกไดจากผปวยในโรงพยาบาลศรราช ไดแก Acinetobacter baumannii , Pseudomonas aeruginosa , Vancomycin- resistant Enterococus faecium (VRE), และ Methicillin- resistant Staphylococcus aureus (MRSA) เชอแตละชนดจะถกท าเปนสารละลายทมความเขมขนของเชอประมาณ 107
CFU/ml 3. วสดแตละประเภทจะถกท าใหปนเปอนเชอครงละ 1 ชนดอยางเทาเทยมกน ดวยการใชไมพนส าลปลอดเชอ ปายสารละลายเชอทเตรยมไวแลวจ านวน 1 มลลลตร ทาบนพนผวของวสดทศกษาขนาด 2 ตารางนวใหทว แลวปลอยใหแหงประมาณ 5 นาท หลงจากนนจงเชดท าความสะอาดพนผวดวยน ายาท าลายเชอตอไป

วตถประสงค 1. ศกษาประสทธภาพของ การท าลายเชอดวยน ายา 6 วธ ในการลดปรมาณเชอกอโรคบนพนผวของวสดประเภทตางๆ 2.ความรทไดจากการศกษาน จะน าไปสการก าหนดแนวทางทเหมาะสมเพอเลอกใชวธท าความสะอาดวสดตางๆทอาจปนเปอนเชอดอยาในหอผปวย

วธด าเนนการ
วธ A: เชดดวยกระดาษส าเรจรปทมสวนประกอบของ Nonyl phenol 8 mole ethoxylate 0.2%, Alkyl dimethyl benzyl ammonium chloride 0.7%, Didecyldimethyl benzyl ammonium chloride 0.7%, และ Polyhexamethylene biguanide hydrochloride 0.4% จนแหง
วธ B: พนน ายาทมสวนประกอบของ Nonyl phenol 8 mole ethoxylate 0.1%, Alkyl
dimethyl benzyl ammonium chloride 0.5%, Didecyldimethyl benzyl
ammonium chloride 0.5%, และ Polyhexamethylene biguanide hydrochloride 0.2%ใหเปยก แลวเชดดวยผาปลอดเชอจนแหง
วธ C: เชดดวยกระดาษส าเรจรปทมน ายาทมสวนประกอบของ Propan-1-ol 50%, Didecylmethyl ammonium chloride 0.00075% จนแหง
วธ D: พนน ายาทมสวนประกอบของ Propan-1-ol 50%, Didecylmethyl
ammonium chloride 0.00075% ใหเปยก แลวเชดดวยผาปลอดเชอจนแหง วธ E: หยด Ethyl alcohol 70% 1 มลลลตร บนพนผววสด แลวเชดดวยผาปลอดเชอจนแหง
วธ F: หยดสารละลาย Virkon 0.5% (1:200) 1 มลลตร บนพนผววสด แลวเชดดวยผาปลอดเชอจนแหง
5. หลงจากนน ผวจยจะใชไมพนส าลปลอดเชอ ปายพนผวบรเวณดงกลาวใหทว แลวน าไปวเคราะหหาปรมาณเชอทเหลออยตอไป

ผลการศกษา ปรมาณเชอทเหลออยบนพนผววสดแตละประเภท หลงจากทาความสะอาดดวยนายา ดงในกราฟแทงเชงซอน
ประสทธภาพในการทาลายแบคทเรยกอโรคของนายาฆาเชอบนพนผวชนดตางๆ
สรปผลการศกษา •70% Ethyl alcohol (วธ E) และสารละลาย 0.5% Virkon (วธ F) มประสทธภาพในการท าลายเชอบนพนผวของวสดทงสชนดไดมากสด สามารถลดปรมาณเชอทปนเปอนบนพนผวของวสดทง 4 ประเภท คอ ผา กระจก โลหะ และไม จาก 107 CFU/ml เหลอนอยกวา 101 CFU/ml ซงเปนปรมาณต าทสดทตรวจวดได
•กระดาษส าเรจรปทมสวนประกอบของ Quaternary ammonium compound (วธ A) มประสทธภาพในการท าลายเชอบนพนผววสดทง 4 ประเภทไดดพอสมควร นาจะเปนทางเลอกทเหมาะสมในการท าลายเชอแบคทเรยดอยาบนพนผวทไมสามารถใช alcohol หรอ Virkonได
•น ายา หรอ กระดาษส าเรจรปทมสวนประกอบของน ายาฆาเชอ 50% Propan-1-ol เปนหลก มประสทธภาพในการท าลายเชอแบคทเรยบนพนผวประเภทผาและกระจก ไดคอนขางจ ากด

การศกษาการปนเปอนของนายาทาลายเชอสาหรบลางมอและภาชนะบรรจทใชในโรงพยาบาลศรราช
• การใชน ายาท าลายเชอส าหรบลางมอใน รพ.ศรราช 4% Chlorhexidine และ 7.5% Povidone iodine เทแบงจากภาชนะบรรจเรมตนจากหนวยผลต ลงในภาชนะพรอมใช • การเตรยมภาชนะพรอมใชส าหรบเทแบงน ายาท าลายเชอแตละหนวยงานมความแตกตางเชน การลางท าความสะอาดผงใหแหง การลางท าความสะอาดไมผงแหง การอบแกส เปนตน • ภายหลงการเทแบงแลวจะเปลยนน ายาและภาชนะบรรจทก 3 วน • น ายาเหลอและถกทงจ านวนมากหลงใชครบ 3 วน • เพมภาระงานบคลากรในการท าความสะอาดและท าลายเชอ
เพอลดความสญเสยทเกดจากการปฏบตน จงมการทบทวนถงความจ าเปนของการเปลยนน ายาท าลายเชอ
แตปจจบนยงไมมหลกฐานการวจยทบงชวา ควรเปลยนน ายาท าลายเชอเมอไหรภายหลงการเทแบงใชงาน

การศกษาการปนเปอนของนายาทาลายเชอสาหรบลางมอและภาชนะบรรจทใชในโรงพยาบาลศรราช
1.ศกษาการปนเปอนของนายาทาลายเชอสาหรบลางมอ หลงเทแบงใสในภาชนะบรรจพรอมใชในของหนวยงานในโรงพยาบาลศรราช คอ 4% Chlorhexidine และ 7.5% Povidone iodine 2. ศกษาการปนเปอนของภาชนะใสนายาทาลายเชอหรอสาหรบลางมอในหอผปวยและในหองผาตด 3. นาขอมลไปปรบเปลยนวธปฏบตเกยวกบนายาทาลายเชอสาหรบลางมอ เชน การทาความสะอาดภาชนะบรรจ, ระยะเวลาทเหมาะสมในการเปลยนนายาภายหลงการเทแบงมาใชงาน
1. สมหอผปวยเพอเกบตวอยางสงตรวจ 13 หอผปวย และหองผาตด 13 หอง 2. เกบขอมลวธการทาความสะอาดหรอการทาใหปราศจากเชอ ของภาชนะบรรจนายาทาลายเชอสาหรบลางมอ 3. ทดสอบการปนเปอนของแบคทเรยในนายาทาลายเชอและภาชนะบรรจโดย 3.1 Swab ดานในของภาชนะบรรจนายาททาความสะอาดหรอทาใหปราศจากเชอแลว หนวยงานละ 1-5 ขวด ตามจดทใชงาน 3.2 เกบนายาทาลายเชอจากภาชนะบรรจของหนวยงานทศกษา สงเพาะเชอในวนแรกทเทนายาใชและวนท 3 หลงการใชงานตามปกต หลงจากนนเปดฝาของภาชนะบรรจออกแตยงตงไวใกลเคยงบรเวณเดม แลวเกบตวอยางสงเพาะเชอในวนท 7, 14, 21 และ 28 ตามลาดบ3

การดาเนนงาน
การศกษาการปนเปอนของนายาทาลายเชอสาหรบลางมอและภาชนะบรรจทใชในโรงพยาบาลศรราช
1 . การปนเป อนของแบคท เรยในภาชนะทบรรจน ายาท าลายเชอ พบวาการท าความสะอาดภาชนะบรรจแลวไมท าใหแหงกอนเตมน ายาท าลายเชอจะพบเชอแบคทเรยกอโรคได 50% ในขณะทภาชนะทท าความสะอาดแลวผงใหแหงจะพบแบคทเรยกอโรคไดเพยง 21.7 % สวนภาชนะทบรรจทท าใหปราศจากเชอกอนใชงานจะไมพบแบคทเรยกอโรค
น า ย า 4% Chlorhexidine แ ล ะ 7.5% Povidone iodine สามารถใชตอเนองไดนาน 14 วน 2. ภาชนะทจะน ามาใชบรรจน า ย า 4% Chlorhexidine แ ล ะ 7.5% Povidone iodine ควรเปนภาชนะทไดรบการท าใหปราศจากเชอ กรณใชในหองผาตด หรอเปนภาชนะทลางใหสะอาด และผงใหแหงกอนน ามาบรรจน ายาในหอผปวยทวไป
ผลการศกษา
2. น ายาท าลายเชอส าหรบลางมอ 4% Chlorhexidine และ 7.5% Povidone iodine ทใชในหอผปวยและหองผาตด ตงแตวนแรกทเทแบงใชจนถงวนท 28 ไมพบการปนเปอนในทกตวอยาง

Results • The study supports the evidence that the hands of surgeons
and scrub nurses carry microorganisms identified as sources of microbial contamination. Common organisms including
coagulase-negative staphylococci (CNS), methicillin- sensitive Staphylococcus aureus (MSSA),
methicillin- resistant Staphylococcus aureus (MRSA), A. baumannii, P. aeruginosa, and Enterobacter species These common organisms may cause surgical site infections.
S. aureus and CNS are the leading causes of surgical site infections.

Recommendations from CDC and (HICPAC) An aggressive infection control strategy, using Contact precautions Implementing CLSI guidelines for CRE detection In areas where CRE are not endemic:- 1) Review microbiology records preceding 6-12 months
to determine whether CRE have been recovered at the facility.
2) If the review finds previously unrecognized CRE,
perform a point prevalence culture survey in high-risk units to look for other cases of CRE.
3) Active surveillance cultures of patients with
epidemiologic links to persons from whom CRE have been recovered.
